Human acclimation to work in warm and humid environments.

The aim of this study was to analyse the acclimation of male Europeans during a forty-one day stay in the hot and humid climate of Thailand. We also tried to examine the phases of acclimation which would eventually be used by trainers in the elaboration of the schedule of athletes' preparation to participate in competitions in a tropical climate. Twelve Polish male subjects ageing 21-38 years participated in these examinations. In Poland and Thailand the cycloergometric exercise test with the load of 53% of VO2max was performed until a 1.2 degrees C (delta Tre) increase in rectal temperature was reached. The exercise test was executed in the same environmental conditions (i.e. 30 +/- 1 degrees C and 70 +/- 3% of relative humidity). The duration of this exercise test (DE) was used as a criterion for the efficiency of thermoregulatory functions. During acclimation, three peaks of greater exercise thermoregulatory efficiency have been found, i.e. on the 4th-5th, 11th-12th and 29th-30th days of stay. These findings are particularly important for professional athletes who wish to prepare themselves for competitions held in hot and humid climates.

[On the need for analysis of the occupational health education system].

The paper includes conceptual premises on which the analysis of the occupational health education system is based as well as the outcome of an attempt to perform a comparative study in this area. The authors present some of the features which characterize the education system designed for occupational health physicians. A comparative approach adopted in the analysis (Poland vs some other countries) seems to be the simplest and the most effective way to draw appropriate conclusions. The analysis covered both undergraduate and postgraduate studies, including specialisation in the area of occupational health. Finally, the work highlights theoretical and methodological aspects of further analyses.

Exercise thermoregulation in Thai and Polish male subjects.

The aim of this study was to compare the efficiency of exercise thermoregulatory functions in Thai and Polish groups. Twelve Thai and 14 Polish healthy male students were included. An exercise test on a bicycle ergometer at 53% of the maximal work load was performed at ambient air temperature of 30 degrees C and relative humidity of 70%. The test was terminated when rectal temperature (Tre) rose by 1.2 degrees C. Duration of this exercise considered as an index of the efficiency of exercise thermoregulatory functions. The duration of test exercise was evidently longer in Thais (44.8 min) than in Polish students (34.1 min). The rate of increase in the internal body temperature was lower in Thais (0.029 degrees C x min-1) than in Poles (0.037 degrees C x min-1) but exercise dehydration as well as the rate of body weight loss were significantly higher in Thais than in Poles. It can be concluded that the exercise thermoregulatory mechanisms are more efficient in Thais. Polish young athletes were poorly adapted to exercise in warm and humid environment and thus they require more exercise training in climatic chamber prior to competitions held in countries with warm and humid climate.

Multiple cycles of dose-intensive cyclophosphamide, etoposide, and cisplatinum (DICEP) produce durable responses in refractory non-Hodgkin's lymphoma.

Patients whose lymphoma is resistant to standard treatment regimens continue to do poorly, with only an occasional patient achieving long-term remission even with bone marrow transplantation. Twenty-three patients with primarily refractory (11), or refractory relapsed (12) non-Hodgkin's lymphoma were treated with repeated cycles of dose-intensive cyclophosphamide, etoposide, and cisplatin (DICEP) without bone marrow transplantation. Each cycle of DICEP consisted of cyclophosphamide (2500 mg/m2/day, days 1-2), etoposide (500 mg/m2/day, days 1-3), and cisplatin (50 mg/m2/day, days 1-3). Twelve patients (52%) have achieved a complete response and 6 (26%)r a partial response. Three of the complete responders remain continuously free of disease for 19, 29, and 32 months, and 3 more are disease-free at 58, 59, and 65 months after receiving further therapy. Three-year survival for all patients is 45%. Patients with a good initial performance status (Zubrod 0 or 1) had a 58% complete response rate and a 2-year survival rate of 53%. One of 19 patients with an initial performance status of 0 or 1 had treatment-related mortality. Repeated cycles of DICEP can produce long-term responses in patients with refractory non-Hodgkin's lymphoma.

[Specialization of physicians of the Industrial Health Care System in occupational medicine].

