19-year follow-up of fetal myelomeningocele brought to term.

We conducted a 19-year follow-up study of 223 infants with myelomeningocele born by cephalic presentation, 68 born by pre-rupture of amniotic membranes cesarean section (PRAM C/S) and 155 born vaginally to determine outcome. We recorded radiographic level as the last intact vertebral arch at the cephalad end of the spinal defect from standard AP films of the spine. Protrusions of the lesions dorsal to the back were obtained from medical records or parental recall. Subsequent spinal cord abnormalities (hydromyelia, tethered cord and hypoplasia) were determined by patient's symptoms and verified by MRI. Data were from our Patient Data Management System and analyzed using Microsoft Excel and Epi Info 6. Flat lesions protruding less than 1.0 cm beyond the plane of the back and those associated with breech position or kyphus were not benefited by PRAM CIS (Fisher's Exact, p = 0.58 between the two types of delivery for motor level). Flat lesions were more common in the vaginally delivered group (p = 0.01). Lesions protruding equal to 1.0 cm or beyond were associated with less paralysis after PRAM C/S (p = 0.01). Although tethered cord syndrome was more common in the PRAM C/S group (p = 0.02), there was no difference in the muscle strength loss between the PRAM C/S and the vaginally delivered groups. Symptomatic hydromyelia was equally distributed but spinal cord hypoplasia was more common in the PRAM C/S group (p = 0.03). Due to the limited numbers of patients in each group available for analysis, we recommend further study to ascertain the appropriate management of fetal myelomeningocele diagnosed in utero and brought to term.

Client personality disorders affecting wife assault post-treatment recidivism.

Previous evaluations of wife assault treatment outcome have focused generically on whether groups "succeed" or not without a clear criterion of what constituted success. The present study examines the question for whom groups generate the greatest reduction in post-treatment abuse and for whom they work least well. It was found that certain types of personality disordered men had the worst post-treatment prognosis. Specifically, men with high scores on borderline personality, antisocial personality, and avoidant personality fared least well after treatment. However, taken as a generic group, men in treatment had significantly reduced post-treatment abusiveness whether reported by themselves or their wives.

[Colorectal carcinoma. Which factors are decisive for development of postoperative complications?].

This retrospective study analyses the prognostic effect of different factors on morbidity and lethality based on selected, primarily resecting colon carcinoma operations (n = 222). In all, 12.2% of our operations were performed under emergency conditions. The total complication rate was 31.5%, the lethality rate 7.7%. The left hemicolectomy cases showed the highest morbidity (48.4%), the rectum amputation the highest lethality (11.8%). Tumour staging tumour differentiation and the sex of the patient showed no significant influence on the postoperative morbidity and lethality. However, a correlation was proved between the age of the patient, tumour localisation, co-morbidity, duration of operation and the conditions under which the operation was performed (emergency or elective), on the one hand, and morbidity and lethality on the other.

[Drug monitoring: theophylline--comparison of 3 methods for determination].

Treatment with theophylline requires monitoring of dosage, preferably by determining the theophylline content in whole blood or serum, because theophylline pharmaco kinetics is most variable and therefore difficult to define, and because of possible interactions with other drugs. In medical practice, it is important and helpful to get the results of theophylline measurements quickly so that the dosage may be corrected directly whenever necessary. Two assays for rapid theophylline determination were compared with a standard method and with each other in respect of accuracy and practicability. Both methods a suitable for practical use.

Congenital malignant melanoma.

The diagnosis and management of a 3-day-old infant with an exophytic pigmented growth on her back and chest are discussed. At 3 days of age, the child underwent a complete surgical excision of this lesion with immediate coverage by partial-thickness skin grafts. Pathologically, the lesion proved to be a thoracolumbar congenital melanocytic nevus with multiple focal areas of malignant melanoma. The child is now 3 years past surgery and shows no evidence of recurrence or distant metastases. The importance of this case lies in the presence of overt malignant melanoma at birth within a giant congenital nevocytic nevus.

Policy alternatives for reducing tobacco sales to minors: results from a national survey of retail chain and franchise stores.

Minors' access to tobacco has become an important public health issue. Little is known, however, about the knowledge, attitudes, beliefs, and behavior toward access among executives from businesses that sell tobacco. This study examined access from the perspective of corporate and regional headquarters of retail chains and franchises that sell tobacco. A total of 148 U.S. companies with the largest overall retail sales volume that sold tobacco were asked to participate; 91 agreed. The sample included grocery stores, convenience stores, gas station mini-marts, liquor stores, and drug stores. Data revealed at least moderate support for policies limiting youth tobacco access. Although most companies reported having in place policies to prevent minors from purchasing tobacco, these policies did not seem intensive. In addition, executives underestimated the extent of youth access. We conclude that the time is right for passage of bold policies to protect young people from tobacco.

[Bilateral chylothorax--case report and literature review].

A bilateral chylothorax developed in a woman patient during recurrence of a non-Hodgkin's lymphoma, originally treated by laminectomy and telecobalt irradiation. Almost simultaneously there was a thrombosis of the brachial vein in the right arm. On the basis of this particular case, attention is drawn to the difficulty in pathogenetic classification. The possible causes, especially of the bilateral nature of the chylothorax, are discussed while referring to the relevant literature.
