[Databank-oriented physician generated medical record as the basis for outcome quality in rehabilitation medicine].

A new approach is presented for the medical reports in rehabilitation medicine. It is realized with the integration of formatted textblocks into a client-server based workflow. This technology is able to reduce hardware and costs. The data material, extracted from the daily routine of the hospital, is characterized by high quality and it supports statistics and quality management. It is a uniform database for rehabilitation medicine and for rehabilitation research. Since 1996 the medical report software has been established in all rehabilitation hospitals accredited by the LVA Württemberg pension insurance agency.

[Preliminary results with digital luminescence radiography in radiotherapy].

Port films in radiation therapy suffer from low image contrast und exposure errors. We examined the potential of computed radiography with a storage phosphor system (FCR-901/Siemens Digiscan) to overcome these drawbacks. Port films of various treatment sites were obtained for both, conventional films and digital storage phosphor screens with a DuPont cassette and steel intensifier screens under identical exposure settings. We examined port films from a linear accelerator and from a 60Co source. We found a superior contrast in the digital images and an almost total elimination of exposure errors. The differences between digital and conventional images, however, were less pronounced with films from the 60Co source. Unsharp mask filtering (medium kernel size, moderate enhancement) further improved image quality. Several sources of artefacts in the digital images could be described.

[Biperiden abuse as a partial factor in polytoxicomania].

We found 16 patients (15%) taking the anticholinergic biperiden because of its psychotropic action, occasionally, rather frequently or regularly, among a subgroup of 120 drug-dependent patients (drugs of the barbiturate and amphetamin types) out of a studied total of 194 imprisoned male addicts. These biperiden abusers suffered without exception from polytoxicomania associated with drug dependence and alcoholism. Most prominent was drug dependence on drugs of the morphine type. We could not prove a case of an isolated "primary" abuse of biperiden. Direct medical prescription was a rather secondary factor in procuring this preparation, in contrast to analgesics, tranquilisers, barbiturates and clomethiazol. Increase of biperiden abuse is due, on the one hand, to a generally noticeable tendency to polytoxicomania, and on the other hand to a change in Federal German drug prescription rules effective 1 August 1986 according to which fenetylline hydrochloride, a sympathomimetic, is now subject to medical prescription. Other centrally acting anticholinergics were unknown among this group of patients and were not abused. The results are discussed on the basis of available literature.

[Analysis of a patient population of the neurologic-psychiatric admission department of a central prison hospital].

This article represents an attempt to analyse the multivarious and problematic groups of neurological and psychiatric inpatients admitted to a central hospital for prisoners in Baden-Württemberg. This special ward for such patients is unique in its character in the entire Federal Republic of Germany. Patients are differentiated according to diagnosis, length of stay, duration of imprisonment, reason for referral, number of patients referred, and kind of treatment. The four most strongly represented groups of diagnosis consist of the patients with psychogenic reactions, drug addiction, endogenous psychoses and psychopathies. These differ from one another according to treatment regimen, reasons for referral, and mode of hospitalisation. Statistics are given reflecting the problems of non-German, mentally abnormal prisoners and of their suicidal or para-suicidal actions.
