[Pappataci fever].

Sandfly fever virus is known to cause pappataci fever. The sandfly fever virus belongs to the Genus Phlebovirus (family: Bunyaviridae) and is endemically found in areas of South Europe, Asia and Africa. In Germany, pappataci fever is only described in connection with travelling to endemic areas. We report on a 15 year-old girl suffering from sandfly fever virus infection after vacation in Turkey. The initial symptoms started with fever for about three days, frontal headache, nausea and arthralgia. After a short time of clinical improvement symptoms recurred and our patient entered hospital with signs of severe meningitis. Liquor analysis showed a lymphocytic meningitis. Due to multiple insect bites on her legs sandfly fever was suspected. Blood analysis confirmed an acute infection with sandfly virus Sicilian from which she completely recovered. ELISA and immunoblot analysis revealed an infection with sandfly virus serotype Sicilian, which was not encountered with meningitis so far. Our case report illustrates that due to increased tourism sandfly fever virus infection has to be considered as a cause of aseptic meningitis in travellers.

Lipids and risk assessment.

Because a standard lipid panel can misrepresent lipoprotein abnormalities that contribute to atherosclerosis, more detailed laboratory analysis should be considered in a patient whose level of risk is unclear. This is especially important for patients who have a family history of premature coronary heart disease of uncertain etiology. Such testing is analogous to performing a more advanced imaging procedure when a plain x-ray lacks the sensitivity to define an anatomic derangement. In selected situations, measuring apolipoprotein B, lipoprotein (a), low-density lipoprotein, or high-density lipoprotein subfractions, or homocysteine can markedly clarify cardiovascular risk. This review describes seven men, themselves cardiologists, who had misinterpreted their own risk based on conventional lipid tests. With further evaluation, one of these men was reassured that he had lower risk than he had feared. Six of the men had greater risk than previously recognized.

Valproate therapy induces von Willebrand disease type I.

To investigate the increased tendency of hemorrhage in patients receiving valproate (VPA) therapy, we studied coagulation parameters in 30 randomized children of a group of 83 children receiving antiepileptic drug (AED) therapy. Besides a reduction in fibrinogen concentration and platelet count, we observed a significant decrease in factor VIII-complex. A decrease in factor VIII:C was noted in 33%, a decrease in von Willebrand factor (vWF:Ag) was noted in 83% and a decrease in ristocetin-cofactor activity (vWF:Rcof) was noted in 66% of the children. We classified a von Willebrand syndrome type I in 67% of our patients receiving VPA therapy. Sixty-three percent of patients had a history of bleeding, and 23% had a prolonged bleeding time. We compared our results with those of a control group and of a group of patients with congenital von Willebrand disease (vWD), from which patients with multimer types II and III were excluded. Because coagulation parameters in patients with congenital vWD are similar to those receiving AED therapy, we designated the increased tendency to hemorrhage as VPA-induced vWD. The decrease in coagulation parameters were not dependent on either VPA dose or period of administration. In patients receiving VPA therapy, this result must be considered, especially during surgical intervention and after traumatic events.

A new digital tomosynthesis method with less artifacts for angiography.

A new nonlinear reconstruction method for tomosynthesis is described. This method is suited for "dilute" objects, i.e., objects in which most of the voxels have negligibly small absorption. Images of blood vessels filled with contrast material approximate this condition if the background is subtracted. The technique has been tested experimentally using a wire phantom and a prepared human heart. The results show significantly less artifacts than the well-known back projection. It is possible to get diagnostic image quality with a few projections. The reconstruction algorithm can be realized with dedicated real-time hardware.
