Electrically-induced collagen calcification in pig skin. A histopathologic and histochemical study.

Deposition of calcium salts on collagen fibres in skin of fully anaesthetized pigs was induced by exposure to direct current (d.c.). In biopsies obtained from cathode areas successively from day 1 to day 7 after exposure the histopathologic and histochemical changes before and after the initial deposition of calcium salts have been examined. For comparison skin sites with intradermal injected calcium hydroxyapatite crystals were studied in addition. Small areas of calcified collagen and elastic fibres were noted in viable tissue 2 days after d.c. exposure. In succeeding days the calcified areas enlarged with new deposits always more superficial and closer to the epidermis than the original calcium deposits. Preconditions for calcification appear to be (1) a pH change in basic direction and/or the electrochemical processes specific to the cathode area and (2) a viable tissue. Elastic fibres appear to have a lower calcification threshold than collagen fibres. A positive staining for glycoproteins (PAS) and glycosaminoglycans (alcian blue pH 2.5) was noted in the calcified collagen fibres simultaneously with the calcification. In succeeding days the intensity of the staining reactions increased. Whether changes in the glycoproteins, collagen and its intimately bound glycosaminoglycans precede the calcification or the staining reactions develop secondarily to this deposition is not known. However, seven days after intradermal injections of Ca-apatite crystals in pig skin small and large crystals were observed ultrastructurally without any relation to collagen fibrils, but the calcified tissue presented a positive PAS and alcian blue reaction from day 2. Thus the PAS and alcian blue stainings in this model develop secondary to the deposition of calcium salts.

Glycosaminoglycans in localized scleroderma (morphoea).

The composition of glycosaminoglycans (GAGs) was analyzed in skin samples of eight patients suffering from localized scleroderma, i.e., three having generalized morphoea and five localized morphoea plaques. From each patients, biopsies were obtained from sclerotic and perilesional areas, and from clinically uninvolved skin of the same region. In the perilesional areas hyaluronic acid concentration was increased (p less than 0.05), while it was decreased (p less than 0.01) in sclerotic areas. Dermatan sulfate concentration was increased in the perilesional (p less than 0.05) as well as in the sclerotic (p less than 0.01) areas. Chondroitin 4/6 sulfate was increased in the sclerotic areas (p less than 0.05). Heparan sulfate showed no changes. No major differences were found in the total concentrations of uronic acid and hexosamine. This study demonstrates that changes in GAG composition in localized scleroderma follow previously described sequences of events of inflammation and fibrosis of connective tissue.

Collagen specific amino acids in skin in localized scleroderma.

Hydroxyproline, hydroxylysine and proline were determined on skin from 18 patients with localized scleroderma (10 with localized morphoea plaque and 8 with generalized morphoea). Three skin biopsies (4mm punch) were obtained from each patient: One from the center of a sclerotic plaque, one from the perilesional area, and one (control) from unaffected skin of the same region. Clinically, the sclerosis was more pronounced (p less than 0.01) in localized morphoea plaque as compared to generalized morphoea. Patients with localized morphoea plaque had an increased concentration of hydroxylysine (p less than 0.01) and an increased ratio of hydroxylysine to hydroxyproline (p less than 0.01) in the plaques. Hydroxylysine concentration was not changed in patients with generalized morphoea. In the entire material, increased hydroxylysine concentration were related to shorter age of the plaques (p less than 0.05) and to advanced degree of sclerosis (p less than 0.05). The hydroxylysine and hydroxyproline content per mm2 skin surface, and the weight of the dried defatted biopsy cores were increased in sclerotic plaques (p less than 0.01) in localized as well as generalized morphoea. There were no changes in the hydroxyproline and proline concentrations in any of the groups. Specimens from perilesional area showed intermediate changes. The results were compared with selected cases of lichen sclerosis et atrophicus and atrophic skin diseases. The increase in hydroxylysine concentration and ratio to hydroxyproline indicate that patients with localized morphoea plaque contain an increased proportion of newly synthesized collagen in the fibrotic plaque.

Ultrastructure of skin and hair of an Egyptian mummy. Transmission and scanning electron microscopic observations.

The skin of a female Egyptian mummy, embalmed 100-300 A.D., was studied by transmission and scanning electron microscopy. After rehydration with cacodylate buffer containing sucrose, the ordinary technique for transmission electron microscopy was performed on skin of the right shoulder. In epidermal cells, mitochondria and their cristae were identified. Between epidermal cells, desmosomes, half the normal size, were found. In the dermis, collagen bundles and elastic fibres were observed. The collagen fibres had a diameter of 67-91 nm, and the band pattern showed 10-12 sub-bands per one period of 55-61 nm length. The elastic fibres had less amorphous substance than normal and a fibrillar beaded pattern. The dermal components of the skin were well preserved in spite of the mummification. Throughout the dermis and the subcutaneous fatty tissue, a number of bacterial spores were found showing a characteristic appearance. Scanning electron microscopy of hairs showed a layer of the preserving material surrounding hairs, and under it a normal cuticle was seen.