One of the principal issues concerning the analysis of industrial health care functioning is the problem of the practitioners' qualifications, including specialization. In the present paper the authors focus their attention on specializations of practitioners employed in regional Industrial Health Care Complexes. The analysis refers to two groups of problems concerning: a) those practitioners who are specialists in occupational health, and b) those who are going to obtain such specialization. The authors refer to the personnel of the industrial health care as a whole and pay special attention to those who are employed in the districts where seats of Industrial Health Care Complexes are located.

Effects of skating and diagonal skiing techniques on results and some physiological variables.

In this study, the effects of skating and diagonal techniques on skiing load at various speeds were compared. When skiing at the same speed, subjects' heart rates were significantly lower when using the skating technique than when using the diagonal technique (p less than 0.05). At maximal speed, skating technique gave a significantly higher speed at the same heart rate (p less than 0.001) than diagonal technique. Above the anaerobic threshold (4 mmol.L-1) blood lactic acid levels remained significantly lower (p less than 0.005) when using the skating technique than when using the diagonal technique. The results suggest that at the same speed, skating technique loads aerobic and anaerobic energy production less than diagonal technique, and that the same maximal energy production gives a significantly higher skiing speed when using the skating technique than when the diagonal technique (p less than 0.001).

The kinetics of peritoneal insulin absorption.

This study examined the relationship between the delivery of insulin into the peritoneal space and its absorption into the peripheral circulation. Studies were performed in conscious dogs receiving somatostatin (5.0 microgram/min) to suppress endogenous insulin secretion, and intravenous glucose (50 mg/min) to prevent hypoglycemia. The biologic effectiveness of the absorbed insulin was determined by its hypoglycemic effect. The possibility of direct absorption of insulin into the portal circulation from the peritoneal space in anesthetized, portal vein-catheterized dogs was examined with radiolabeled I125 insulin. Our results suggest that absorption of insulin from the peritoneal space is volume, concentration, and time-dependent. Maximal absorption of insulin was observed at 50 min when 1.92 U of insulin in a volume of 3 ml was infused intraperitoneally over 30 min. More rapid absorption was observed at 30 min when this quantity of insulin was given in a 1-min intraperitoneal bolus, compared to 30 min of intraperitoneal infusion. Least rapid absorption of insulin followed the delivery of 1.92 U of insulin in a volume of 15 ml. Intermediate absorption of insulin was observed at 40 min when the 1.92 U was delivered in a volume of 0.6 ml. Peripheral intravenous insulin delivery of 1.92 U reached a maximal plasma concentration at 20 min, which was more than three times the concentration observed with intraperitoneal insulin. Isotopic tracer studies, in which radioiodinated insulin was placed into the peritoneal space in anesthetized dogs, demonstrated greater radioactivity in the portal vein than in the aorta throughout a 30-min observation period. These studies demonstrate that intraperitoneal administration of insulin results in absorption of insulin which is volume, concentration, and time-dependent. Thus, the peritoneal space may be an appropriate site for insulin delivery through a transcutaneous catheter.

Relationship between plasma volume reduction and plasma electrolyte changes after prolonged bicycle exercise, passive heating and diuretic dehydration.

Plasma volume was decreased by prolonged bicycle exercise, by passive heating in warm water, by sauna dehydration, and by diuretically induced dehydration in eleven well trained subjects. Blood samples from an arm vein were taken before and after this pre-treatment, as well as after a subsequent standard exercise test (SET) on a bicycle ergometer (50%, 70% and 105% of max VO2; the SET with no pre-treatment was used as a control condition. The changes in plasma concentration of Na+, K+ and Cl- were not proportional to the calculated plasma volume changes. The Na+ and Cl- concentrations always increased in the plasma, while plasma potassium concentration was increased after prolonged exercise, but decreased after the other types of dehydrations. The standard exercise test produced a pronounced fall in total calculated plasma potassium and in K+ concentration measured 3-5 min after exercise in all types of experiments. In the standard exercise test the calculated water loss from the plasma volume was relatively large. It amounted to about 2/3 of the total water loss in the standard exercise test and was independent of the pre-treatments.
