Determination of thyroid volume by ultrasound from the neonatal period to late adolescence.

The volume of the thyroid gland was determined by ultrasonography in 256 euthyroid subjects aged 0-20 years in Brussels, an area with borderline iodine intake (median urinary iodine: 6.8 micrograms/dl). The volume of each lobe was calculated separately using the formula of an ovoid (Depth x Length x Width x pi/6). The total thyroid volume was obtained by summation of the volume of both lobes. In neonates, mean volume (SD) was 0.84 (0.38) ml and the distribution was asymmetric, skewed towards elevated values (median: 0.76 ml); the volume was best correlated with body surface area (P less than 0.01). Thyroid volume significantly increased (P less than 0.001) until the age of 8 without being influenced by sex and thereafter varied widely: it increased from 2.7 (0.8) ml in prepubertal subjects aged 8-11 years to 11.6 (4.4) ml in late pubertal aged greater than 17 years. This increase was significantly correlated not only with chronological age but also with pubertal stage and seemed to happen early, with the onset of the first clinical signs of puberty. At all ages, the volume of the right lobe was slightly higher than the left lobe but the difference was not significant.

Differences and similarities in the treatment of diffuse goiter in Europe and the United States.

In two separate studies, members of the American Thyroid Association (ATA), and the European Thyroid Association (ETA) were surveyed by questionnaire on their management of Graves' disease. The aim was to determine how expert clinical thyroidologists employ three different therapies which are available for this disorder. Herein we summarize, compare and contrast similarities and differences in the results of these surveys. For the index patient, radioiodine (RAI) was the therapy of choice for 69% of ATA respondents but only 22% of ETA respondents. In contrast, only 30.5% of ATA respondents chose antithyroid drugs as first line therapy compared to 77% of ETA respondents. There was consensus on the relative lack of a role for thyroidectomy. The implications of these differing approaches to the treatment of hyperthyroidism due to Graves' disease are discussed.

In three separate studies, members of the American Thyroid Association (ATA), the European Thyroid Association (ETA), and the Japan Thyroid Association (JTA) were surveyed by questionnaire on their management of Graves' disease. The aim was to determine how expert clinical thyroidologists employ diagnostic procedures and the three different therapies that are available for this disorder. In this report, we identify, summarize, compare, and contrast similarities and differences in the results of these surveys in these three different regions of the world. In general, ATA members used fewer diagnostic tests than did their European or Japanese colleagues. For the index patient, radioiodine was the therapy of choice for 69% of ATA respondents but only 22% and 11% of ETA and JTA respondents, respectively. In contrast, only 30.5% of ATA respondents chose antithyroid drugs as first-line therapy compared to 77% of ETA and 88% of JTA respondents. There was consensus on the relative lack of a role for thyroidectomy except for narrow indications. The implications of these differing approaches for the diagnosis and treatment of hyperthyroidism due to Graves' disease are discussed.

Contribution of thyroid ultrasound and serum calcitonin to the diagnosis of congenital hypothyroidism.

UNLABELLED: To elucidate the role of thyroid ultrasound (TU) in the diagnosis of congenital hypothyroidism (CH), we compared 1) TU and thyroid scintigraphy (TS) in 6 CH newborns and 2) TU results in the 6 CH newborns, in 8 newborns with "false positive" results at screening, in 13 CH children aged 2 mo to 12 yr treated since the neonatal period and in 235 controls aged 0-12 yr. RESULTS: 1) In all 6 CH newborns with no thyroid uptake at TS, TU evidenced small posterior hyperechogenic masses in the thyroid area [Vol: 322 +/- 180 (SD) mm3]; 2) In all normal controls and in the 8 "false positive" cases at screening TU showed normal thyroid structures. The thyroid volume was 831 +/- 383 mm3 in normal newborns and progressively increased with age. In the older CH children, TU also demonstrated the hyperechogenic masses, but their volume barely increased with age: as a consequence, the difference between the volume of the masses in CH patients and the thyroid tissue in controls, already significant in newborns (p less than 0.01), markedly increased with age. The exact nature of these masses is unknown; they could represent poorly vascularized ultimobranchial remnants containing the calcitonin - secreting cells: this hypothesis is supported by our finding that serum concentrations of calcitonin (measured by a sensitive extraction method) (mean +/- SD, pg/ml) were lower in the CH patients (2.9 +/- 1.5) than in controls (13.0 +/- 6.9; p less than 0.001) at birth. In conclusion, in all cases of CH, TU showed abnormal structures in the thyroid area. TU and TS provide complementary information in the diagnosis of CH, and TU should be routinely performed in all newborns suspected of CH to avoid unnecessary use of TS in unaffected infants.

Calcitonin

Health and social inequities in Belgium.

