The manufacturing process of recombinant factor VIII, recombinate.

In summary, this brief report documents the cell culture and rAHF purification systems used to produce rFVIII and has focused on the unique characteristics of the cell line, the production process, and purification system used. This review emphasizes the general philosophy used throughout the research and development of this innovative and complex therapeutic agent, in order to provide a safe and efficacious rAHF in the management of individuals with hemophilia A. This review provides a brief description of the in-process system and controls that have been incorporated to meet the high standards and safety objective.

Tissue-specific expression of insulin-like growth factor binding protein-3 protease activity during rat pregnancy.

The abundances of insulin-like growth factor binding proteins (IGFBPs) in sera and tissue homogenates of rats at days 12, 15, and 18 of pregnancy were determined by ligand- and immunoblotting. As in serum, IGFBP-3 was abundant in day 12 uterus, placenta, and fetuses, and decreased by day 15. On day 18 of pregnancy, IGFBP-2 was predominant and IGFBP-3 was less than 25% of day 12 values in fetus and placenta, and was undetectable in uterus and decidua. In contrast, IGFBPs in the nonreproductive tissues did not change significantly. IGFBP-3 was more abundant in muscle than heart and liver, and was not detected in lung, kidney, or brain. The decrease in IGFBP-3 in serum and reproductive tissues between days 12 and 15 of pregnancy was temporally related to the appearance of IGFBP-3 protease activity. Proteolytic activity was detectable only at low levels in brain, liver, and spleen, and was undetectable in lung, heart, muscle, and kidney. The specific protease inhibitors that blocked IGFBP-3 proteolytic activities in pregnant rat serum and decidua were virtually identical and suggested inhibition of a divalent cation-dependent tryptic-like serine protease. Furthermore, exposure of bovine IGFBP-3 (in nonpregnant bovine serum) or radiolabeled recombinant human IGFBP-3 to day 18 pregnant rat serum, decidua or uterus resulted in the generation of IGFBP-3 fragments with the same apparent Mrs (29-31 K and 18-23 K). We postulate that tissue-specific degradation may be as important as synthesis in determining IGFBP-3 abundance, and that the dramatic changes in IGFBPs in reproductive and fetal tissues may cause changes in IGF availability which are necessary for rapid tissue growth and differentiation.

[Views on simple surgical aspects of gastrointestinal cancer in Norway].

A questionnaire was used to study various aspects of the surgical treatment of cancer of the oesophagus, pancreas, stomach and colon/rectum in Norway. 48 of 51 departments replied. Half of the departments perform radical operations for oesophageal cancer and nearly all do total gastrectomies and low anterior resections for rectal cancer. Palliative surgery is seldom performed for oesophageal cancer, but is frequently performed for gastric cancer. Obstruction of the colon is relieved preferably by primary resection. Low anterior resection is usually performed when a free resection margin of 2 cm can be achieved. Jaundice is relieved palliatively preferably by endoscopy. University hospitals perform total gastrectomy to a greater extent than other hospital and have greater faith in radical operations for pancreatic cancer. Otherwise there are no conspicuous differences of opinion between the different categories of hospitals.

[Prevention of thrombosis. Surgical departments in Norway].

Routine perioperative thrombo-embolic prophylaxis was studied by means of a questionnaire to departments of abdominal, orthopedic, urological and gynecological surgery. 171 departments in university, central and county hospitals answered, giving a response rate of 92%. It was found that 90% of the departments in all hospital categories used routine prophylaxis. This practice was most frequent in orthopedics and least frequent in urology. Dextran was preferred in all three hospital categories. Central and county hospitals also applied heparin and heparin/dihydroergotamin. Heparin was always given as 5,000 IU subcutaneously twice daily. Heparin/dihydroergotamin was equally often given as 2,500 IU as 5,000 IU twice daily, irrespective of the kind of surgery. Indications of thrombo-embolic prophylaxis varied within each surgical speciality. The prophylaxis regimen was chosen mostly on medical grounds. The convenience of the nurses was seldom and economic differences only infrequently considered of importance for the choice of prophylaxis regimen.

[Skin closure. A prospective randomized study].

This prospective randomized study included 100 patients with inguinal hernia and 66 patients operated on for acute appendicitis. In half of each group the skin was closed with a continuous subcuticular resorbable suture, and in the other half with a monofilament nylon matress suture. The patients were followed up after three weeks and again after three months. There was no obvious difference in cosmetic result between the two methods of skin closure. In the patients with inguinal hernia or non-perforated appendicitis there was no increase in postoperative infection of the wound after subcuticular suture. In the case of perforated appendicitis there may have been an increase in infection of the wound after subcuticular suture. Continuous, subcuticular resorbable suture used on the right indications can give a good cosmetic result and few complications of the wound. Further more, a separate outpatient visit for removal of skin sutures or staples can be avoided.

Appendicitis

A new intraperitoneal metastasis from thymoma.

Distant metastasis from thymoma is rare. A case with an intraperitoneal metastasis from a thymoma 14 years after removal of the primary tumor is reported. The metastasis was sited in the left hypochondrium and adherent to the spleen, but with no connection to the diaphragm or retroperitoneum. To our knowledge, this location of distant metastasis from thymoma has not been described previously. The malignant potential of thymoma is discussed, and the pattern of reported metastases is summarized.

Anemia, Aplastic

Coracoid process transfer for acromioclavicular dislocation.

Seventeen patients with complete separation of the acromioclavicular joint were operated on acutely with transposition of the coracoid tip to the clavicle. All the patients were examined after a mean follow-up time of 7.5 years. Twelve patients had good function, 3 had slight pain in extreme positions, and 2 had poor function with pain at rest. Eleven patients had the screw removed, 2 of them because of deep infection. In 3 patients the coracoid tip fragmented during the operation. Two patients had an unsatisfying cosmetic result. As long as nonoperative treatment gives equal or better long-term functional results, we do not recommend this operation in acute dislocations.