The ultrastructure of bacterial spores in skin of an Egyptian mummy.

The ultrastructure of bacterial spores found in the skin of an Egyptian mummy was studied. The mummy was embalmed during year 100 to 300 of our era. The bacterial spores showed a characteristic appearance with a core, cortex, coat and exosporium. Their survival and species could not be established.

Local panatrophy with linear distribution: a clinical, ultrastructural and biochemical study.

A young female with local panatrophy of the right-side extremities affecting all mesodermal layers--dermis, subcutaneous tissue, muscles, and bone--is described. The atrophies were strictly linear. Ultrastructurally, collagen fibrils of the reticular dermis from the atrophic area consisted of two types, a large population of fibrils with decreased diameter and round cut-surface, and a small population with increased diameter and a "flower-like" cut-surface. Collagen analysis revealed a reduced hydroxyproline concentration and content per mm2 skin in atrophic area as compared with the perilesional and regional control. Urinary excretion of hydroxyproline was normal. Glycosaminoglycan analysis showed a less reduced content of hexosamine and uronic acid in the atrophic area per mm2 skin, but no differences in the concentrations. Consistently, the gel/fibre ratio (uronic acid/hydroxyproline) was found slightly elevated in atrophic skin. The proportions of the individual glycosaminoglycans heparin, dermatan sulphate, hyaluronic acid and chondroitin sulphates were not affected. It is concluded that reduced collagen and ultrastructural changes of collagen fibrils are significant features in local panatrophy. The significance of local foetal factors affecting the somatic mesoderm is discussed.

Treatment of dyskeratosis follicularis Darier with the retinoic acid derivative Ro 10-9359 (Tigason).

The results of treatment of 29 patients with Darier's disease with the aromatic retinoid Ro 10-9359 are reported. Therapeutic tests for a period of more than 18 months seem to justify its use in Darier's disease. With a modest dose it is possible to keep the symptoms at an acceptable level, i. e., 50% clearing or more without too troublesome side effects. Because of the side effects the dosage must be fixed individually. It is pointed out that with the development of Ro 10-9359, an efficient remedy for Darier's disease has become available for the first time.

Squamous cell carcinoma of the skin and internal malignant neoplasms.

This study comprised 211 patients in whom squamous cell carcinomas of the skin (SCC) had been diagnosed in the Department of Dermatology from 1950 to 1959. These patients were followed up as of Jan 1, 1976, to establish the incidence of internal malignant neoplasms. A systematic statistical study was carried out through the population register, the central register for death certificates, and the Danish Cancer Register. These registers cover the entire country, making it possible to calculate the number of expected cases of internal cancer during the above-mentioned prolonged period of observation. The investigation did not show increased incidence of internal malignant neoplasms in connection with SCC of the skin.

Metastases in dermatological patients with squamous cell carcinoma.

Two hundred eleven patients in whom squamous cell carcinoma (SCC) of the skin had been diagnosed between 1950 and 1959 were followed up from Jan 1, 1976, to establish the incidence of metastases. A systemic study was carried out through follow-up examinations and tracing through population registers. We found 3.3% metastases in 153 patients with skin SCC and 11% metastases in 55 patients with labial SCC. Three patients with genital SCC were free of metastases. These results and the available literature on patients with SCC of osteomyelitic foci and scars from burns or x-ray treatment indicate that sharp distinctions must be made among three groups of SCC; mucocutaneous, primary cutaneous, and cutaneous SCC secondary to inflammatory and degenerative processes. The incidence of metastases in the three groups is approximately 11%, 3%, and 10% to 30%, respectively. Percentages found are important to dermatology clinics because skin SCC should be considered a malignant tumor with a higher incidence of metastases than previously assumed.

Multiple basal cell carcinoma and internal malignant tumors.

One hundred ninety-six patients with multiple basal cell carcinoma were studied to clarify the relationship between multiple basal cell carcinoma and internal malignant neoplasms. No substantial deviations between the expected number and the observed number of cases of internal malignant neoplasms were found. On examination of patients who had histories of arsenic intake, a markedly high number of internal tumors were found in women but not in men. The figures are too low to prove that arsenic treatment can cause an increase in the incidence of internal malignant neoplasm, but they do indicate that more comprehensive studies are needed.
