[Effect of ginsenosides in inducing proliferation and transcription factor of erythrocytic, granulo-monocytic and megakarocytic cell lines].

OBJECTIVE: To investigate the action of Ginsenosides (GS) in inducing transcription factor c-fos and GATA-1 to explore the mechanism of GS in hematopoietic cells. METHODS: The proliferation effects of GS on granulocytic (HL-60), monocytic (U937), erythrocytic (K562) and megaryocytic (Meg-01) cell lines were observed by using proliferation test of MTT and colony formation of progenitor cells. The combining reaction of transcription factors c-fos and GATA-1 with nuclear protein antigen were analyzed by Western Blot after being treated by GS. RESULTS: (1) GS (10 micrograms/ml) could stimulate and promote proliferation of 3 cell lines with significant difference between GS and non-GS control (P < 0.05 in all) in both MTT test and colony assay. (2) After treatment with GS, c-fos protein in HL-60, K562 and Meg-01 cell lines was increased by 1.5, 2.0 and 2.5 fold respectively, while U937 cell did not express c-fos. (3) Except that U937 cell hadn't expressed GATA-1, the other cell lines after the treatment by GS, the GATA-1 protein level was elevated to 1.5, 2.1 and 1.3 fold of that before treatment. CONCLUSION: The proliferation of three lines initiated by GS was involved in transcription factor c-fos or GATA-1, which could pay the role in the GS induced up-regulation correlated with proliferation and differentiation of hematopoiesis.

[Comparative effects of carvedilol and metoprolol in preventing from left ventricular remodeling after acute myocardial infarction in rats].

OBJECTIVE: To compare the effects of carvedilol and metoprolol in preventing from left ventricular remodeling (LVRM) after acute myocardial infarction (AMI) in rats. METHODS: Twenty-four hours after ligating left coronary artery, 105 surviving female SD rats were randomly assigned to AMI control, carvedilol 1 mg/(kg.d) and metoprolol 2 mg/(kg.d) groups. Sham-operated rats (n = 16) were selected randomly as non-infarction control. After four weeks of drugs therapy, hemodynamic studies and pathologic analysis were performed. Exclusive of MI size < 35% or > 55%, complete experimental variables were obtained in 46 rats, which were comprised of AMI (n = 11), carvedilol (n = 12), metoprolol (n = 11), and Sham-operated (n = 12) groups. RESULTS: Compared with sham-operated group, left ventricular (LV) end diastolic pressure (LVEDP), volume (LVV) and weight (LVW), were all significantly increased (P < 0.05-0.001), while maximal rate of rise and fall (+/- dp/dt) of LV pressure as well as their corrected values (+/- dp/dt/LVSP) were all significantly decreased (P < 0.01-0.001) in AMI group. In comparison with AMI group, the LVEDP and LVV were all significantly decreased (all P < 0.001), while +/- dp/dt and +/- dp/dt/LVSP were significantly increased (P < 0.05-0.001) in both carvedilol and metoprolol group, with LVW and RVW only decreased in carvedilol group (P < 0.05-0.01). CONCLUSIONS: 1. Carvedilol can effectively attenuate LVRM, and improve hemodynamics and LV function after AMI in rats, 2. Metoprolol has equivalent beneficial effects as carvedilol on hemodynamics, LV dilatation and function, but not LV hypertrophy.

[Clinical study on tongxinluo capsule in treatment of patients with angina pectoris caused by coronary heart disease].

OBJECTIVE: To observe the effects of Tongxinluo capsule (TXLC) in treating angina pectoris (AP) caused by coronary heart disease (CHD) and evaluate its safety. METHODS: Randomized single-blind controlled design were adopted. Three hundred and forty two patients were treated with TXLC (4 capsules, three times daily), and 150 patients in the control group done with Shuxin oral liquor (SXOL, 20 ml, two times daily). After 4 weeks of treatment, the data of AP, ECG, main symptoms, total effects were collected and evaluated. RESULTS: TXLC appeared to be more effective than SXOL for patients with mild, moderate, severe AP (P < 0.01), except with mild stomach discomfort for a few patients, TXLC has no side effect and toxicity. CONCLUSION: TXLC is an effective drug in treating AP and has no side effects and toxicity.

[Application of silicone oil in management of complicated retinal detachment surgery].