This paper presents two different yet complementary on-going studies related to the understanding of the mechanisms leading to social inequalities in health. The first part is devoted to a differential morbidity survey held in southern Belgium. It confirms that striking differences exist in the period around birth between social categories, and between the three districts under study. In a multivariate approach, differences remain between the social categories and between the district samples, which classically studied socio-demographical, behavioural and medical characteristics cannot fully explain. The role of cultural factors is analysed and discussed through the concept of 'health culture' and alternative hypotheses are reviewed in the light of the results. The second part reviews the studies conducted on the so-called avoidable mortality in the EEC and more specifically in Belgium. The concept of avoidable mortality is discussed, as well as its utility from the standpoint of the present concern on social inequalities. Differences between EEC countries are large, and even within Belgium there are important disparities between the districts. The role of health care supply has not been demonstrated yet in these two contexts. For Belgium, it appears that a major part of the unequally distributed mortality is constituted by causes of death considered as avoidable. Moreover, the most discriminating causes of death are overrepresented in socially deprived districts. The two perspectives are confronted in order to delineate perspectives for future research and operational outcomes for policy making and interventions.

Current trends in the management of Graves' disease.

Members of the American Thyroid Association were invited to participate in a survey of the management of Graves' disease. One primary case and several variations were provided, which differed in respect to age, sex, goiter size, severity, etc. The questionnaire was based on the format used in a similar survey of members of the European Thyroid Association. The aim of the survey was to determine 1) how expert thyroidologist employ diagnostic procedures for this disorder, and 2) the choice of therapy of the three treatment options and its manner of implementation. Questionnaires were sent only to clinically active members. The overall response rate was 62%. Data analysis was possible on 52% of members surveyed and was performed using SPSS and a specific Fortran program. In the laboratory evaluation of the primary case a radioiodine uptake, scan, serum total T4, and basal TSH were requested by 92%, 47%, 83%, and 66%, respectively, with 84% of respondents using an ultrasensitive TSH assay. For management of the primary case, radioiodine treatment was the first choice of 69% of the respondents. Antithyroid drugs were used briefly (3-7 days) before 131I by 28%, whereas 41% said they would employ thioureas after 131I. Of those using 131I, 66% tailored the dose to achieve euthyroidism as the goal of therapy, while 34% aimed for hypothyroidism requiring T4 replacement. Only 30% of respondents chose thioureas as a first line of treatment (72% propylthiouracil; 28% tapazole). The duration of drug therapy was a predetermined fixed interval for 80% of the respondents, with 90% treating for 1-2 yr. Other specific trends in diagnostic approach and therapeutic preferences were identified for the eight variations on the primary case problem.

Perinatal morbidity in Belgium.

This paper reviews the results of published and unpublished epidemiological studies on perinatal morbidity in Belgium. The most frequently studied variables are birthweight and gestational age, which are not only morbidity indicators but also morbidity and mortality predictors. Congenital anomalies are studied in Belgium through the results gathered by two regional birth defects registries, which have been operating since 1979. Major results of these registries are presented and discussed with relation to their implications on prevention policy. Subjective and reported health indicators are less commonly available for epidemiological studies, although some were collected in an interview survey conducted in 1981 in three districts of Wallonia. The results show important social inequalities and geographical disparities. Finally, the long-term consequences of perinatal problems are presented and discussed. A prospective study conducted at Leuven University Hospital on infants born between 1981 and 1986 suggests that improving survival of low-birthweight infants with appropriate perinatal care will not increase the number of handicapped children.

[Maternal and child health in the Fourth World. Initial results of a socio-epidemiological survey].

This paper presents the results of a qualitative survey of young mothers in deprived conditions, which was conducted as a part of a broader socio-epidemiological research on health inequalities amongst mother and child. This survey reveals that this group is not solely distinguished in terms of socio-economic categories. Cultural dimensions play also an important role in explaining their health related behaviour. The survey was aimed at determining the logic and principles of coping with health and sickness. The specific attitudes and behaviour of the survey group was described through fourteen dimensions (definition and perception of health, prevention, relations with professionals, networks used...). The results show that any approach of the health problems of this group has to take its specific health culture into account and that this approach is inevitable in any effort to promote health and reduce social inequalities for the so-called "Fourth World".

[Carbon monoxide poisoning in the Brussels metropolitan area. Home survey technics and proposals for action].

A previous study, preliminary to the organisation of a prevention campaign against carbon monoxide poisoning in Brussels, has confirmed the magnitude of this problem and shown the following two main risk indicators: the "age of the building" and the "nationality of the resident". Therefore, the actions conducted as part of this campaign, were principally focused on run down areas with high concentrations of immigrants. What's more, the possibility has been offered to every diagnosed victim to benefit from a free expertise of his equipment, in order to get a technical diagnosis. The analysis of our data related to all visits conducted from october 87 to april 88 shows that: 1. "resident's nationality" is not a risk factor, strictly speaking. When socio-economic standard is held constant, the risk appears to be identical in all nationalities; 2. most of the victims are underprivileged families; 3. among all apparatuses having been considered responsible for the accidents, around 2/3 are water-heaters; 4. if all kinds of apparatuses are considered, the most frequent faults seems to imply the users' responsibility, lack of maintenance, defective connection with the smoke evacuation system), but for water-heaters, which are responsible for the majority of accidents, the main cause appears to be linked to a defective installation, namely the ventilation of the room not being in accordance with the officials norms.

