Dioctyl sodium sulfosuccinate (Aerosol OT) has been used in a remarkably short period to obtain rumen ciliate-free cows. Two 30-g doses of Aerosol OT given on consecutive days appeared to effectively eliminate all types of rumen ciliate protozoa without harming the health of the host animal. Measurement of the rumen metabolic parameters of gas production, carbon dioxide to methane ratio, pH value, volatile fatty acids, ammonia, and in vitro cellulose digestion, along with total amylolytic streptococci counts of the rumen contents, showed that conditions in rumens of defaunated animals are normal 4 days after such treatment. It appears that such animals may be used in ruminal studies that require defaunated animals.

We have utilized new methods of dialyzer-augmented whole blood and plasma exchange tranfusion in the treatment of hepatic coma. The method employs the new fast flow Buselmeier shunt so that the exchange can be done from a peripheral radial artery shunt site rather than a shunt site in a more major vessel, such as the brachial artery. The method employs in-line dialysis of citrated (CPD) blood or plasma to normalize pH, electrolytes, and blood sugar while infusion heparinization decreases heparin requirements. The application of a single roller pump to the administration and withdrawal lines (placed in opposite directions) helps equalize inflow and outflow where whole blood exchange is done while identical opposing pumps accomplish the same with plasma exchange. The administration of albumin and metaraminol bitartate prevents hypotension due to fluid shifts or compartmental venous dilatation. A closed circuit which does not require disconnection throughout the 30- 60-minute whole blood exchange or the 2 1/2-hour plasma exchange decreases the risk of septic contamination to both medical staff and patients.
When frequent hospital admissions of patients with acute congestive heart failure who also had alcoholic cardiomyopathy were observed at John Cochran Veterans Administration Hospital, St. Louis, Missouri, nursing interventions were developed in an effort not only to reduce the frequency of hospitalizations but also to improve the patient's clinical condition. Over an 18-month period, 26 patients with alcoholic cardiomyopathy were followed in a modified bedrest program in the patients' homes. The program was individualized, based on each patient's stage of cardiomyopathy. Agressive nursing interventions reversed significantly (p less than .01) the condition in 14 patients. These patients' recovery prompted a one-year follow-up with continuing close medical and nursing support. Data showed significant (p less than .01) positive findings one year later.
Providing comprehensive orientation programs that prepare nurses for their role as staff nurses is an integral aspect of retention. Therefore, it is vital that staff development educators assess the effectiveness of their nursing orientation programs. In this article, the authors employ a utilization-focused evaluation format to assess a nursing orientation program and, based on the results, offer recommendations for strengthening such programs.
BACKGROUND: Despite the fact that advance directives are legal documents that allow people to direct their future health care, few Americans have enacted them. Because lack of knowledge about what they are, and confusion regarding the process needed to complete one are the primary barriers for completion, patient education can be the key element in promoting the use of advance directives. CONCLUSION: This article offers some teaching strategies nurses may use to address patients' ignorance and confusion and help patients understand that, although advance directives do not help people make end-of-life decisions, they serve as a mechanism by which a person's wishes may be carried out.
