Purine metabolism of erythrocytes in myotonic dystrophy.

The uptake and subsequent metabolism of adenine and adenosine was studied with the intact erythrocytes of patients with myotonic dystrophy (MD). Washed erythrocytes of both controls and patients were incubated with 14C-labeled adenine or adenosine at 20 degrees C for 5 to 120s to characterize the uptake process. No differences in the uptake process of adenine or adenosine were observed between normal and MD erythrocytes. Formation of adenine nucleotides was determined at 37 degrees C after incubation for 30 min. Compared to controls, the incorporation of adenine into adenine nucleotides was 2.6 times higher in MD erythrocytes. This depends mainly on an increase of ATP formation. The mean ratios of ATP:ADP + AMP for normal red cells and MD erythrocytes were 0.92 and 1.39 respectively. No difference was found in the conversion of adenosine to adenine nucleotides. In spite of a normal amount of intracellular ATP a greater demand for ATP exists. This might be due to leakage of adenine nucleotides out of MD erythrocytes.

Evaluation of serum guanase in hepatic diseases.

Serum guanase activity was measured in 20 healthy adults and in 62 patients with acute viral hepatitis, chronic active hepatitis, chronic persistent hepatitis, liver cirrhosis and fatty liver. Guanase and gamma-GT in patients were elevated in 87 and 64%, respectively. Elevated guanase activities were found in most cases of acute viral hepatitis, as well as in chronic hepatopathies. In patients with acute viral hepatitis pathologic activities of guanase were found following partial or total normalization of other liver function tests.

[The effect of benzbromaron in gout patients with limited kidney function].

The effect of the uricosuric drug benzdromarone was investigated in 15 patients with hyperuricemia and renal failure. Benzbromarone micron was given in a dose of 80 mg/day for 6 days. The mean serum uric acid level fell from 8.9 to 4.24 mg/100 ml. After therapy uric acid concentrations in serum returned to 5.2 mg/100 ml. Renal urate elimination was increased from 487 mg/day to 831.3 mg/day; after medication urate elimination decreased to the initial values. Uric acid clearance was increased by benzbromarone from 3.97 to 11.1 ml/min, and even after therapy a uric acid clearance of 6.4 ml/min could be measured. A decrease of uric acid reabsorption from 4.73 to 2.3 mg/min was observed, after medication the reabsorption rate increased to 3.42 mg/min.

Enzymes of the purine interconversion system in chronic lymphatic leukemia: decreased purine nucleoside phosphorylase and adenosine deaminase activity.

Activities of adenosine deaminase (ADA), adenosine kinase (AK), adenine phosphoribosyltransferase (APRT), hypoxanthine guanine phosphoribosyltransferase (HGPRT), and purine nucleoside phosphorylase (PNP), all enzymes of the purine interconversion system, were determined in lymphocytes of 25 patients with chronic lymphatic leukemia (CLL) and in 23 controls. A statistically significant decrease of PNP activities and a reduction of ADA activities at borderline levels were found in the patients, whereas for the other enzymes assayed no deviation from normal values was observed.

[Serum fucosyltransferase activity in malignant diseases].

Serum Fucosyltransferase activities were measured in 41 controls and in 55 patients with neoplastic diseases. Incorporation of 14C-fucose into the endogenous acceptor of serum and after addition of desialofetuin as exogenous acceptor, was determined. About 83% of patients suffering from various malignant diseases showed pathological fucosyltransferase activities. In all patients with testicular tumours and no detectable metastases after surgery, normal enzyme levels were obtained. After successful chemotherapy, fucosyltransferase activities returned to normal.

Influence of metabolic control of insulin-dependent diabetes on plasma nucleotide levels (cAMP, cGMP) during bicycle exercise.

Plasma concentrations of cyclic nucleotides (cAMP, cGMP) were measured before and during bicycle exercise in 8 well-controlled (mean pre-exercise blood glucose 5.3 mmol/l; HbA1 8.6%) and 8 moderately controlled (mean pre-exercise blood glucose 12.2 mmol/l; HbA1 10.8%) patients aged 18-32 years with insulin-dependent diabetes mellitus (IDDM) and in a group of non-diabetic control subjects matched for age and sex. Pre-exercise plasma cAMP concentrations and the rise with exercise were similar in all study groups. Significantly lower resting cGMP levels were found in well-controlled IDDM patients (3.5 +/- 0.3 pmol/ml, mean +/- SEM) compared to controls (5.6 +/- 1.1 pmol/ml; p less than 0.05) and moderately controlled IDDM patients (5.6 +/- 1.0 pmol/ml; p less than 0.05). By contrast, plasma cGMP levels increased during exercise in the diabetics but not in the controls. These findings indicate a significant difference in responses of plasma cGMP to exercise between IDDM patients and controls.
