[Verbal and written enlightenment of the eye patients: special problems in Switzerland ].

In Switzerland, as in most other western European countries, the patient-physician relationship is based on the mandate relationship in private law. Its main provisions are laid down in the Swiss civil code, the law of obligations and the penal code. The physician is obliged at all costs to fulfill his responsibility to enlighten the patient. The authors discuss the extent and nature of this duty to provide information, in particular regarding the patient's right of access to medical and laboratory reports.

[Extracapsular cataract extraction combined with the implantation of an intraocular lens (author's transl)].

Between 1973 and 1980 349 intraocular lenses were implanted after intracapsular and 470 after extracapsular cataract operation. The results and complications are evaluated: following intracapsular lens extraction a 4-loop Binkhorst lens (iris clip lens) was inserted in all cases. Following extracapsular cataract operation a 2-loop Binkhorst lens (iridocapsular lens) was implanted in 194, a 4-loop lens in 276 cases. The two complications occurring most frequently were an Irvine-Gass syndrome in 10% following intracapsular, and a disturbing secondary cataract necessitating a discision in 19% after extracapsular lens extraction. A dislocation-decentration of the pseudophakos was observed in some cases after extracapsular cataract operation. It is therefore recommended that the extracapsular be preferred to the intracapsular cataract extraction, that a 4-loop lens be implanted, secured by a transiridectomy suture, but that the haptic part of the intraocular lens should not be inserted in the capsular bag. It is of paramount importance, especially in young pseudophakic patients, to prescribe carefully an additional optical correction, preferably progressive glasses.

Effects of cortisone and thyroxine on suckling rat liver N-acetyl-beta-glucosaminidase isozymes. Comparison with adult reactivity.

Suckling rat liver N-acetyl-beta-glucosaminidase (hexosaminidase) activity undergoes considerable fluctuation during the first two weeks of life. As two major forms of hexosaminidase (A, heat-labile, and B, heat-stable) are known to exist in both human and adult rat liver, we choose to examine the effect of the maturative hormones, thyroxine and cortisone, upon these isozymes during the suckling period. Between days 7 and 15, the observed developmental change is attributable solely to an increase in the 'A-like' (heat-labile) form of the enzyme; an enhanced response is seen in thyroxine-injected 11-15-day old animals. The response may be considered 'age-independent', as adult animals react in the same manner. In contrast, cortisone-injected sucklings show a decrease in both A and B isozymes, while in adults no changes in total activity or isozyme distribution are evoked. The ratio of hexosaminidase A to hexosaminidase B in suckling rat liver appears to shift in favor of the labile (A) isozyme early in development.

Prognostic significance of cytological differentiation grading in estrogen-treated prostatic carcinoma diagnosed by fine-needle aspiration biopsy. (Five-year follow-up of 496 patients).

The value of fine-needle aspiration biopsy of carcinoma of the prostate and its cytological grading is demonstrated. The grade of morphological differentiation in cytological smears has proved to be a reliable criterion for the prognosis in 496 estrogen-treated prostatic carcinoma patients. The life expectancy of low-differentiated and anaplastic cancer patients undergoing treatment is so poor that the justification for antiandrogen treatment in this group of carcinomas appears extremely doubtful. For this reason, the degree of differentiation of the tumor should be considered when undertaking therapeutic measures in view of its relevance for the prognosis.

[Macular edema following implantation of Binkhorst lenses].

The incidence of cystoid mascular edema following intracapsular or extracapsular cataract extraction with implantation of a Binkhorst 4-loop lens is reported. The results, showing an incidence of macular edema of 13% after intracapsular operation and 2.1% after extracapsular operation, correlate well with reports published elsewhere. Most of the cases were recognized at the earliest three months after operation. A correlation with intraocular pressure was found. Examples of fluoresceinangiographic findings are presented.

[Differential diagnosis of hereditary optic atrophy (author's transl)].

The problem of differential diagnosis is discussed with regard to Leber's optic atrophy and dominant optic atrophy occurring in childhood. The discussion is based on case reports on four members of the same family who were affected at very different ages and in whom the disease developed differently. Various symptoms are compared with data already published. It is impossible to classify the members of this family according to any particular group on the basis of these data.

Effect of lidoflazine on membrane currents and contraction in voltage-clamped frog atrial fibers.

