[Diagnostic and therapeutic problems of nasopharyngeal carcinoma in childhood (author's transl)].

Between 1974 and 1978 three children with a nasopharyngeal carcinoma (stages: T3N3MO, T4N2MO, T4N1MO) were treated with primary radiotherapy (tumor dose: 45-67 Gy). Local control was achieved in two children, one of these (T4 primarily) had a local relapse five months after diagnosis. Only one of the three children had radiotherapy to the neck region. This child developed lung and mediastinal metastases with hypertrophic osteoarthropathy twelve months after diagnosis. In the other two children lung metastases were observed two and seven weeks after the diagnosis respectively. In these two radiotherapy of metastases and chemotherapy were considered more important than radiotherapy of the neck region. All three children showed disappearance or considerable reduction of lung metastases due to radio- and chemotherapy. Yet, all three died of progressive disease within a few months. Early diagnosis of nasopharyngeal carcinoma in children will only be possible when pediatricians are more aware of this disease, also in our regions. Radiotherapy should be done only with megavoltage equipment using extensive radiation fields for the primary tumor area and elective radiation of the neck region. The use of additional primary chemotherapy is suggested for stages T3/T4. This mode of management may help to prevent distant metastases.

Different keratin polypeptides in epidermis and other epithelia of human skin: a specific cytokeratin of molecular weight 46,000 in epithelia of the pilosebaceous tract and basal cell epitheliomas.

Cytokeratin polypeptides of human epidermis, of epithelia microdissected from various zones of the pilosebaceous tract (outer root-sheath of hair follicle, sebaceous gland), and of eccrine sweat-glands have been separated by one- and two-dimensional gel electrophoresis and characterized by binding of cytokeratin antibodies and by peptide mapping. The epithelium of the pilosebaceous tract has three major keratin polypeptides in common with interfollicular epidermis (two basic components of mol wts 58,000 and 56,000 and one acidic polypeptide of mol wt 50,000); however, it lacks basic keratin polypeptides in the mol wt range of 64,000-68,000 and two acidic keratin-polypeptides of mol wts 56,000 and 56,500 and contains an additional characteristic acidic cytokeratin of mol wt 46,000. Another cytokeratin polypeptide of mol wt 48,000 that is prominent in hair-follicle epithelium is also found in nonfollicular epidermis of foot sole. Both epidermis and pilosebaceous tract are different from eccrine sweat-gland epithelium, which also contains two major cytokeratins of mol wts 52,500 and 54,000 (isoelectric at pH 5.8-6.1) and a more acidic cytokeratin of mol wt 40,000. A striking similarity between the cytokeratins of human basal-cell epitheliomas and those of the pilosebaceous tract has been found: all three major cytokeratins (mol wts 58,000; 50,000; 46,000) of the tumor cells are also expressed in hair-follicle epithelium. The cytokeratin of mol wt 46,000, which is the most prominent acidic cytokeratin in this tumor, is related, by immunological and peptide map criteria, to the acidic keratin-polypeptides of mol wts 48,000 and 50,000, but represents a distinct keratin that is also found in other human tumor cells such as in solid adamantinomas and in cultured HeLa cells. The results show that the various epithelia present in skin, albeit in physical and ontogenic continuity, can be distinguished by their specific cytokeratin-polypeptide patterns and that the cytoskeleton of basal-cell epitheliomas is related to that of cells of the pilosebaceous tract.

Immunological and biochemical characterization of the keratin-related component of Mallory bodies: a pathological pattern of hepatocytic cytokeratins.

Mallory bodies induced by long-term griseofulvin feeding in mouse liver were isolated and analyzed by one- and two-dimensional gel electrophoresis and reaction of the separated polypeptides with cytokeratin antibodies using the blotting technique. Comparison with normal intermediate filament cytoskeletons from mouse hepatocytes revealed that Mallory bodies contain two polypeptides: Component II (Mr: 55,000; apparent isoelectric point values: 6.45, 6.1, 5.9) and component III (Mr: 48,000; apparent isoelectric point values: 5.7, 5.5, 5.43, 5.38, 5.2) which appear to be similar, if not identical, to liver cytokeratins A and D, respectively. By contrast, component I of Mallory bodies (Mr: 65,000; apparent isoelectric point values: 5.4, 5.38, 5.2) was not found in appreciable amounts in normal hepatocytes. Component II was positive in immunoreaction with antibodies to murine hepatocyte keratins A and D as well as epidermal prekeratin. Component III showed reaction with the antibodies to murine hepatocyte keratins A and D but not with those raised against epidermal prekeratins. By contrast, the unusual component I reacted with antibodies to murine hepatocyte keratin D and to epidermal prekeratins. The results prove that cytokeratin polypeptides are major constituents of Mallory bodies and suggest that the pattern of liver cytokeratin polypeptides is altered during the toxic treatment and/or Mallory body formation.

