Systolic time intervals and ejection fraction in assessing left ventricular performance following acute myocardial infarction. Comparison of systolic time intervals and equilibrium radionuclide ventriculography.

Left ventricular performance in 54 patients with acute myocardial infarction was studied by means of systolic time intervals (STI) and ejection fraction (EF) determined using radionuclide technique. The data were correlated to clinical parameters of left ventricular heart failure and size of infarction. Thirty-seven patients were studied in the convalescent period with repeated registrations at 6, 10 and 24 weeks. Patients with left ventricular heart failure had markedly depressed EF, significantly different from EF in the non-heart failure group. STI did not show any difference. A strong correlation between infarct size and EF could be demonstrated. STI discriminated significantly transmural from non-transmural infarctions, and presented significant differences between those with normal EF (greater than 50%) and patients with EF below 35%. No significant changes could be found in left ventricular performance during the convalescent period, except for an increase in corrected preejection period (PEPc) probably due to drugs. The correlation between EF and STI was poor, the highest correlation coefficient being 0.55 between EF and the ratio PEP/LVET.

Effects of metronidazole (Flagyl) on the determination of serum ASAT on the SMA 12/60 Auto Analyser.

Therapeutic serum concentrations of metronidazole will lead to under-estimation of ASAT activity when a SMA 12/60 Auto Analyser is used. This interference is due to metronidazole entering the colorimeter through the dialyser membrane in the SMA 12/60 Auto Analyser. This drug is one of the few with a relatively high therapeutic serum concentration, absorbing light in the 340 nm range. Metronidazole will similarily affect other 340 nm methods (LDH, ALAT) available on the SMA 12/60. This interference with metronidazole on the SMA 12/60 can easily be solved by the introduction of a separate serum blank channel for the 340 nm methods.

Intoxication with sodium valproate. A case report.

Sodium valproate is a relatively new compound in the treatment of epilepsy. A female patient was admitted to the hospital after voluntary intake of 30 g sodium valproate. Her serum concentration was 4925 mumol/l, but the course of the intoxication was mild and the patient was soporous for only 12 hours after intake of the compound. Biochemical data may suggest a slight transitory liver affection.

An automatic method for Thrombotest on the 'Cobas Bio' centrifugal analyser.

The Thrombotest assay has been adapted to the 'Cobas Bio' centrifugal analyser and an excellent correlation (r = 0.9972) to results obtained on a coagulometer was found. When the centrifugal analyser is linked to a computer, this method will reduce the manual labour and time by approximately 50% when larger series are concerned.

Autoanalysis

Morphological changes in rat brain induced by L-cysteine injection in newborn animals.

A single subcutaneous injection of L-cysteine (1.2 mg/g body weight) to rats 4 days after birth was followed by atrophy of the brain which was well developed 27--32 days after the injection. It was apparent that the lesioned animals could be divided into two groups (type 1 and 2) on account of the degree of brain atrophy. In type 1, which was observed in 80% of the animals, the body weight was unchanged, but the total brain weight was reduced by about 20%. The brain structures most affected were cerebral cortex, hippocampus and thalamus, each having a 30--40% reduction in wet weight. The atrophy of the posterior part of cortex was particularly pronounced in this type of lesion. In type 2 lesion, which appeared in 10% of the survivors, the atrophy was much more severe. There was a 50% reduction in wet weight of brain and in body weight. The most prominent finding was the atrophy of the whole cortex and the hippocampus which were reduced by 80 and 60% of wet weights respectively. In this type of lesion significant morphological changes were observed in several brain regions such as caudato-putamen, thalamus, pons, medulla oblongata, spinal cord and cerebellum.

Reference values for erythrocyte acetylcholinesterase and plasma cholinesterase activities in children, implications for organophosphate intoxication.

Acetylcholinesterase and cholinesterase activity was measured in the blood of both healthy children 0-12 years of age and adults by a recently developed radiochemical method. Children less than 4 months of age were found to have lower levels for these enzymes than adults. Above this age, however, similar values were obtained for children and adults. A case report of an organophosphate intoxication is presented and is discussed in the light of our results.

Acetylcholinesterase

A rapid method for the determination of urea stable lactate dehydrogenase on the 'Cobas Bio' centrifugal analyser.

The urea-stable lactate dehydrogenase assay has been adapted to the 'Cobas Bio' centrifugal analyser. It was found that incubation of the serum samples with 2 mol/l urea for 2 min was sufficient for inhibition of the LD liver isoenzymes. Under these conditions enzyme activity of cardiac origin was mainly unaffected. The method is fully automated, rapid and inexpensive.

Centrifugation

Radionuclide imaging in the diagnosis of liver trauma.

Thirty-three consecutive patients were examined on the clinical suspicion of traumatic rupture of the liver. In 13 patients liver rupture or liver haematoma was found on scintigraphy. In 5 of these patients the scintigraphic observation was verified by surgery. In two of these patients rupture of the spleen was suspected by the clinician, but rupture of the liver was found instead.

Diagnostic Errors

Subphrenic abscess.

A series of 39 patients with subphrenic abscess is presented. Nine of the patients had primary abscesses. In 27 patients, the abscess occurred postoperatively, after various abdominal operations, most frequently after gall bladder and gastric surgery. The value of combined lung-liver scintigraphy is stressed, as is promptly established and adequate drainage, using a selective surgical approach. There were 4 deaths in the series.