[Geographical variations in mortality due to diseases amenable to medical intervention in Europe--Commission of European Communities: atlas of avoidable deaths].

The urgent need to develop measures of the outcome of health-care services has led to the collaboration of 10 countries of the European Community in the production of the European Community atlas of avoidable death (1974-1978). Seventeen disease groups were chosen for which it was considered that death within specified age groups should be either wholly or substantially avoidable when appropriate medical care is sought and provided in good time. Mortality from these causes was compared across 360 health-service administrative areas in the participating countries. For all diseases there was considerable variation in mortality both within and between the countries of the European Community and it is suggested that high levels of mortality from these causes should be viewed by health authorities as warning signals of potential failures of health-care services. Work is in progress on a further edition of the Atlas for the years 1980-1984. Changes in avoidable mortality over time could indicate which health authorities have persistent problems and which authorities are succeeding in reducing avoidable mortality.

Comparison of the attack rates for acute myocardial infarction in two Belgian towns AMI attack rates in two Belgian towns. Belgium is divided in two main regions: Flanders in the North with a Dutch-speaking population, and Wallonia in the South with a French-speaking community. From 1982 onwards, a register of acute myocardial infarction has been in operation in Ghent, a Flemish town, and in Charleroi in Wallonia, following the procedures of the MONICA (Multinational Monitoring of Trends and Determinants in Cardiovascular Diseases) study. Annual attack rates of myocardial infarction are presented for a 5-year period. During this period, the annual attack rates for men are 10-20% higher in Charleroi than Ghent. In women, the ratio between the two cities is less clear. The results of this community registers confirms the regional differences observed previously in Belgium using other epidemiological techniques.

Seroepidemiological study on sexually transmitted diseases and hepatitis B in African promiscuous heterosexuals in relation to HTLV-III infection.

A seroepidemiological study was performed on HTLV-III, T. pallidum, C. trachomatis and Hepatitis B virus (HBV), in Butare, Rwanda, among 33 female prostitutes, 25 male customers of prostitutes, and 60 male and female controls. As compared with female controls the prostitutes had a higher prevalence of antibodies to HTLV-III (29/33 versus 4/33, p less than 0.001), T. pallidum (TPHA: 27/33 versus 6/33, p less than 0.001; RPR: 19/33 versus 2/33, p less than 0.001; FTA-Abs: 27/33 versus 5/33, p less than 0.001) and C. trachomatis (IgG IF: 31/33 versus 13/33, p less than 0.001). HBV serological markers were more often detected in the prostitutes than in the female controls (31/33 versus 18/33, p less than 0.001) although HBs antigen carriage rate was similar in both groups. As compared with male controls, the male customers of prostitutes had more frequently detectable antibodies to HTLV-III (7/25 versus 2/27, p = 0.05), and a positive RPR (10/25 versus 1/27, p less than 0.01). Among the 118 individuals studied, odds ratios and trend analysis disclosed a significant association between HTLV-III seropositivity and a positive TPHA, RPR, FTA-Abs, Chlamydia IgG IF test and serological markers to HBV. No association was found between HTLV-III seropositivity and HBs Ag carriage. This study suggests that HTLV-III has to be considered as an infectious agent transmitted among promiscuous Central African heterosexuals by sexual contact and/or parenteral contact with unsterile needles used for STD treatments.

[Avoidable mortality in Belgium].

The concept of avoidable mortality leads to an attempt at using specific mortality rates as output measures of health services. The analysis covered 43 Belgian districts between the years 1974 and 1978. Two Belgian areas were compared along a dimension defined by two axes of a correspondence factor analysis: Flanders which is associated with low SMR of avoidable mortality and Wallonia which has high rates. The persistence of high mortality in Wallonia was confirmed. Factorial scores for each district were used as indexes for geographical heterogeneity. Variations in these indices, including patient consultation rates and technical medical procedures, remained even after adjustment for socio-economic differences.

Treatment of bacillary dysentery: a comparison between enoxacin and nalidixic acid.

A prospective randomized study has been conducted in Rwanda to compare the efficacy of enoxacin versus nalidixic acid for the treatment of bacillary dysentery in adults. Seventeen patients received nalidixic acid and 21 received enoxacin. All isolates (37 shigella, 1 campylobacter) were sensitive in vitro to the two drugs. All patients showed a favourable clinical and bacteriological response and the results obtained with both drugs were comparable. Enoxacin seems to be a good alternative to nalidixic acid for bacillary dysentery due to multiresistant shigella.