[Bridging a lymphatic vessel defect using vein interposition (an experimental study)].

By means of improved microvascular surgery 10-15 mm long vein grafts were interposed into the lymphatic vessels in the groin area of 12 dogs. 6-12 weeks later a histological, clinical and radiological reexamination was performed. In 8 cases out of the 12 the interposed veins were patent, in 4 cases the lumina were occluded by fibrotic tissue. The clinical application of lymphatic vessel-defects in cases of lymphedemas is discussed.

[Treatment of Wilms' tumor (author's transl)].

Uniform treatment based on the therapeutic approach of the 1st and 2nd US National Wilms' Tumor Study was decided on in March 1976 by paediatricians, surgeons, urologists and radiotherapists in Austria. Wilms' tumour was diagnosed in 34 children between 1 january 1976 an 29 february 1980 (stage I: n = 11, stage II: n = 8, stage III: n = 8, stage IV: n = 7). Parents of two children refused treatments; both children have since died of metastases. Of the remaining 32 children 29 (90.6%) are alive, 10 for more than 4, 15 for more than 3 and 19 for more than 2 years after diagnosis. 21 children are without need of treatment. Three children have died, one due to postoperative complications, one due to haemorrhagic chickenpox, but free of tumour, and one after insufficient treatment. Two of the five children with a recurrence between 2 1/4 to 15 months after diagnosis had been treated inadequately in the initial phase. The tumour free survival rate in 74.2%. Two children with early occurring or recurrent lung metastases have survived for 53 1/2 and 54 months up to now.

Keratin-like proteins in normal and neoplastic cells of human and rat mammary gland as revealed by immunofluorescence microscopy.

Normal and neoplastic human breast tissue as well as lactating and nonlactating rat mammary glands and 7,12-dimethylbenz(alpha)-anthracene-induced mammary adenocarcinomas of rat, were examined by indirect immunofluorescence microscopy using guinea pig antibodies to human and bovine epidermal prekeratin and to cytokeratin polypeptide D from mouse hepatocytes. In normal mammary glands of both species, lactating rats included, the antibodies raised against human and bovine epidermal prekeratins strongly stained ductal and myoepithelial cells, whereas antibodies to hepatic cytokeratin D revealed, in addition, fibrillar staining in cells of the alveolus-like terminal lobular units and in milk secreting cells of the rat. The presence of some finely dispersed intermediate-sized filaments of the cytokeratin type in lactating alveolar cells of rat mammary gland was also demonstrated by electron microscopy. In human intraductal mammary carcinomas the antibodies to epidermal prekeratins showed staining in myoepithelial cells and intralumenal papillary protrusions of the tumor, whereas the antibodies to hepatic cytokeratin D presented an almost complementary pattern in that they showed strongest staining in the more basally located layers of tumor cells. Intraductal adenocarcinomas of rats showed strong staining with all keratin antibodies examined. In contrast to previous studies using exclusively antisera raised against epidermal prekeratin, out results show that all types of neoplastic and non-neoplastic epithelial cells of mammary gland of both species contain-at least some-filaments of the cytokeratin type identifiable by immunologic reaction, if antibodies are used that recognize a broad range of epidermal and nonepidermal cytokeratins. Consequently, such broad range antibodies to keratin-like proteins provide adequate tools to identify and characterize neoplastic and non-neoplastic epithelial cells and to eliminate false negative immunocytochemical findings in tumor diagnosis. In addition, our observation that in the same human carcinoma two cell types can be distinguished by their reaction with two different antibodies to cytokeratins from epidermis and liver, respectively, indicates that the cells of a given carcinoma can differ in their cytoskeletal composition, thus presenting further criteria for diagnostic differentiation.