The effect of lidoflazine on action potential, membrane currents and contraction of frog atrial fibers was tested using the double sucrose voltage clamp technique. Lidoflazine was found to decrease the sodium conductivity of the heart cell membrane, probably by blocking the sodium channels. The availability of the sodium system at resting potential was slightly enlarged by lidoflazine and the recovery from inactivation was prolonged in most of the preparations tested. A small decrease of the slow inward current and a reduction of phasic and tonic tension was observed. The outward current at higher depolarizations was increased by lidoflazine resulting in a shortened action potential duration. The data suggest that lidoflazine's antifibrillatory properties are less pronounced than those of classical antiarrhythmic agents; but the slight antifibrillatory and negative inotropic effect might be helpful in the treatment of angina pectoris.

[On the history of intraocular lenses (author's transl)].

A short review on the world-wide and Swiss history of the three generations of intraocular lenses is recorded: the implantation of posterior and anterior chamber lenses is completely given up in Switzerland. Prepupillary 2- and 4-loop-lenses of the Binkhorst type are used at the Lucerne Eye Hospital since 1973. A major brakethrough of artificial lens'implantation in Switzerland is expected in 1979.

[A review of 400 intraocular lens implants (author's transl)].

400 intraocular lenses (iris-clip and iridocapsular pseudophakoi of the Binkhorst type) were implanted at the Lucerne Eye Hospital between 1973 and 1978. The present review emphasizes the paramount position of the ophthalmosurgeon among the complicating factors. The 2-loop metallic iridocapsular lenses were abandonned; secured iris-clip lenses are now after intra- and extra-capsular lens extraction. Aspiration of lens masses or needling of a secundary cataract were necessary in 27% of extracapsular lens extraction. Precipitates on the psuedophakoi were absent after intrakapsular lens extraction, indicating the uveitis being of phacogenic, not pseudophacogenic origin.

[Special indications, contraindications, particular methods of treatment in correction with intraocular lenses (author's transl)].

For the past 5 3/4 years intraocular lens implantion belongs to the routine operations performed in the Lucerne Eye Clinic. Whereas unilateral cataracts are the main and frequent indication for intraocular lens implants, there are certain cases of uni- and bilateral cataracts also, where the implantation of intraocular lens, due to its advantages, is specially indicated. Such cases are presented briefly in this paper. Good results are achieved through strictly observing the methods of treatment before and after the implantation of the lens.

[General guidelines for the implantation of prepupillary lenses. (author's transl)].

The general guidelines for the implantation of prepupillary lenses concern the patient (right of impartial orientation, right of decision with regard to the nature of the future aphakic prosthesis), the ophthalmologist (duty of relevant orientation, duty to accept the patient's decision) and the ophthalmo-surgeon (right to reject an implantation). Following a thorough training in implant surgery general conditions, such as technical and personnel means have carefully to be estimated before starting with the implantation of intraocular lenses.

Influence of the axis on the enzymes of protein and amide metabolism in the cotyledons of mung bean seedlings.

The growth of the mung bean (Vigna radiata) seedling is accompanied by the biosynthesis and accumulation of the endopeptidase vicilin peptidohydrolase and the catabolism of the reserve proteins in the cotyledons. If the axis is removed from the dry seeds and the cotyledons incubated on moist sand the accumulation of vicilin peptidohydrolase is reduced by 77% and the catabolism of reserve proteins slowed to 25% of the rate in intact seedlings. The cotyledons and the cotyledon exudate are rich in asparagine and this amino acid accounts for more than half of the reduced nitrogen exported from the cotyledons. Glutamine synthetase and asparagine synthetase, two key enzymes in the pathway of asparagine synthesis, are under temporal control in the cotyledons. Their activities increase 3.5- and 10-fold, respectively, then decline again. These increases in enzyme activity occur to the same extent in excised cotyledons and are prevented when the cotyledons are incubated in 5 micromolar cycloheximide. The results indicate that the axis may control certain key metabolic events in the cotyledons, such as the synthesis of vicilin peptidohydrolase, while many other anabolic activities may not depend on a growing axis.

[On the correction of posttraumatic unilateral aphakia in children and in adults with artificial intraocular lens implants (author's transl)].

The problems of optical correction of unilateral traumatic aphakia are discussed. The treatment of choice is a combined Cataract-operation with intraocular lens implants. The operation must be done as soon as the eye is free of inflammation. Prescription of bifocal glasses in addition to pseudophakoi is necessary. With this method a "malignant" Strabismus can be avoided and binocular vision, stereopsis as well as minimal grade of aniseikonia can be achieved.
