[Thermoradiotherapy as a component of desmoid tumor management: 10-year experience].

Data on radio- and thermoradiotherapy of 83 patients with extra-abdominal desmoid tumors are discussed. In a group of 57 patients followed up for 10 years or less, the relapse-free survival rates, in thermoradiotherapy-treated cases, were significantly higher (74.4% and 28.6%) than in those receiving radiotherapy (9.3% and 57.1%). Monitoring tumor temperature during local hyperthermia is a factor of relapse-free survival of vital importance.

[Thermoradiochemotherapy of inoperable soft-tissue sarcoma].

Three groups of patients with inoperable soft-tissue sarcoma received preoperative radiotherapy (57), thermoradiotherapy (102) and thermoradiochemotherapy (16) (n=175). Five year recurrence-free survival in group 1 was 37+/-7%, group 2 48+/-6%, and group 3 - 56+/-1,7%. Patients survived 5 years and more in group 3 (60+/-2%), group 2 - 50+/-7%, and group I 44+/-8% (p>0.05). Local hyperthermia used in conjunction with radio- and chemoradiotherapy was followed by a significant rise in the rate of complete and partial tumor regression.

[Results of repeat large-scale dissection of soft-tissue sarcoma following non-radical removal].

The reports deals with the data on repeat large-scale dissection of soft-tissue sarcoma following non-radical removal in 23 patients (13 males and 10 females , aged 14-75; mean age 42+/-30.2 yrs). Repeat surgery was carried out 3-6 weeks after primary one. Tumor progression was registered in 8 (34.8%): local recurrences which required radical intervention 4 (17.4%), distant metastases - 6 (26.1%) (lung - 3; liver - 1; lung and mediastinal lymph nodes - 1; lung and lumbar vertebra - 1). During the study, 5 patients died of tumor progression; all of them revealed distant metastases while 2 had local recurrences. Overall 5-year survival in 23 patients was 78%; whereas disease-free one - 65%.

[Range of combined treatment of locally advanced skin cancer].

Due to combined use of surgery, chemo- and radiotherapy, 58.8% of patients with locally advanced squamous cell carcinoma survived for 5 years. More organ-saving operations could be performed as a result of administering cisplatin, bleomycin and 5-fluorouracil chemotherapy in conjunction with radiation and subsequent surgery. Greater extent of tumor excision and microsurgery involved lower incidence of relapse. Yet, the preliminary results of our combined treatment pointed to relatively high frequency of objective response matched by lower incidence of relapse which calls for further investigation.

[Thermoradiotherapy in locally extensive soft tissue sarcoma].

The report discusses 138 cases of soft-tissue sarcoma: 92 patients initially received thermoradiotherapy, 46-radiotherapy. Local electromagnetic hyperthermia was found to potentiate the antitumor effect of ionizing radiation and to be followed by a significant increase in the frequency of complete and considerable regression of tumor, as compared with exposure to total doses of 30-32 Gy (66-70 Units). As a result, 5-year survival rate in cases of post-thermoradiotherapy conservative surgery rose to 54 +/- 5.2%, as compared with 26 +/- 6.8% after conservative surgery + radiotherapy. The percentage of patients who survived more than 5 years after conservative surgery + thermoradiotherapy or radiotherapy was 37 +/- 7 and 23 +/- 7.8, respectively.

[Ultrastructural features of synovial sarcomas and their prognosis].

25 synovial sarcomas in patients with various duration of life were studied electron-microscopically. Quantitative correlation between dark and clear tumour cells is the most informative prognostic index in patients operated on because of these sarcomas: the more numerous are dark cells in the tumour, the worse is the prognosis. The appearance of intermediate filaments in the cytoplasm of epithelioid tumour cells and the presence of secretory material in the intercellular spaces are the sign of a less favourable prognosis.

[Prognosis in patients with malignant fibrous histiocytoma of the soft tissues].

The paper deals with a retrospective clinico-anatomical study in 108 cases of malignant fibrous histiocytoma which identified 18 factors influencing the development of recurrences and metastases as well as survival. Computed data were used to form groups suitable for establishing individual prognosis. The latter was done by summing up the relevant specifically-numbered factors. Credibility for prognosis of recurrence development was 85.7%, metastatic development--82.9, three-year survival--71.4 and five-year survival--95.8%.

[Intraosseous highly differentiated osteogenic sarcoma].

A man of 20 developed well-differentiated intraosseous osteogenic sarcoma of the femur. Clinico-roentgenologic, computed tomography and histological findings revealed a number of features uncommon for the tumor: (1) a torpid course, (2) histological appearance typical for parosteal sarcoma, (3) low malignant potential.

[Case of desmoplastic fibroma of the femur].

A rare case of femoral desmoplastic fibroma that provoked essential difficulties in diagnosis is described. Histological and electron microscopic examination allowed the differential diagnosis between fibrosarcoma and desmoplastic fibroma.

[Surgical treatment of the intrathoracic metastases of synovial sarcoma].

The results of an analysis of 13 cases showed that isolated intrathoracic metastases of synovial sarcoma may be treated surgically if there are no recurrences. Since response of metastases to preoperative chemotherapy was registered in 3 out 6 patients, it is suggested that research in this problem should be recommended. A similar approach should be used in postoperative antitumor medication of cases of newly-detected lung metastases.