60 eyes of 60 cases with complicated retinal detachment were treated by vitrectomy combined with silicone oil tamponade, including grade D of proliferative vitreoretinopathy (PVR) in 27, giant retinal tears with posterior flaps folded up in 15, posterior polar or macular holes in 13 and traumatic PVR in 5 eyes. After 3-24 months of follow-up, it is shown that 48 eyes obtained anatomic reattachment, the success rate being 80%, and the post-operative visual acuities improved in 43 eyes of which 32 eyes achieved > 0.05. The authors consider that vitrectomy and membrane peeling create a situation for silicone oil tamponade to play its role fully, vitrectomy is perfected by the combination of silicone oil tamponade and the rate of success of the retinal detachment operation is increased. The theory, indications, advantages and disadvantages of silicone oil tamponade were briefly discussed.

[Percutaneous transluminal coronary angioplasty: relationship between the characteristics of the lesions and the immediate outcome].

In order to clarify the relationship between characteristics of lesions and the immediate outcome after percutaneous transluminal coronary angioplasty (PTCA) in Chinese patients, 170 cases of PTCA performed in this institution from December 1986 to July 1993 were analysed. There were 149 cases of angina pectoris, 75 complicated with old myocardial infarction; 21 cases of acute myocardial infarction (AMI) including 8 complicated with cardiogenic shock, PTCAs were performed under assistance of IABP in 4 and under assistance of centrifugal pump in 1.236 lesions, including 15 lesions of total occlusion in AMI, 27 of chronic total occlusion and 9 lesions in bifurcation using double balloon technique, were attempted to be dilated. 213 (90.3%) of 236 lesions were successfully dilated. According to the standard set by the report of ACC/AHA task force, the success rate in type A lesions was 47/48 (97.9%), in type B lesions 136/151 (90.1%) and in type C lesions 30/37 (81.1%). The success rate in type A lesions was significantly higher than that in type C lesions (P < 0.01). In 7 of 8 patients with cardiogenic shock complicating AMI, PTCA was successful. 4 of the 7 survived. The major factors for failure in dilatation included chronic total occlusion, excessive tortuosity of proximal segment, eccentric and tubular lesions, etc. Acute occlusion occurred in 2 cases with type C and 1 with type B lesion, one patient died after 24h of emergency CABG. In one patient complicated with acute closure and 2 with threatened closure Gianturrco-Roubin stents were implanted and the patency of the vessels were maintained.(ABSTRACT TRUNCATED AT 250 WORDS)

Reperfusion arrhythmias in acute myocardial infarction.

In 27 patients with acute myocardial infarction (MI) and their infarct-related coronary artery being completely occluded who received thrombolytic therapy or percutaneous transluminal coronary angioplasty, reperfusion was confirmed by immediate coronary angiography in 24. Reperfusion arrhythmias (RA) occurred in 19 (79.2%) of the patients, including ventricular arrhythmias in 13 (54.2%). Ventricular fibrillation and sustained ventricular tachycardia developed in 2 (8.4%) and accelerated idioventricular rhythm in 5 (20.8%), the latter was a reliable indicator of coronary recanalization. Transient sinus bradycardia or AV block occurred in 10 (66.7%) of the 15 patients with inferoposterior MI. The occurrence of RA was not related to the duration of ischemia; ventricular RA was also not related to the location of MI and the occurrence and severity of ischemic arrhythmias before reperfusion.

[Micro-vitreoretinal surgery for the treatment of complicated retinal detachment].

Micro-vitreoretinal surgery combined with scleral buckling and intraocular tamponade were used in the treatment of 108 cases of complicated retinal detachment, including 5 cases with turbid media, 14 cases with traction bands, 53 cases with severe proliferative vitreoretinopathy, 13 cases with giant tears, and 23 cases with posterior retinal breaks. After a follow-up of 3-28 months, 69 cases (63.9%) achieved total retinal reattachment. Visual acuity was improved in 69 cases, unchanged in 24 cases, and decreased in 15 cases, 47 cases obtained visual acuities of over 0.05. The surgical procedures in different groups were discussed.

[Clinical characteristics and coronary angiographic findings of spontaneous angina].

