Early infantile variant of Krabbe globoid cell leucodystrophy with lung involvement.

An 8-week-old boy presented with a history of irritability, progressive feeding difficulty, generalised weakness, tachypnoea, and minor motor seizures. The clinical course was characterised by rapidly progressive respiratory failure, and neurological deterioration culminating in death at age 15 weeks. Electron microscopical examination and histological studies of the lung showed the presence of numerous intra-alveolar and a few interstitial macrophages. Enzyme studies and subsequent histopathological studies on brain confirmed the diagnosis of an unusual variant of Krabbe globoid cell leucodystrophy.

Chlamydia trachomatis and pneumonia in infants: report of two cases.

In two cases of pneumonia associated with Chlamydia trachomatis in infants the symptoms began in the second week of life and the illness was severest at 4 weeks of age. Both infants were afebrile. One had a history of conjunctivitis. Both presented with a characteristic staccato cough and tachypnea but little evidence of peripheral airway obstruction. Chest roentgenograms showed interstitial and alveolar pulmonary infiltration in hyperexpanded lungs. The serum IgM concentrations were markedly elevated. C. trachomatis was cultured from specimens from both infants and one mother, and titres of antibody to the organism were substantially elevated in one infant and one mother.

A new form of nucleoside phosphorylase deficiency in two brothers with defective T-cell function.

Two brothers, age 9 and 11, respectively, have marked deficiency of nucleoside phosphorylase associated with defective T-cell function and normal B-cell function. Unlike the previously described five patients with this syndrome, each of these children has sufficient NP catalytic activity in their red blood cells (below 1% of the normal level) to be visualized after electrophoresis and staining for the enzyme. Their healthy sibling has normal NP activity and a normal isozyme pattern. The nonconsanguineous parents have about half-normal NP activity, but their electrophoretic patterns differ from each other's and from those of their affected children. These findings are consistent with genetic heterogeneity at the NP structural gene locus, resulting in compound heterozygosity for two different abnormal alleles.

Acute fenitrothion poisoning.

A technician was accidentally exposed to the organophosphorus insecticide fenitrothion and subsequently treated with pralidoxime for the autonomic, somatic and psychiatric manifestations of intoxication. Although long-term therapy with pralidoxime is not recommended, this patient required and obtained symptomatic benefit from its prolonged use. Erythrocytic and plasma cholinesterase activities had been monitored prior to exposure and were monitored throughout treatment and following recovery. Continual monitoring of cholinesterase activities of individuals occupationally exposed to organophosphorus ester insecticides and early diagnosis are essential.

Lethal interaction of ubiquitous insecticide carriers with virus.

Large quantities of presumably nontoxic petroleum oil by-products are introduced into the environment as pesticide dispersal agents and emulsifiers. An increase in viral lethality with a concomitant influence on the liver and central nervous system occurs in young mice previously primed with such chemicals.
