Pyoderma gangrenosum in pediatric acquired immunodeficiency syndrome.

We describe two children with human immunodeficiency virus infection in whom pyoderma gangrenosum developed. Although pyoderma gangrenosum most commonly occurs in children with inflammatory bowel disease, it has also been described in patients with a variety of immunodeficiencies. In such patients a vigorous search to exclude a treatable infection should be made before the lesions are treated as pyoderma gangrenosum.

Size of lethality target in mouse immature oocytes determined with accelerated heavy ions.

Mouse immature oocytes were irradiated in vivo with highly charged, heavy ions from the Bevalac accelerator at the Lawrence Berkeley Laboratory. The particles used were 670-MeV/nucleon Si14+, 570-MeV/nucleon Ar18+, and 450-MeV/nucleon Fe26+. The cross-sectional area of the lethality target in these extremely radiosensitive cells was determined from fluence-response curves and information on energy deposition by delta rays. Results indicate a target cross-section larger than that of the nucleus, one which closely approximates the cross-sectional area of the entire oocyte. For 450-MeV/nucleon Fe26+ particles, the predicted target cross-sectional area is 120 +/- 16 microns2, comparing well with the microscopically determined cross-sectional area of 111 +/- 12 microns2 for these cells. The present results are in agreement with our previous target studies which implicate the oocyte plasma membrane.

Cardiomyopathy with arrhythmias and ectodermal dysplasia: a previously unreported association.

Hidrotic ectodermal dysplasia represents a group of congenital or hereditary disorders that involve ectodermal derivatives. It is characterized by partial or complete alopecia, dystrophic nails, and dental abnormalities. Dilated cardiomyopathy has not previously been reported in association with this illness. We report the cases of three children with fatal dilated cardiomyopathy with associated cardiac arrhythmias and ectodermal dysplasia. Laboratory investigations revealed no specific cause for the cardiomyopathy. It is speculated that this association is not simply coincidental.

Sonography of the gallbladder, biliary tree, and pancreas in adults with cystic fibrosis.

Real-time sonography is an established method of evaluating the gallbladder, biliary tree, and pancreas. Adequate visualization with B-scan equipment has been a problem in adults with cystic fibrosis, a group with an increased incidence of gallbladder disease. We performed real-time ultrasonography on 23 adult patients with cystic fibrosis. The gallbladder was visualized in 22 (96%) and was found normal in 14 (61%). Abnormalities in the remainder included cholelithiasis (17%) and microgallbladder (30%). No dilatation of the biliary tree was demonstrated. The pancreas was well visualized in 18 (78%) and showed increased echogenicity in most. We conclude that real-time sonography of the abdomen reliably permits examination of the gallbladder, biliary tree, and pancreas in adult patients with cystic fibrosis.

Efficacy of the Drionic unit in the treatment of hyperhidrosis.

A portable iontophoretic device (the Drionic unit) has recently been introduced for the treatment of hyperhidrosis. Twenty-two patients with hyperhidrosis were treated at twenty-seven sites (axillae, palms, soles) with this unit. Sweat output was measured by use of Persprint paper and the data were quantified by an image analysis computer. The data clearly demonstrate the efficacy of the Drionic unit at all treatment locations in the great majority of subjects. One month posttreatment follow-up showed a statistically significant continued sweat inhibition at the palmar sites. Side effects were common, but none was severe enough to necessitate discontinuation of treatment. Based on the results of this study, we conclude that the Drionic unit appears to have a definite place in the treatment of hyperhidrosis.