The clinical characteristics and coronary angiographic findings in 50 patients with spontaneous angina (SA) were studied. Coronary angiography did not disclose any severe fixed narrowing in 7 patients with SA only; the episodes of SA in these patients is the result of subtotal occlusion of a major coronary artery induced by spasm. 43 patients with both SA and effort angina (EA) had more severe coronary obstructive lesions. In these patients, the localization of electrocardiographic ischemia during episodes of SA and EA were always identical, indicating that the episodes of SA are related to the coronary narrowing lesions. Our study showed that incidence of SA was highest with 90% stenosis of the ischemia related artery, suggesting that besides coronary spasm, physiological coronary vasoconstriction or increase of coronary vasomotor tone could play an important role in the production of SA. In addition, turbulent flow, stasis, platelet aggregation and transient thrombosis might be individually responsible for episodes of SA in patients with severe coronary stenosis.

[X syndrome--report of six cases].

Six patients (2 males and 4 females, mean age of 46 years) with X syndrome were reported in this paper. All patients presented with typical exertional angina pectoris. In 4 patients the angina had a variable threshold of onset, it often occurred at rest and occasionally nocturnally. The electrocardiogram during chest pain showed ST segment depression of more than 0.05-0.1 mV in all 6 patients. The treadmill or bicycle ergometer exercise test was positive in 4 cases (ST segment depression > 0.1 mV), equivocal in 1 (ST segment < 0.1 mV) in whom the 201Tl exercise myocardial perfusion scan showed sign of ischemia, and negative in 1 in whom atrial pacing at heart rate of 135 beats/min induced angina and ST segment depression of 0.1-0.15 mV. Echocardiograms and X ray chest films revealed no sign of ventricular hypertrophy or enlargement. The 201Tl exercise myocardial perfusion scan was performed in 5 patients, which showed signs of ischemia in 4 patients and suspected to have ischemia in 1. Left ventriculograms and coronary angiograms were normal in all 6 patients. Ergonovine provoking test (total dose of 0.4 mg) was negative in 5 patients, it was not performed in 1 in whom there was no evidence of coronary artery spasm by angiogram during appearance of electrocardiographic ischemic changes and chest pain. Left ventricular endomyocardial biopsy was performed in 1 patient, which showed significant smooth muscle cell proliferation in the medial layer of a small artery with diameter of 62.5 mu which produced narrowing of the lumen.(ABSTRACT TRUNCATED AT 250 WORDS)

[Effect of total saponins of Panax ginseng on hematopoietic progenitor cells in normal human and aplastic anemia patients].

Ginseng was said to be benefit for anemia in TCM. Proliferation effects of total saponins of panax ginseng (TSPG) on hematopoietic progenitor cell in normal individuals and 29 patients with aplastic anemia (AA) were observed by bone marrow culture of BFU-E, CFU-E, CFU-GM in vitro compared with methyltestosterone (MT). The results showed that TSPG might prompt proliferation of normal progenitor cells at the concentration of 20 micrograms/ml. The number of BFU-E, CFU-E and CFU-GM had increased by 37.8 +/- 2.9%, 31.4 +/- 2.9% and 33.3 +/- 4.0% over the controls respectively; furthermore TSPG was still useful to BFU-E, CFU-E growth without Epo in vitro, although the colony numbers were very lower. Otherwise MT was useless to CFU-GM. 14 of the 29 patients with AA who responded to MT showed sensitivity to TSPG in marrow culture (the rising rate of colony formation exceeded 30%), but immune-mediated AA (patient's PBMNC suppressed normal hematopoiesis) and stem cell-decreased AA (few of colony was formed) showed almost no expression for TSPG activity because of immunological suppression system and absence of progenitors.

Transcoronary chemical ablation of ventricular tachycardia.

Transcoronary chemical ablation of ventricular tachycardia was recently performed in one patient with refractory ventricular tachycardia in our hospital. The procedure was successful. The arrhythmia was cured and no recurrence occurred in a 24-month period. The clinical application and mechanism of transcoronary chemical ablation were discussed.

[Vitrectomy plus air tamponade for treatment of retinal detachment with proliferative vitreoretinopathy].

Closed vitrectomy, membrane peeling, intraocular air tamponade and scleral encircling were performed for 35 consecutive cases of retinal detachment with proliferative vitreoretinopathy (PVR), comprising 2 cases of grade C2, 4 cases of grade C3, 6 cases of grade D1, 17 cases of grade D2, and 6 cases of grade D3. 21 eyes (60%) attained anatomic retinal reattachment with improved visual acuity in 20 eyes. The authors believed that vitrectomy combined with membrane peeling was a logical approach to the treatment of retinal detachment with PVR. The surgical success was related to the extent of membrane peeling, and intraocular air tamponade was a simple and effective adjunctive measure.

