Lateral epicondylitis: correlation of MR imaging, surgical, and histopathologic findings.

PURPOSE: To determine the value of magnetic resonance (MR) imaging in the clinical management of chronic refractory lateral epicondylitis. MATERIALS AND METHODS: Coronal three-dimensional Fourier transform, multiplanar gradient-recalled-echo, and fat-suppressed sagittal images were obtained in 33 patients. Twenty of these patients underwent surgical débridement and/or primary tendon repair and were included in the correlative study. Surgical and pathologic reports were reviewed to determine the location and gross characteristics of the tissue. RESULTS: Findings at MR imaging correlated with the surgical findings of primary degeneration of the extensor carpi radialis brevis (n = 20). Histopathologic examination demonstrated neovascularization, disruption of collagen, and mucoid degeneration without inflammation. CONCLUSION: The use of MR imaging in patients with recalcitrant lateral epicondylitis assists in surgical planning. The definition of tendon degeneration and degree of tear, as depicted on MR images, correlate well with surgical and histologic findings.

Ulnar collateral ligament injury in the throwing athlete: evaluation with saline-enhanced MR arthrography.

PURPOSE: To determine whether magnetic resonance (MR) arthrography of the elbow can demonstrate precisely an ulnar collateral ligament (UCL) abnormality in the throwing athlete. MATERIALS AND METHODS: Forty college-level and professional throwing athletes (age range, 18-40 years) underwent saline-enhanced MR arthrography of their injured elbows. MR findings were compared with the surgical findings. RESULTS: Saline-enhanced MR arthrography was positive in 24 of 26 individuals with UCL tear confirmed at the time of surgery. Eighteen (95%) of 19 complete UCL tears and six (86%) of seven partial UCL tears were diagnosed with MR arthrography. Two false-negative findings and no false-positive findings were obtained. CONCLUSION: Saline-enhanced MR arthrography of the elbow is a new application of a previously described technique used in the shoulder. It is useful for demonstration of subtle and gross UCL abnormalities in the throwing athlete.

Myositis ossificans traumatica.

Myositis ossificans is a benign condition of heterotopic bone formation. It is a generalized term for four separate clinical entities. Although not a rare disease process, diagnosis often remains difficult. This article reviews the various forms of myositis ossificans, as well as the pathology, diagnostic tests, and treatment options.

Diagnostic arthrogram of a Salter I fracture of the proximal humerus in a newborn.

Proximal humeral fractures occurring during breech delivery are rare and are generally Salter-Harris Type I physeal fractures. The absence of a radiographically demonstrable metaphysis attached to the proximal fragment of these fractures often makes them exceedingly difficult to diagnose at first. A recent case demonstrates the usefulness of arthrography and videofluoroscopy as an adjunct to plain radiography in diagnosing such injuries.
