Use of imipramine hydrochloride for treatment of urospermia in a stallion with a dysfunctional bladder.

An 8-year-old stallion was evaluated because of recurrent urinary tract infections and chronic intermittent urospermia. After extensive diagnostic testing, it was hypothesized that the stallion had a reflex dyssynergia of the bladder and urethral sphincter. Initial attempts to manage the urospermia included semen fractionation, semen collection after voluntary urination, and use of semen extenders. None of these efforts reliably yielded a quality ejaculate. Administration of imipramine hydrochloride (1.2 mg/kg of body weight, PO, 4 hours prior to semen collection) was initiated in an attempt to enhance bladder neck closure during ejaculation. This treatment, combined with voluntary urination prior to ejaculation, resulted in ejaculates containing little or no urine. Using this protocol, 19 of 20 mares bred during the subsequent 2 years became pregnant. By the third year, the bladder dysfunction had progressed, and the urospermia was no longer manageable. Bladder catheterization, followed by manual expression of the bladder per rectum, were necessary prior to each semen collection to obtain a urine-free ejaculate. Three-and-a-half years after initial examination, transitional cell carcinoma of the bladder with metastasis was identified, and the stallion was euthanatized. It is not known whether the transitional cell carcinoma was related to the dysfunctional bladder. Imipramine hydrochloride did not eliminate, but did reduce, the frequency and degree of urospermia in the affected stallion for approximately 2 years.

Abdominal Neoplasms

Use of pharmacologically induced ejaculation to obtain semen from a stallion with a fractured radius.

Ejaculation was pharmacologically induced in a 13-year-old Quarter Horse stallion with a spiral fracture of the radius. The owners desired to have semen from the stallion frozen prior to euthanatizing the horse, but because of the debilitating injury, standard methods of semen collection could not be used. With the stallion standing quietly in a stall, a plastic collection bag was positioned over the stallion's penis, and clomipramine hydrochloride (2.2 mg/kg of body weight, IV) was administered. Fifty-five minutes later, xylazine hydrochloride (0.5 mg/kg, IV) was administered. The stallion ejaculated 2 minutes after xylazine administration. The semen sample was of low volume and had a high concentration of spermatozoa; however, motility of spermatazoa was poor and the semen was unacceptable for freezing. The stallion was euthanatized. Semen aspirated from the epididymis after euthanasia was of similar quality to that of the ejaculated sample. Pharmacologic induction of ejaculation can be useful for obtaining semen from stallions when standard techniques cannot be applied.

Saline magnetic resonance arthrography in the evaluation of glenohumeral instability.

Sixty-five patients underwent magnetic resonance (MR) shoulder arthrography. Forty-eight of these patients underwent examination under anesthesia (EUA). MR images were retrospectively evaluated for signs felt to be imaging indicators of shoulder instability, including evaluation of various capsular measurements and the presence of glenoid labral tears, as well as Hill-Sachs fractures. Statistical analysis of the results showed that no correlation between capsular indicators with EUA-documented instability was found. However, there was a statistically significant correlation between the presence of a Bankart cartilaginous deformity (p = 0.000) and Hill-Sachs fractures (p = 0.022) with EUA-documented instability. Sensitivity to labral tears was 89% and specificity was 98%, whereas Hill-Sachs fracture detection was 69% and 87%, respectively. We believe that MR saline arthrography is of benefit in the evaluation of the anterior labrum when unenhanced MR imaging is inconclusive, and we speculate on the role of MR arthrography as a primary investigative tool.

Comparative efficacy and safety of mezlocillin, cefoxitin, and clindamycin plus gentamicin in postpartum endometritis.

The efficacy of mezlocillin versus cefoxitin versus clindamycin plus gentamicin was evaluated in 152 patients with postpartum endometritis. There were no statistically significant differences in rate of cure among the three groups (87% with mezlocillin, 82% with cefoxitin, and 92% with clindamycin-gentamicin). There were no severe adverse reactions observed in any of the three treatment regimens. Mezlocillin is as safe and effective as cefoxitin and clindamycin-gentamicin for treatment of postpartum endometritis.

Bacteriuria

Isolation of Mycoplasma species and Ureaplasma urealyticum from obstetrical and gynecological patients by using commercially available medium formulations.

