Treatment of metastatic colorectal adenocarcinoma with fluorouracil and high-dose leucovorin: a pilot study.

In Veterans General Hospital-Taipei, 25 patients with metastatic or recurrent colorectal adenocarcinoma were treated with a 5-day course of 5-FU (370 mg/M2/d) and high dose leucovorin (200mg/M2/d), repeated every 28 days until a progression of disease was evident. Twenty-one patients had evaluate tumor response. Among the 16 patients with measurable disease, three patients achieved partial response (PR), and their response durations were 2 months, 6 months and 6+ months. Five patients had stable disease (SD) for 4,4,6,7, and 10 months before its progression. Among the 5 patients with documented abdominal carcinomatosis, four remained stable without any progression for 5,6,12 and 12 months after treatment. The overall response rate was 57% (3PRS, 9SDs). Seven of the 12 responders had prior 5-FU exposure. Nine patients had progressive disease and 7 of them died within a few months after the start of treatment. The median duration of response was 6.7 months. The median survival for responders was 9.8 months, and for non-responders, 3.7 months. Performance status (PS) was a main determinant of response. Among the patients with PS = 0 (6 patients), there were 2 PRs and 4 SDs and with PS = 1 (8 patients), 1PR and 4 SDs. When PS = 2 or 3 (7 patients), only 2 SDs were noted. All the 25 patients were put into toxicity test to find in a mild to moderate, easily controlled state. There were 3 (12%) patients with grade 2-3 mucositis, 6 (24%) with grade 2 diarrhea, 13 (50%) with nausea and vomiting. Only 3 cases developed leucopenia but without thrombocytopenia.(ABSTRACT TRUNCATED AT 250 WORDS)

Bone marrow cryopreservation and clinical implications in autologous bone marrow transplantation.

A simple, rapid and effective technique using the IBM (Cobe)-2991 cell processor for the concentration of buffy coat cells from large volume marrow has been well adopted (n = 16). Only about one-eighth of the original volume was obtained while retaining more than 90% of the total nucleated cells to be cryopreserved in polyolefine bags with TC-199 culture medium and final 10% dimethylsulfoxide (DMSO) (n = 9), processed by a computerized Nicool ST-20 (France) programmed freezer and stored in a vapor phase of liquid nitrogen at -196 degrees C. Stem cell assay by CFU-GM after thawing yielded a mean of 50.39 +/- 19.54% which has been satisfactory for clinical implementation. So far, three cases with hematological malignancies had been rescued by autologous cryopreserved marrow after supralethal doses of chemoradiotherapy. Two patients with acute nonlymphocytic leukemia transplanted in 1st remission as of Oct. 31 had been disease free for 178+ and 157+ days, respectively, after transplant which was taken at the corresponding age of 53 and 42 years. The other patient who was a victim of Hodgkin's disease, stage IV, and was transplanted in 3rd remission, expired on the 59th day because of the complication of idiopathic interstitial pneumonitis despite excellent granulocytopoietic reconstitution. The preliminary results are encouraging for further exploitation, especially for those who would otherwise be candidates for allogeneic bone marrow transplantation but are limited by age or lack of an HLA-identical sibling to serve as marrow donors.

International programs of the National Library of Medicine.

The National Library of Medicine (NLM) has a broad mission in biomedical information service. There are three major reasons for NLM, as a national institution, having an international program: first, the global nature of disease; second, the international scope of medical literature; and third, the universal goal of better communication. This paper reviews NLM's programs in relation to international medical information exchange: International MEDLARS Centers, collaboration with WHO and PAHO, NLM Special Foreign Currency Program, and development of the NLM collection.

Weekly CAF chemotherapy for advanced breast cancer patients.

In a prospective phase II study, 102 women with advanced breast cancer were treated with low doses of cyclophosphamide, Adriamycin and 5-fluorouracil (CAF) at weekly intervals by intravenous injection. Seventy-five patients were evaluable for treatment response and the overall response rate was 52% (95% confidence interval, 41-63%). Of the evaluable patients, 15% had complete response and 37% had partial response. The median survival after therapy was 15.6 months, the median time to progression was 6.8 months and the median duration of response was 9.1 months. The main toxicities were mild vomiting and moderate myelosuppression. There was only 1 patient who experienced heart failure. Weekly CAF appears to have an efficacy with tolerable side effects comparable to standard CAF with an every-3-week schedule.

Automated multiphasic health testing. Diagnostic and testing results obtained at the Health Evaluation Center. Public Health Service Hospital, Baltimore.

The results of automated multiphasic health testing (AMHT) were evaluated with special attention to diagnoses made by physicians and to false positive results, as well as to laboratory test results. The study population consisted of 1,157 patients at the Health Evaluation Center of the Public Health Service Hospital in Baltimore. Although 95 percent of the patients had at least one newly diagnosed disease or condition., the percentage dropped to 78 percent when dental abnormalities were excluded and to 70 percent when dental, vision, and hearing abnormalities were excluded. Abnormal laboratory test results were observed for 98 percent of the patients, and 36 percent had at least one false positive test result. The study results indicated that AMHT is a highly productive method for comprehensive medical testing with a variety of uses other than mass screening. The productive diagnostic yield combined witha high percentage of false positive results dictate the need for careful planning for followup care, strict attention to quality control, and excellent communication between the AMHT center and the practicing physician.
