[Solitary nontuberculous mycobacterial infection of a cervical lymph node caused by Bartonella henselae (cat scratch disease)].

Cat scratch disease (CSD) is a common cause of subacute regional lymphadenopathy. Cats are the principal reservoir of Bartonella henselae, the etiologic agent in most cases of CSD. Because the histopathological findings are typical but not specific, a wide range of other agents must be considered in differential diagnosis. We report on a patient with a solitary swelling of the neck which remained over months with initial features suggestive of unspecific lymphadenitis, but who, after careful history taking and further serological testing, had cat scratch disease.

[Localization of primary small cell carcinoma with liver metastasis: a rare combination of colonic adenocarcinoma and undifferentiated small cell carcinoma].

Neuroendocrine or small cell cancer (SCC) is a rare tumor, accounting for less than 1% of all colorectal cancers. There is a high rate of metastasis in SCC. Overlying adenomas are commonly present in colorectal SCC. We present a case of a 67-year-old female patient with liver metastasis of SCC. Initially, the primary tumor was not found and the patient underwent chemotherapy. Ten months later, an adenocarcinoma of the right hemicolon was endoscopically diagnosed due to anal hemorrhage and right hemicolectomy was performed. Microscopic examination revealed that the adenocarcinoma was combined with an undifferentiated carcinomatous component. Immunocytochemistry was positive for synaptophysin and chromogranin. In our case a very rare combination of colonic adenocarcinoma and SCC within an overlying adenoma was found.

MIB-1 immunohistometry of follicular adenoma and follicular carcinoma of the thyroid gland.

OBJECTIVE: To analyze cellular proliferative activity, MIB-1 immunopositivity of normal tissue (n = 20), follicular adenoma (n = 30) and follicular carcinoma (n = 32) of the thyroid gland was analyzed by means of immunohistometry. STUDY DESIGN: Immunohistochemical reactions were performed on 3-micron sections from routinely formalin fixed and paraffin embedded surgical specimens using an indirect peroxidase method. The rate of immunostained cells was determined using the CM-2 TV image analysis system (Hund, Wetzlar, Federal Republic of Germany). Forty viewing fields (1.94 mm2) were measured with 20:1 objective magnification. An average of 5,965 cells were assessed in each case. RESULTS: Mean MIB-1 immunopositivity was higher in follicular carcinoma (average, 2.30%) and follicular adenoma (0.58%) than in normal thyroid tissue (0.14%). The distribution of single values differed significantly between groups (P < .001). To test the suitability of MIB-1 immunohistometry for the differential diagnosis of follicular adenoma and follicular carcinoma, different four-field tables with varying thresholds were calculated. Using a threshold of 0.9%, follicular carcinoma could be detected with a sensitivity of 75% (24/32) and a specificity of 83% (25/30). If a specificity of 90% is required (27/30), the sensitivity of the test decreases to 69% (22/32), based on a threshold of 1.1%. CONCLUSION: As some overlap of single values has to be considered, MIB-1 immunohistometry, although presenting new insights into the proliferative potential of thyroid lesions, is of only limited value for the differential diagnosis of follicular lesions in routine surgical pathology.

[The permanent endourethral prosthesis (Wallstent) for the treatment of posterior urethral strictures].

We present our experience with the self-expanding urethral endoprosthesis Wallstent for the treatment of recurrent posterior urethral strictures. The stent is made of a surgical-grade stainless steel alloy wire, woven into braided mesh and shaped to a tubular form. It was originally developed for use in endovascular stenoses. Immediately before implantation of the stent an internal urethrotomy with subsequent urethral dilatation is done. So far, 23 patients have been treated by this new technique of therapy, with follow-up periods of up to 25 months (mean 16.8 months). The endoprosthesis has been tolerated very well by all patients. Successful treatment of the strictures is confirmed by the physiological uroflow obtained and the absence of residual urine after emptying of the bladder. We have seen no recurrences of stricture. After 6 months, urethroscopic control examination demonstrates complete epithelialization of the urethral stent. One year after implantation the endoprosthesis is completely incorporated. The original diameter of 21.5 Charr is maintained, which allows subsequent transurethral endoscopic manipulation if necessary. Our encouraging results so far give reason to hope that this is a simple, safe and recurrence-free treatment for posterior strictures of the urethra.

[Polysegmental andersson lesion in ankylosing spondylitis (roentgenological-histological synopsis) (author's transl)].

