[Plastic filling of bleeding duodenal ulcers in the course of truncal vagotomy with pyloroplasty according Finney].

The procedure of plastic filling of bleeding ulcer of the duodenum during vagotomy procedure and pyloroplasty by Finney was developed and used in 26 patients. The study of early postoperative results showed steady hemostasis in all patients and no recurrences of bleeding. Follow-up results up to 3 years were studied in 22 patients. No relapses of ulcer disease were recorded. Thus truncal vagotomy with pyloroplasty by Finney and plastic filling of bleeding ulcer is an effective method for hemostasis and adequate treatment of duodenal ulcer.

[Functional status of musculo-venous pump of shin in various types of of operations for varicose disease].

Functional conditions of the muscular-venous pump of the shin were studied in 280 patients with varicose veins before surgery, in 164 patients in remote terms after various operations and in 54 healthy subjects on the base of guantitative plethysmographic estimation of hemodynamic parameters. At the stage of compensation all volume indices of the bloodflow were close to normal values; in the stage of decompensation the substantial increase of volume effectiveness and volume flow velocity was observed at carrying out the test for direct deep blood flow when walking. It was determined that in natural condition for venous hemodynamics the adaptation mechanisms of musculo-venous pump were not realized. The operation of Linton upsets the function of musculo-venous pump of the shin. The Kocket operation does not eliminate the pathologic overload when working due to insufficient radicalism in liquidation of perforation escape (steal phenomena). The subfascial ligation of perforating veins of the shin through the minor approach, used in clinical practice, proved to be sufficiently radical and does not impair the function of the musculo-venous pump of the shin.

[Emergency heart surgery in the practice of the flight surgeon].

The authors analyze the materials from departments of emergency and advisory aid (sanitary aviation) over the past 20 years. Emergency care was rendered to cardiosurgical patients with cardiac and pericardial traumatic injuries in the surgical department of central district hospitals. They consider the reasons for a consultant's call, diagnostic aspects and board surgeon's organizational policy. The organizational policy in rendering cardiovascular patients an emergency surgical care consists in compulsory surgical interventions at the site of an accident. Patients with early postoperative complications should be evacuated to specialized departments. When the complications can occur in the absence of adequate management in the postoperative period in the central district hospitals the patients should be also evacuated to the specialized departments. Improving the qualification of a general surgeon is a basis for successful surgical aid rendered to patients with cardiac injuries.

[Use of ferromagnetic rheological suspensions in the treatment of non-formed intestinal fistulas].

Data on treatment of 42 patients with nonformed intestinal fistulas with the help of ferromagnetic rheological suspensions are presented. The authors have shown that their method tested under experimental conditions is close to universal since it does not require selection of the individual obturator. The use of the method is not possible in patients with one or multiple abscesses of the abdominal cavity, through which the formed fistula passes.

[Therapeutic fibro-bronchoscopy with the sanative controlled high-frequency jet ventilation].

Sanative effect of the target high-frequency jet oxygen ventilation was utilized at fibro-bronchoscopy of the bronchial tree. The procedure improved external respiration in 81.5% of patients with marked disorders of tracheobronchial permeability while in control subjects no effect was observed. A marked clinical response and high oxygenating potential of the technique (a 102.3% increase in pO2) make in useful for wide clinical introduction.

[Use of sanative fibrobronchoscopy with controlled high-frequency pulmonary ventilation after operations on the thoracic organs].

The authors developed a method of sanative fibrobronchoscopy with the use of directed high-frequency stream ventilation with oxygen. A total of 108 procedures were conducted in 64 patients for removal of pulmonary complications after operations on the thoracic organs. The positive results of the method were confirmed by authentic improvement of the indices of external respiration function and increase of blood pO2 by 102% on the average. No improvement of similar indices was encountered in patients of the control group, while the blood pO2 reduced by 13.4% on the average. It is concluded that the method is very effective and safe, as compared to the traditional method of sanative fibrobronchoscopy in patients after interventions on the thoracic organs.

[Multiple relaparotomies].

Under analysis was the material of the department of urgent and consultation medical aid (sanitary aviation) for the last 20 years. It was found that under conditions of surgical departments of central regional hospitals multiple relaparotomies made up 12.5% of all reoperations. It was shown that results of the multiple relaparotomies were dependent on urgent coming of a consultant and active surgical treatment on the spot. Reconstructive operations for intestinal fistulas must be performed under conditions of a specialized proctological department.
