Chronopharmacologic issues in space.

Times of heightened susceptibility are expressed by nonrandom patterns in the incidence of various diseases, not only along the scale of a day but also of a week and a year. Whereas these rhythms can be synchronized by the environment, their endogenicity is revealed by their persistence in the absence of time cues with a period slightly but statistically significantly different from the environmental match. In the case of some circadians, the gene involved has been identified and heritabiliy in humans determined by studies on twins. Vital signs are now amenable to automatic monitoring around the clock. When analyzed by chronobiologic software, the data provide information regarding the given individual's time structure (chronome). Such physiologic monitoring serves the multiple purposes of deriving time-specified reference norms on the basis of which rhythm alteration can detect an elevated risk early, thus prompting timely preventive action and timed treatment whenever warranted. For long journeys in space, the design of a multi-disease prophylactic pill poses a chronopharmacologic challenge. Drugs such as aspirin and carnitine, used for the prevention of strokes, myocardial infarctions and depression, all show striking chronome-dependent effects which can determine not only the presence or the absence of a desired effect, but even yield effects in opposite directions.

Comprehensive assessment of the effectiveness of nitrates with prolonged action in patients with acute myocardial infarction.

In 64 patients with extensive acute myocardial infarction, the effect of the Sustak-forte preparation (nitroglycerin with prolonged action) on central and peripheral haemodynamics and the size of the periinfarction zone was studied. A diminution of the extent of the periinfarction zone was found on the 3rd day of the disease (according to the results of precordial ECG mapping and enzymologic examinations). Analysis of the findings in 476 patients with AMI, using the method of retrospective randomization, documented a statistically significantly better effect of treatment in patients who were given nitrates than in those not receiving them.

Differences in some circadian patterns of cardiac arrhythmia, myocardial infarctions and other adverse vascular events.

BACKGROUND: Results from unpublished data on the incidence of adverse vascular events and from several published studies are reevaluated chronobiologically. METHODS AND RESULTS: Cosinor methods indicate 1. a circadian variation in the incidence of paroxysmal supraventricular tachycardia (PST), of broadly classified ventricular arrhythmia (VAr), and of atrial fibrillation (AF); 2. a statistically significant difference in the timing of the circadian rhythm of PST and VAr versus that of AF; and 3. a further difference in the timing of these rhythms from that in the incidence of myocardial infarctions (MI). Electrocardiographic records for spans longer than 24h show the extent of day-to-day variability in circadian characteristics of the given patient and indicate the presence of even lower-frequency components, notably along the scale of a week, that may underlie weekly and half-weekly patterns of morbidity and mortality. CONCLUSION: Beyond alterations in the about 1-Hz periodicity of the heart, predictable changes along the scales of the day and the week may constitute a clue to the etiopathology of a given condition and provide a basis for treatment timing. The assessment of unfavorable changes in the lower frequency components may provide a lead time long enough to prompt the institution of preventive, rather than curative, intervention.

Chronobiologic aspects of urinary gonadotropin peptide (UGP).

When using urinary gonadotropin peptide (UGP) as a marker for ovarian cancer, a circadian rhythm of large amplitude is best assessed for improving preanalytic quality control, for specifying how often and when to sample and for resolving chronobiologic endpoints. In a patient (EH, 73 y) with an ovarian (Müllerian) adenocarcinoma, overall high UGP values occurred consistently around 07:00 (standard deviation, SD = 2 h), similar to the timing observed in a presumably healthy woman. The circadian rhythm of EH averages in amplitude about 45% of the daily mean value (SD = 15%). The results indicate the desirability of deriving time-specified reference limits in health for an improved diagnostic value of UGP. The circadian UGP pattern awaits tests of applications for a chronodiagnosis of ovarian cancer, for optimizing treatment efficacy by timing according to rhythms, and conceivably for detecting earliest precancer risk.