Basing on roentgenological observation of the course of the disease, and on postmortem examination, of a patient with polysegmental Andersson lesions (known as spondylodiscitis) in ankylosing spondylitis, the article reports on the histological and often also clinico-roentgenological possibility of differentiating between an inflammatory and non-inflammatory type of this destructive lesion. The non-inflammatory type of Andersson lesion reflects a fracture due to fatigue, or its sequel, eg pseudarthrosis, in the stiffened axial skeleton. In this particular patient, renal osteopathy had favoured the genesis of the disease.

[Lymphangitis carcinomatosa of the lung in carcinoma of the prostate: frequency, radiological appearances and clinical significance (author's transl)].

Lung metastases from carcinoma of the prostate usually manifest themselves as radiologically diffuse lymphangitis carcinomatosa. Unlike the pathological findings, a radiological diagnosis is relatively uncommon. We have investigated this retrospectively in 230 patients; in 192, radiographs were available for study. In over 10% lung metastases were found, mostly in the form of lymphangitis carcinomatosa. In view of the bad prognostic significance and therapeutic implications of lymphangitis carcinomatosa, it is important to bear this in mind and to look for it radiologically.

[Rapid intra-operative histological diagnosis of bone tumours (author's transl)].

The main deficiency of rapid intra-operative histological diagnosis of bone tumours is the possibility of biopsy error. If this is avoided, rapid sections can be made from tumour biopsy specimens either with the simple freeze-microtome or a cryostat. The specially thin cryostat sections are particularly suitable for revealing cytological details and may be superior even to paraffin sections. On the other hand, cell-deficient tumours with a great deal of intercellular substance or carcinoma metastases can be diagnosed more reliably from usual frozen section than cryostat section. Considerable experience is required to avoid misdiagnosis. One of the most difficult problems is the diagnosis of chondromas of the long bones and the pelvis. The term "semimalignancy" is a suitable one for many forms of those giant-cell tumours which have special tendency towards recurrence even after years. Dangers and limitations of the rapid diagnosis of bone tumour are demonstrated most clearly on purely histomorphological assessment, without knowledge of history, clinical findings or X-ray diagnosis. This is especially striking in the case of fracture callus which may be misdiagnosed as osteosarcoma.

[Histomorphology of the sacroiliac joint in ankylosing spondylitis and relevance to treatment (author's transl)].

The sacro-iliac joint of a patient who had been ill with ankylosing spondylitis for half a year was investigated histologically at necropsy. Two basic histological phenomena were apparent: inflammation and aggressive proliferating chondroid metaplasia at the chondro-osseous junction and in the subchondrium. Joint cartilage degeneration, joint cartilage fusion, capsule ossification and subchondral bone apposition were observed as epiphenomena at the sacro-iliac joint. Both basic morphological phenomena are due to the same cause which has not yet been clarified. They progress independently of each other with varying intensity. This assumption explains differing opinions and the relation of inflammation and pathological ossification of the skeletal axis as well as the unsatisfactory objective results of treatment. Only a combination therapy directed against both morphological basic phenomena will result in an objective therapeutic success and not only in a symptomatic improvement of ankylosing spondylitis.

[Immunoblastic sarcoma with clinically primary manifestation in the testis (author's transl)].

Three own observations of immunoblastic sarcomas with clinically primary manifestation in the testis are reported. In all cases an already generalized tumor stage was evident. In two patients a combined surgical, radiological, and chemotherapeutic treatment was performed with satisfactory success. The essential functional-morphological and clinical criteria of the immunoblastic sarcoma were summarized and discussed.

Transmission and scanning electron microscopy of experimentally induced thyroid tumors in rats.

Irradiation of the neck region of 10 days old Wistar rats with 300 rads X-rays and their subsequent treatment with MTU leads to the development of thyroid tumors. The ultrastructure of the thyroid tumors induced was studied by transmission (TEM) and scanning electron microscopy (SEM). In follicular carcinomas, cylindrical or high columnar cells with abundant microvilli, well-developed ergastoplasmic reticulum and a hypertrophied Golgi complex predominate. The cytoplasm of the tumor cells of papillary carcinomas was composed of large pleomorphic cisternae, irregular in shape mitochondria, dense bodies, phagolysosomes and lamellar or multivesicular structures. Scanning electron microscopy revealed in the control animals a regular pattern of thyroid follicles tied to each other by tissue bridges and covered by set of blood and lymph vessels. The relief of the follicular carcinomas was irregular indicating follicles different in size and shape. The internal surface of the neoplastic follicles was also irregular as result of deep invaginations and intrafollicular cell growth. The apical region of the normal and neoplastic thyroid follicular cells was covered with abundant microvilli protruding into the colloid.