Clinical evaluation of 99Tcm-MIBI SPECT in the assessment of coronary artery disease.

To evaluate the clinical efficacy of 99Tcm-MIBI myocardial tomography for detecting coronary artery disease (CAD) 115 consecutive patients who underwent both 99Tcm-MIBI single photon emission computed tomography (SPECT) and coronary arteriography were studied. Thirty-three patients without and 82 with significant coronary artery disease were documented by coronary arteriography. The overall sensitivity and specificity of 99Tcm-MIBI SPECT for detecting coronary artery disease were 96 and 87.9%, respectively. The sensitivity for identifying patients with CAD without myocardial infarction was 88%. The sensitivity of 99Tcm-MIBI SPECT for detecting individual coronary artery lesions was 86% for left anterior descending artery (LAD), 69% for left circumflex artery (Lcx) and 86% for right coronary artery (RCA), lesions respectively. In conclusion, 99Tcm-MIBI SPECT provides a reliable method for detecting CAD.

[Clinical study of 63 patients with aplastic anemia by using in vitro culture of BFU-E, CFU-E, CFU-GM and of the relation between pathogenesis and treatment].

63 patients with aplastic anemia were studied by using in vitro assay for committed erythroid and granulomonocytic progenitors from the bone marrow. The T cell-mediated effects of suppression of normal hematopoiesis were observed by PBMNC of the patients when cocultured with normal bone marrow. The stimulated effects of androgen on BFU-E and CFU-E with methyl testosterone were also studied. The results showed that the PBMNC of 44.4% of the 63 patients suppressed normal hematopoiesis. 41.3% of the patients responded to methyl testosterone (MT) and 14.3% of the patients had very obvious decrease or absence of BFU-E, CFU-E and CFU-GM without evidence of immunological effects or response to androgen. According to these findings, it may be useful for clinicians to choose better therapeutic regimens for aplastic anemia. Such as BMT for the patients with hematopoietic stem cells deficiency; immuno-suppression treatment or splenectomy may be of benefit for those who suffered from immune mediated aplastic anemia (IMAA) and androstenones chosen for those sensitive to MT in vitro. There were 15 patients with IMAA treated with immuno-suppressive agents and 19 patients sensitive to MT treated with androgens. All of them had satisfactory results.

Transcoronary ethanol ablation of experimental ventricular tachycardia after epicardial ice mapping and localizing.

Thirty-seven reproducible ventricular tachycardias (VTs) were induced in 19 dogs after the onset of myocardial infarction. The site of origin of VT was localized in 19 (59%) of 32 VTs by ice epicardial mapping. After 0.3-1.2 ml of 95% ethanol was injected into a small coronary artery supplying the arrhythmogenic area, VT was no longer inducible in 10 of 14 dogs. Intramyocardial ethanol (1-3 ml) was injected into the site of origin of VT in 9 dogs including 4 with VTs reinduced after intracoronary ethanol. Six of these VTs were not reinduced. Thus, the total efficacy rate was 84%. In 7 dogs, after injection of 0.4-1.2 ml (mean 0.5 ml) of 95% ethanol into a small normal coronary artery, the extent of the changes in ECG, CK-MB and pathology was found to be related to the size of myocardial damage and to the dose of ethanol. The smaller the dose of ethanol was given and the more distal the branch of coronary artery into which the ethanol was injected, the smaller the myocardial damage was. The data demonstrated that intracoronary or intramyocardial injection of ethanol may ablate the experimental VT induced by programmed heart stimulation in dogs after myocardial infarction, indicating that this approach may be useful and meaningful in some selected instances. However, it is necessary to limit the myocardial damage as far as possible.

[Specular microscopy of the corneal endothelium after entry of silicone oil into the anterior chamber].

Specular microscopy of the corneal endothelium in 42 cases (42 eyes) with silicone oil in the anterior chamber after pars plana vitrectomy and silicone oil injection revealed reduced cell density, pleomorphism of endothelial cells, and increased corneal thickness. The optical characteristics of silicone oil in contact with the endothelium. i.e. bright reflection, reversed luminance at the cell boundaries, and black and white contours at the margins of posterior corneal folds, were also observed.
