[Echo-color Doppler in the study of hypothyroidism in the adult].

Color-Doppler US was performed on 20 patients with sub-clinic hypothyroidism which had been confirmed by laboratory tests. In all cases, color-Doppler US showed increased parenchymal flow, whose semiology was similar to the one known as "thyroid inferno" and currently associated, in the literature, with diffuse hyperfunction conditions. Quantitative measurements yielded no further element for differential diagnosis, while showing high flow speeds which were similar to those in hyperfunction. On the basis of consequent physiopathologic considerations, hypervascularization, as observed in hypothyroidism, is likely to be referred to the hypertrophic action of TSH, which was reported as high in all patients. In conclusion, the color-Doppler "thyroid inferno" pattern, which has been to date considered as specific of thyroid hyperfunction, has lost part of its diagnostic specificity, and further investigation--e.g. hormonal titers, scintigraphy--is needed for an unquestionable diagnosis to be made.

[Flowmetric analysis of thyroid diseases: hypothesis on integration with qualitative color-Doppler study].

After premising the value of qualitative color-Doppler analysis in the characterization of the main thyropathies through the identification of 4 main reference patterns, the authors investigate whether the quantitative information obtained by means of spectral analysis can improve the predictive value of color-Doppler US. One-hundred and seventy-six patients were examined and divided into 4 groups according to qualitative color-Doppler patterns, as reported in the literature. For each patient, flowmetry included the peak systolic frequency shift and the diastolic frequency shift. The Doppler shift was seen to differ greatly within each semiology class, with overlap of values between the various qualitative classes and between the different diseases. In conclusion, flowmetry provides no more useful diagnostic information than color-Doppler US in nodular or diffuse thyroid diseases.

[Echo-color Doppler in thyroid disease].

One-hundred and sixty-six patients affected with thyroid diseases were examined, together with 41 healthy volunteers, by means of color-Doppler US. Four main reference patterns were identified: no appreciable glandular flow (I); presence of signal in perinodular location (II); peri- and intranodular flow (III); increased vascularization (IV). In 72 cases, parenchymal and nodular goiters, with no signs of thyroid hyperfunction, were characterized as belonging to pattern III, while pattern II was observed in 57 nodular lesions; only in 7 patients was pattern III detected. Hyperfunctioning autonomous nodules exhibited pattern III in 10/11 cases and the same pattern was followed by the six carcinomas. Pattern IV resulted peculiar to Basedow disease and to glandular hyperplasia related to high values of circulating thyroid hormone (6 cases). When color-Doppler specificity in typifying the major thyreopathies can be confirmed, the possibility is actually in sight of reducing, in the near future, the number of thyroid scintigraphies and of US-guided needle biopsies.

[Echotomography in the diagnosis of bone tumors. Work in progress].

Fifteen patients with positive conventional radiographs for morpho-structural neoplastic involvement of bone, were systematically submitted to US with a high-frequency probe (7.5 MHz). US evaluation was aimed at testing, relative to conventional radiography, the sensitivity of the former technique in recognizing the main elementary signs of bone involvement--e.g., cortical interruption, periosteal reaction, and extraosseous spread. In 13 cases where conventional radiography had shown cortical bone involvement, US always confirmed the radiographic findings, showing cortical interruption in all cases and allowing a preliminary evaluation of lesion size in 10 cases. Morphological and structural alterations of bone profile--i.e., interruption or evident thinning of the hyperechoic line corresponding to the cortical profile--were the main and preliminary signs of pathologic skeletal involvement. US is suggested as a complementary and integrative technique to conventional radiology, because it allows elementary signs of morpho-structural alterations of the cortical bone to be identified, and a preliminary evaluation of tumor size to be made.

Approach to ultrasound in oncology.

In a multicenter investigation carried out in 13,590 patients, 85 primitive and secondary abdominal neoplastic lesions were detected, the incidence being 0.6%. On the basis of this investigation, the role of ultrasonography in the clinical diagnosis, staging, and follow-up of oncologic diseases is summarized. Ultrasonography is employed for three basic steps in the staging of neoplastic lesions: 1. detection of lesions, 2. diagnosis of their nature, 3. balance of extension (TNM). Ultrasonography provides a lower diagnostic accuracy than CT and MRI as the balance of extension is concerned, but it yields reliable results for the diagnosis of malignancy if completed by ultrasonography guided percutaneous biopsy. The possibility of ultrasonography in the follow-up of neoplastic lesions and its applications in the treatment of some tumors are discussed.

[The evolution of dental echography: clinical and experimental research. The ultrasonic anatomy of the teeth and periodontal tissues].

Early clinico-experimental findings with real-time B-mode Echography of the odontological district are reported. On the basis of previous in vitro application references and using a unidimensional A-Mode technique, preliminary data of in vivo ultrasound semeiotics regarding the tooth and its periodontal structures are outlined experimentally. Although current "in vivo" experience relates exclusively to front central and para-central teeth, it can be hypothesised that further technical and methodological developments of echographic instruments may permit systematic employment of ultrasound diagnosis in the odontological sector.

[Does CDDP have radiosensitizing properties in the treatment of solid tumors in advanced stage?].

Various experimental and clinical studies have signalled the radiosensitizing properties of CDDP (Cis-dichloro diammine platinum). Twenty eight patients with advanced solid tumours were included in a pilot study incorporating classic fraction (2.00 Gy X 5 weeks) radiotherapy and weekly doses of 30 g/sq.m CDDP. A complete response was obtained in 21 patients with 11 of them NED at a mean follow-up period of 4-16 months. The toxicity of the treatment was low and no kidney damage or ototoxicity was encountered. The results obtained by the combined treatment are encouraging but a correct assessment will require a randomised trial on a bigger patient sample.

[Diffuse liver diseases: possibility or limitations of echography?].

Reports on echography as a diagnostic tool in diffused liver diseases are quite numerous in the literature. As it is shown by the analysis of such reports there is no general agreement as far as including echography into the diagnostic protocol of liver diseases is concerned. Reliability of echography for the assessment of fat liver is quite ascertained, while its role in the diagnosis of acute and chronic hepatitis is still quite controversial. On the basis of personal experience, percent incidence of the most relevant echographic features is checked retrospectively and their diagnostic reliability is evaluated. The role of echography in the follow-up of sclerotic liver diseases is discussed as far as the evaluation of parenchymal structure, of associated features and of complications is concerned. Echographic data should always be completed by clinical data, laboratory tests and histological examination, during diagnosis and follow-up of patients with liver diseases.

[Possibility of using B-mode echography in the early diagnosis of the carotid localization of atherosclerosis].

B-Mode ultrasonography is an important complementary tool in the early diagnosis of carotid pre-atheromatous vascular disease. The authors report their personal experience concerning 86 patients affected or not by neurological transitory or persistent symptoms, and point out two main semeiological remarks of endoluminal pathologic conditions. The findings obtained are related to the more recent physiopathological and pathological-anatomical data related the growth and the evolution of atheromatous lesions of the carotid district.
