[Evaluation of fetal maturity using various components of amniotic fluid].

It is reported on the lecithin-sphingomyelin-ratio, the total phospholipids, creatinine, glucose and content of orange cells in the liquor amnii in dependence on the age of pregnancy. All methods are criticized in their value for the diagnosis of fetal maturity. Basing on the results of this report is proposed a table of points of maturity like the model of womens hospital at the University of Halle (GDR). This table enables the shure judgement of the actual fetal maturity from the amniotic fluid.

[Induction of lung maturation in pending premature delivery and scheduled premature delivery].

At the Women's Hospital of the Medical Academy of Erfurt we have examined the efficiency of dexamethasone on fetal lungs maturation at imminent premature birth and planned untimely delivery. Dexamethasone was given in a randomized series at a dosage of 12-mg per day orally on three successive days. The effectiveness of induction was measured at the percentage of the total phospholipid fraction in the amniotic fluid and at the mortality of RDS in both groups. A successful induction with dexamethasone we can only reach after the 32nd week of pregnancy.--Because this effect is not always continuous, we recommend to make the induction of fetal lungs maturation with dexamethasone and--if it necessary--its repetition after one week conditional upon the content of total phospholipids in amniotic fluid.

[Practical significance of umbilical cord complications for the management of delivery].

In the Gynecological Hospital of the Academy Erfut, the deliveries of the year 1973 were analysed prospectively in regard of the frequency of umbilical cord complications and their effects to the fetus. At a frequency of 39.3%--i.e. every 5th delivery--, umbilical cord complications became a menacing factor to the fetus. The clinical symptoms of fetal threat are discussed in regard of their influence and importance to taking controlling obstetrical measures.

[Immunologic studies in relation to cryotherapy of gynecologic diseases].

The interesting nexus of problems related to the behaviour of the T-cell effected immunity situation of patients treated by means of cryosurgery and operations, was commenced by examinations of 118 women in regard of the transformability of lymphocytes. By the help of swab-preparations and especially of the PHA-stimulated lymphocyte transformation test, patients have been controlled in regard of vulva carcinoma, cervix carcinoma, condyloma, kraurosis vulvae and other portio findings being in need for medical attendence. Though the results are not to be regarded without reserve, the cryotherapy seems to improve the T-cell effected immunity situation in some of the patients suffering from vulva carcinoma. Up to now, the controls have also shown that they may reveal prognostic suggestions in certain cases.

Augmentation of 17-1A-induced antibody-dependent cellular cytotoxicity by the triple cytokine combination of interferon-alpha, interleukin-2, and interleukin-12.

Previously, interferon-alpha (IFN-alpha), interleukin-2 (IL-2), and interleukin-12 (IL-12) were shown to increase the antibody-dependent cellular cytotoxicity (ADCC) induced by the murine monoclonal antibody 17-1A, which recognizes the tumor-associated antigen EpCAM. In this study, the authors wanted to determine whether the combination of these three cytokines would yield greater cytotoxicity than the single cytokines. For cytotoxicity assessment, a new flow cytometric assay was used that allows the analysis of long-term ADCC exerted by macrophages. Peripheral blood mononuclear cells from healthy donors were used as effector cells against the colorectal carcinoma cell line HT29 at a low effector-to-target ratio of 4.5:1. With this test, the effectiveness of the combinations IL-2 and IFN-alpha, IL-2 and IL-12, and IL-12 and IFN-alpha were compared with each other. The combinations IL-2 plus IL-12 and IFN-alpha plus IL-12 were more potent at the concentrations tested. Furthermore, the triple cytokine combination of IFN-alpha, IL-2, and IL-12 revealed significantly greater ADCC than dual cytokine combinations. Next, CD14+, CD4+, and CD4- cells were isolated by paramagnetic beads and magnetic activated cell sorter (MACS) columns. The CD14+ and CD4- cell populations contained the ADCC effectors. The addition of CD4+ cells to CD14+ or CD4- cells resulted in augmented ADCC, indicating that cooperation between immune cells occurs. These results suggest that multiple cytokine combinations with monoclonal antibodies may be more effective for cancer immunotherapy.

Geotrichum capitatum septicaemia in neutropenic patients: case report and review of the literature.

We report a case of systemic infection with Geotrichum capitatum in a patient with acute myeloid leukaemia. Three days before death, the patient developed acute renal failure, probably caused by occlusion of glomerula with hyphae of G. capitatum. Up until now, prophylaxis and treatment of infections caused by Geotrichum capitatum have not been established. However, the prophylactic administration of high-dose itraconazole and the therapeutic use of liposomal amphotericin B are subjects of discussion.
