Facial clefting in Kuwait and England: a comparative study.

A retrospective study of all new cases of facial clefts seen between January 1985 and December 1987 was carried out at the lbn Sina Hospital, Kuwait, and West Midlands Regional Plastic and Jaw Surgery Unit at Wordsley Hospital, Stourbridge, England; 234 arab children in Kuwait and 181 white Caucasian children in the West Midlands were studied. Despite vast differences in racial characteristics, environment and customs the pattern of clefting was very similar in the two groups. The incidence of clefts was calculated for the two regions and this demonstrated an increase in the incidence of cleft lip and palate (CL/P and CP) in the West Midlands since the last study was reported from this region in 1953.

Scald accidents during water aerosol inhalation in infants.

A study of 193 infants admitted for burns over a 4-year period revealed 11 infants (5.7 per cent) had sustained scalds during the process of water aerosol inhalation. The water aerosol inhalation therapy was prescribed for respiratory tract infection and carried out at home using either an electric kettle or a saucepan. It appears that infants are prone to this type of injury because of the difficulty of keeping them still during therapy and their inquisitiveness to explore their surroundings. The resulting scalds added to the morbidity of those children already suffering from respiratory tract infections. These accidents can easily be prevented by the alternative use of a humidifier.

Mortality from burns in Kuwait.

There were 96 deaths among 1433 burns patients admitted to the Plastic Surgery and Burns Unit at Ibn Sina Hospital and Plastic Surgery Unit at Mubarak Al-Kabeer Hospital from January 1982 to December 1987. This represents an overall mortality of 6.7 per cent; 74 per cent of the patients who died had sustained more than 50 per cent body surface area (BSA) burns. A large number of deaths occurred in two age groups, 0-5 years (21 deaths) and 21-30 years (24 deaths). The male to female ratio of burn-induced deaths was 1.18:1. Flame burns due to domestic accidents were the aetiological factors in the majority of patients; 84 (87.5 per cent) of those who died sustained flame burns, although flame burns were only responsible for 46.6 per cent of all burns cases admitted. The mean survival period was 16.8 days postburn, the longest period being 79 days for a patient with 60 per cent BSA burns. Forty-seven patients (49.9 per cent) died of septicaemia which, with its related complications, remains the most important cause of death in our burns patients.

Congenital sinuses of the lower lip: reappraisal of Van der Woude syndrome on the basis of nine patients.

Congenital sinuses on the lower lip in association with cleft lip, cleft palate, or both are rare and occur in four syndromes, namely Van der Woude syndrome, popliteal pterygium syndrome, orofacial digital syndrome, and the ankyloblepharon filiform adnatum. The most common and best studied among these--the Van der Woude syndrome--is believed to be due to an autosomally dominant gene with 80 to 100% penetrance and a variable expressivity. We present our study of 9 patients with congenital lower lip sinuses and the penetrance in their families. On the basis of our observations we conclude that this syndrome may have low penetrance among the populations studied.

Comparative study of skin wound closure with polybutester (Novafil) and polypropylene.

A prospective study of skin wound closure using polybutester (Novafil) and polypropylene was carried out in 100 elective procedures in 77 patients over a 1-year period at the Plastic Surgery Unit of Mubarak Al Kabeer Hospital, Kuwait. In this comparative study based on clinical evaluation, particular note was made of the handling characteristics of the suture material, wound infection, healing and eventual scar in each case. The tensile strength of both sutures was comparable and the wound infection rate and wound healing characteristics in the two groups were more or less the same. Our study has shown Novafil to be a superior suture for skin wound closure because of its handling qualities, easier removal from the healed wound and because of a cosmetically better scar in a significant number of patients.

Epidemiology of burns in Kuwait.

A prospective study of 765 burns cases treated as inpatients at the Burns Unit, IBN Sina Hospital and the Plastic Surgery Unit, Mubarak Al Kabeer Hospital from January 1984 to December 1986 is presented. Hot water scalds were the commonest type of injury. Most of the burns occurred at home but coinciding with standard working hours. Children up to 5 years old were commonly affected, but most of the fatal burns were among adults. The overall mortality rate was 7.2 per cent with a male to female ratio of 1.1:1. Our survival rate from burn injuries is improving due to advances in treatment, but prevention remains the best cure. We consider our epidemiological survey to be important as the basis for a preventive programme in Kuwait's extremely heterogeneous society.

Burns during pregnancy.

Eight cases of burns during pregnancy were treated in Kuwait during the last 3 years. In all burns above 30 per cent body surface area (BSA), abortion, premature labour and intrauterine fetal death are ever-present complications and the survival of the foetus in burns above 50 per cent BSA is uncommon. Nearly all these complications occur during the first week after the burn. For patients in the second and third trimesters of pregnancy with burns above 50 per cent BSA the prognosis is usually poor unless the pregnancy is terminated within 24-48 h after the burn.

The value of the Hoffmann skeletal fixation in the management of cross-leg flaps, particularly those injuries complicated by open fractures of the tibia.

The Hoffmann external fixator has been used to immobilize lower limbs for the application of cross-leg flaps in the presence of tibial fractures. The legs and the fracture are held rigidly in the optimal position during and after the operation, with ease of adjustment and excellent access to the flap. There is relatively less weight on the legs than when using a plaster cast, and with the joints free, the patient is much more mobile in bed.

Case report: Use of skin grafting to keep a scleroderma patient ambulant.

A case of scleroderma is presented in which there was extensive skin involvement with contractures, but without any systemic manifestation. This patient has been kept ambulatory by repeated skin grafting. The grafts were taken from the few areas which were not involved with the disease. The grafting was successful, and so far the skin grafts have remained free of disease.

Keloid and hypertrophic scars: trace element alteration.

The state of excessive fibroblastic proliferation for wound healing results in hypertrophic and keloid scars. It has been well established that some of the trace elements (such as zinc) are essential in wound healing, and there are appreciable changes in trace elements in various disease states. The levels of zinc (Zn), copper (Cu), manganese (Mn), and selenium (Se) in serum, normal skin and scar of 40 keloid and hypertrophic scar patients were assessed. There was a significant increase of manganese (Mn) level in skin of burn, trauma, and surgical incision patients compared to controls with p < 0.0013, p < 0.0001, and p < 0.046, respectively. Furthermore, the zinc, copper, and selenium contents of the skin in incision patients were decreased significantly when compared to other groups. No significant changes occurred regarding serum levels of zinc, copper, manganese, and selenium in the different groups. From this study, no relationships between the hypertrophic and keloid scar to trace elements were found; however, because of the limited numbers of patients, a definite conclusion could not be drawn.
