2,3-Diphosphoglycerate: its role in health and disease.

2,3-Diphosphoglycerate (2,3-DPG) was first discovered and isolated in 1925. However, it was not until 1967 that the function of 2,3-DPG was explained. This resulted in multiple research projects devoted to elucidating the mechanism by which 2,3-DPG exerts it effect on the oxygen affinity of hemoglobin. In addition, a vast amount of research has been devoted to assessing the role of 2,3-DPG in oxygen transport in various physiological and pathophysiological states. In many instances, the results of this research have produced conflicting data which have dampened the initial enthusiasm which followed the discovery of the function of 2,3-DPG. However, much of this conflicting data can be explained by the fact that 2,3-DPG is only one of a number of factors influencing the transport of oxygen to the tissues. Several of these factors influence oxygen transport independently as well as by altering the synthesis of 2,3-DPG and modifying its effect on hemoglobin. In spite of the conflicting results, the overall data gathered thus far appears to be sound enough to warrant the extensive research now being done, particularly in the area of blood storage and transfusion therapy.

The 1978 College of American Pathologists Therapeutic Drug Monitoring Interlaboratory Survey Program.

In 1978 the College of American Pathologists introduced a Therapeutic Drug Monitoring Interlaborabory Survey Program. Each participant recieved six vials of lyophilized human serum on two occasions, approximately 13 weeks apart. The specimens contained various combinations of 12 drugs in subtherapeutic, therapeutic, and toxic concentrations. The drugs included in the specimens were phenytoin, phenobarbital, primidone, ethosuximide, carbamazepine, digoxin, procainamide, N-acetylprocainamide, quinidine, theophylline, lithium, and gentamicin. The weighed-in quantity of each drug was used as the target value for that drug. The specimens were analyzed by a variety of analytic methods. Statistics were calculated for all results regardless of the analytic method used and for each individual method. This report presents the data from the Program.

Serum osmolality. A CAP survey analysis.

Serum osmolality has been included as a constituent to the comprehensive Chemistry Series of the College of American Pathologists Survey Program since 1973. The data for 1975 are reviewed in depth and compared with those for the two previous years. The results show that the interlaboratory variation of osmolality analysis is minor and concurrence has shown progressive improvement each year. The mean concentrations of duplicate pairs of specimens circulated to participants in 1975 show very close agreement except for one pair of duplicates for those laboratories using the Advanced Instruments and Fiske osmometers. The source of this difference is unexplained. The mean concentrations achieved by the participants using the various osmometers were compared with the "all-methods" mean. The bias is greatest for those laboratories using the Fiske and Wescor instruments. The number of laboratories submitting osmolality data has increased each year. There has been some change in the relative popularities of the various osmometers used by the participants.