One hundred duplicate endocervical specimens from obstetrical and gynecological patients were cultured for Mycoplasma spp. and Ureaplasma urealyticum. Rates of recovery of these organisms from commercially prepared A7 medium and the Mycotrim-GU system were compared. We detected 14 (93%) of the total 15 isolates of Mycoplasma spp. on A7 plates and 11 (73%) in the Mycotrim-GU system. We detected 34 (89%) of the total 38 isolates of U. urealyticum on A7 plates and 32 (84%) in the Mycotrim-GU system. The times of detection for both types of organism were similar in the two systems. We conclude that cultivation on A7 medium as described is a more cost-effective method of recovery of Mycoplasma spp. and U. urealyticum than the Mycotrim-GU system.

A comparison of the inhibitory effects of clonidine and guanfacine on the behavioral and autonomic components of morphine withdrawal in rats.

Intravenous administration of naloxone (0.5 mg/kg) to morphine dependent rats elicited classical autonomic and behavioral symptoms of narcotic abstinence including hypertension, tachycardia, withdrawal body shakes, escape attempts, diarrhea, etc. Pretreatment of dependent rats with either clonidine (3-90 micrograms/kg) or guanfacine (3-900 micrograms/kg) produced a dose-dependent reduction in the hypertensive response to subsequent injection of naloxone. Clonidine was about 12 times more potent than guanfacine in inhibiting this autonomic symptom of withdrawal. Both drugs were less effective at blocking body shakes and escapes, however, when all symptoms were combined in a ranked score, guanfacine was less effective than clonidine at reducing the ranked abstinence intensity score. Since clonidine blocked the autonomic component of withdrawal at doses more consistent with its clinical anti-withdrawal actions, it is possible that 1) measurement of behavioral signs of withdrawal in rats is a less sensitive index than is measurement of autonomic changes associated with withdrawal, or, 2) a reduction in autonomic outflow in general is most relevant to suppressing the apparent intensity of the abstinence syndrome.

Assessing the impact of cognitive differences in the treatment of obsessive-compulsives.

Demonstrated a set of research techniques that combine multidimensional scaling, a quantitative cognitive procedure, and cost outcome analysis. The multidimensional scaling results supported Reed's (1969) notion that obsessive-compulsive clients are underinclusive and did not support Foa and Steketee's (1979) contention that they are overinclusive. Significant differences in extent of follow-up therapy were found between obsessive compulsives (N = 12) with the underinclusive cognitive style and those with a normal cognitive style.

A multiple perspective analysis of schizophrenics' symptoms and community functioning.

The present investigation examined the extent of agreement among schizophrenic clients, hospital clinicians, and independent evaluators' views of the client's symptoms and community functioning status using the Symptom Checklist-90, the Denver Community Mental Health Questionnaire, and the Personal Adjustment and Role Skills Inventory. The results suggest there is significant agreement among viewpoints and that this finding is generalizable across community assessment instruments. The average shared variance among the three perspectives across the three instruments was .68. The estimates of shared variance between the SCL-90, the DCMHQ, and the PARS ranged from .54 to .85, indicating these outcome instruments measure a common phenomenon or share significant method variance. The previously assumed necessity of multiple perspective assessment as the only valid approach is challenged.

Polymyalgia rheumatica: a general practice experience.

In the eight years from April 1974 to April 1982, 10 diagnoses of polymyalgia rheumatica were made in an urban general practice. The natural history of the condition, diagnostic problems and occasional unpredictability of outcome are discussed. It is suggested that general practice research may provide answers to some unsolved questions.

Parasuicide in an urban general practice, 1970-1979.

In the 10 years from 1970 to 1979 there were 224 known episodes of parasuicide involving 158 patients (71 per cent women) registered with doctors in a group practice in a health centre in South East London. Nearly 40 per cent of patients were known to have repeated parasuicide at least once. Self-poisoning by drugs was the commonest method. Nearly three quarters of the drugs used had been prescribed by doctors and just under 60 per cent of the study patients had consulted their doctor within the 28 days preceding parasuicide. Personality disorder was the most frequent psychiatric diagnosis and was often associated with depression. There was no past or present evidence of psychiatric abnormality in 22 per cent. The most frequent precipitating cause of parasuicide was a breakdown in personal relationships. The annual total of episodes fell in the last two years of the study, but it is not yet clear whether this marks a significant development. Six of the patients died from suicide during the study period.
