Technical and instrumental requirements of arthroscopy of the knee joint.

The use of the endoscope for the inspection of the interior of the knee joint makes high demands on OR hygiene, which is a general prerequisite for the examination. Knee joint arthroscopy can be carried out either under general or local anaesthesia; the advantages and disadvantages of the two procedure are compared. Various optical systems are in common use, the best quality images both for the examination itself and for photographic documentation, area obtained when the interior of the joint, distended by a gaseous medium, is inspected using a lens-type optical system.

[Benign solitary enchondroma (author's transl)].

Solitary enchondroma is a benign bone tumor with a doubtful prognosis changing by topical but not by clinical, roentgenological or histological characteristics. Therefore benign solitary enchondroma should be subdivided in such of peripheral and such of central localisation. Both forms of enchondroma normally show an asymptomatic development. Diagnostic problems occur in central enchondroma because of its radiological, angiographical and szintigraphical aspects. Peripheral enchondroma should be treated by local excision. Recurrences after local treatment are extremely rare. In treatment of central enchondroma bone resection should be performed following common principles of onkological radicality. Local excision is not sufficient; amputation should be reserved for treatment of chondroma close to the trunk or chondrosarcoma.

Artificial organ support strategies in combined renal, circulatory, pulmonary and liver failure.

Within recent years we have significantly broadened the indication for the treatment of acute renal failure in intensive care patients, even in the presence of multiple organ failure. This was made possible by: 1. safe heparinization of the extracorporeal circulation while avoiding bleeding tendencies 2. hemofiltration of patients threatened by pulmonary complications early during renal failure and 3. the simultaneous use of various special intensive care measures. By this approach we have succeeded in a number of cases to break the chain of complications leading to multiple organ failure and death.

[About the plurality of the manifestations of cytomegalovirus infection in adult (author's transl)].

Three various cases of cytomegalovirus infection will be described. At first will be represented a double infection of cytomegalovirus and toxoplasmosis. In the second case it's about the organmanifestation of cytomegalovirus of the liver as hepatitis. At last will be discussed a severe infection of the central nervous system on the one hand in form of Guillain-Barré-syndrome, on the other hand manifestated as encephalitis.

Bilateral pituitary adenomas occurring with multiple endocrine neoplasia type one.

We report a case of synchronous bilateral pituitary adenomas in a patient with multiple endocrine neoplasia type one (MEN1). The patient was previously known to have a pancreatic gastrinoma and had first-degree relatives with MEN1. Both adenomas were concurrently revealed by high-resolution MR imaging of the pituitary gland as part of the investigation of the patient's severe, persistent headaches and elevated serum prolactin level.