[The accuracy of transrectal aspiration biopsy in routine diagnosis of prostatic carcinoma].

Over a period of four years 352 patients suspected of having a carcinoma of the prostate were investigated and 463 cytological and 343 histological examinations were undertaken. In 340 cases the cytological findings on transrectal aspiration biopsy according to Franzen [14] were compared with the histological findings on transperineal punch biopsy. This comparison showed an accuracy of 64.5% and if the PAP III findings were included, of 76.6% in diagnosing prostatic carcinoma by the cytological method. Since the majority of cases were first biopsies and a repeat biopsy was not made in view of the positive punch findings, the chances of detecting a carcinoma of the prostate by aspiration biopsy alone theoretically increase to almost 90%. The reasons for failures in concurrence of findings by the two methods are discussed.

A further case of submucosal lymphangioma of the duodenum diagnosed during endoscopy.

Submucosal lymphangiomas are benign tumors which rarely occur in the gastrointestinal tract. The current report describes the tenth case of a submucosal cavernous-cystic lymphangioma of the duodenum. The diagnosis was made during endoscopy and was confirmed histologically in a biopsy specimen. Following a brief review of the localization and symptoms of submucosal lymphangiomas of the gastrointestinal tract published thus far in the literature, therapeutic considerations are discussed.

[The value of exfoliative cytology of the urine in the diagnosis of bladder tumors].

It is reported on the cytological and histological results of 253 bladder tumors. There was an exact correspondence of the results in 80.3%; in 11.4% it was not possible to prove the existence of the bladder tumor cytologically. Most of these were low-grade malignant tumors. In 8.3% of the cases the results were obtained through PAP III according to the scheme of Papanicolaou.

[The lung phospholipid content in newborn infants with idiopathic respiratory distress syndrome in relation to lung histology and survival time (author's transl)].

In an autopsy series on 25 newborn infants who died in the perinatal period (19 with and 6 without respiratory distress syndrome [RDS]) the total phospho-lipid, lecithin, and sphingomyelin content was determined in lung homogenates as parameters of the surfactant content. Histologically, the newborn infants with RDS were subdivided into those with and those without hyaline membranes. Newborn infants with RDS hat a lower lung phospho-lipid content than mature newborn infants without RDS but there was no difference in phospholipid content in relation to the presence or absence of hyaline membranes. The lecithin content of the lungs in those infants with RDS who had survived for more than 24 hours was higher in the absence of hyaline membranes than in their presence.

[Unviable micromelic dwarfism: a syndrome with dystrophy of the thorax and polydaktyly type Saldino-Noonan (author's transl)].

Besides classical achondroplasia various severe osteochondrodysplastic disorders have been differentiated in recent years. Their most important features include obligatory perinatal mortality, severe dystrophy of the thorax and shortening of tubular bones. Additionally some of these syndromes present visceral malformations, cleft-lip, and malformations of the genitals. A detailed case report of a patient is given. Various features: dystrophy of the thorax, micromelia, postaxial polydactyly, metaphyseal dysplasia, malformations of the great vessels, and polcystic kidney suggest the classification as short rib-polydactyly syndrome, type Saldino-Noonan.

The normal squamous epithelium of the human uterine cervix: a histological study.

The normal squamous epithelium of the uterine cervix was investigated in 50 foetuses and newborns, as well as in 207 adult women operated upon because of various benign conditions of the uterus. The epithelium was divided according to the presence or the absence of prominent epithelial papillae. Epithelial papillae occurred in 32% of the cervices in the paediatric age and in 17% in adult women. They were present in the whole squamous epithelium covering the portio vaginalis in all pediatric patients. In 49% of the adult patients epithelial papillae covered the whole portio and/or the transitional zone; in the remaining 51% only a reduced area presented epithelial papillae. Micrometric determinations demonstrated that normal epithelium with papillar formation was thicker than epithelium with smooth epithelial border. This was most prominent in the paediatric patients. The possibility that oestrogenic stimulation (in paediatric patients) and oestrogenic stimulation as well as chronic stromal inflammation (in adult patients) could account for the formation of prominent epithelial papillae was discussed.
