[Variable treatment of distal radius fractures. Results of a prospective study].

The paper presents a new strategy for treatment of distal radius fractures. The strategy is based on a prognosis-related classification model from which an appropriate treatment strategy is derived. Between 1 January 1987 and 31 December 1988, a series of 205 patients with a total of 207 fractures were treated. An average of 12 months after the accidents 198 persons (96.6%) with 200 fractures were re-examined. Wrist function was good or excellent in 83.5%, fair in 15.5% and poor in 1% of the patients studied.

[Conservative treatment of distal radius fractures. Results of a retrospective study].

Between 1 January 1984 and 31 December 1986, 175 patients with a total of 178 distal radius fractures were treated conservatively in our department. An average of 25 months after the accidents, 124 patients with 126 fractures (71%) were re-examined. Wrist function was good or excellent in 52.4%, fair in 41.3% and bad in 6.3% of the patients studied. Anatomical results were slightly better, with the patients' subjective evaluation markedly better. The results are analysed and the causes of the unsatisfactory results pointed out. Thereafter, the indications for conservative treatment are discussed and a regimen presented. Conservative treatment alone is not justifiable.

Adolescent

[Dupuytren's contracture--is partial palmar aponeurectomy still justifiable?].

Partial or total palmar fasciectomy was applied to one or both hands of 42 patients, between January 1st, 1980, and May 31st, 1985. Follow-up checks were made on 35 patients, the average postoperative period amounting to 41 months. Rates of recurrence were 33 per cent, following partial palmaraponeurectomy, but only five per cent, following total removal. No difference was established regarding postoperative complications. In 67 per cent of all follow-up checks the functionality of hands was good to very good, with poor functionality being recorded from only seven per cent of all cases.

Adult

[Sarmiento functional treatment of humeral shaft fractures. Results and experiences].

Functional treatment according to Sarmiento was applied to 20 cases of humerus shaft fracture, in the course of three years. All fractures healed with good functional result. The approach proved to be effective. Functional treatment, using Brace, was indicated even for fractures in the proximal or distal third of the humerus or in cases of complete failure of the N. radialis. The author's results are described and discussed.

[Results of surgical therapy of open tibial shaft fractures].

Surgical was applied to 36 cases of open tibial shaft fractures at the author's department, between January 1st, 1975 and May 31st, 1986. The osteitis rate amounted to 8.3 per cent, while pseudo-arthrosis was recorded from 14 per cent of all cases. All infections and pseudo-arthroses were cured, and no amputation was necessary. Two and more operations were required in 28 per cent. The cases described in this paper are classified by medical records and outpatient documentation.
