Early metabolic changes following destruction of the ventromedial hypothalamic nuclei.

Studies were performed to determine whether increased gluconeogenesis precedes or is necessary for increased lipogenesis after ventromedial hypothalamic destruction. Weanling male rats were injected with 14C-bicarbonate or with 3H2O to assess gluconeogenesis or lipogenesis, respectively, at various short time intervals after placement of hypothalamic lesions. Lipogenesis increased within 2 hr of lesion placement whereas gluconeogenesis did not change for at least 4 hr. The results indicate that lipogenesis increases before gluconeogenesis and, therefore, that enhanced glucose production is not necessary for increased lipogenesis to occur in these rats.

Idiopathic fibrosis of the lateral compartment of the neck.

We report a case of idiopathic fibrosis of the lateral compartment of the neck. Twenty-four years after the onset of swelling of the neck 14 years after biopsy, the patient remains only mildly sympatomatic. Histologically, it appears to be a homologue of the other idiopathic fibrosing syndromes such as retroperitoneal fibrosis, mediastinal fibrosis, sclerosing cholangitis, fibrosis of the orbit, and Riedel's struma.

Treatment of Streptococcal Pharyngitis. II. In vitro studies of antibacterial activity.

One hundred and forty isolates of beta-hemolytic streptococcus cultured from patients with clinical pharyngitis were studied by disc diffusion for antibiotic sensitivity to lincomycin, erythromycin, cephalexin and penicillin and by agar dilution to cephalexin and penicillin. All isolates were sensitive to </= 0.1 mug./ml. penicillin and </= 1.56 mug./ml. cephalexin. The disc-diffusion test was reliable in predicting the sensitivities in vitro. One strain of group A betahemolytic streptococcus was resistant to erythromycin by disc diffusion. When compared to Lancefield grouping 18% of strains were incorrectly identified as group A by the bacitracin-disc test. Cephalexin was uniformly effective in vitro in inhibiting beta-hemolytic streptococci and the 30 mug. cephalexin disc was reliable in predicting these sensitivities.

Treatment of streptococcal pharyngitis. I. Clinical evaluation.

A study was undertaken to evaluate the therapy of streptococcal pharyngitis. The compliance of 118 patients with beta-hemolytic streptococcal pharyngitis to follow-up was 72%. Of 74 patients checked by means of urine tests 66 took their oral medication. No differences were detected in the clinical and bacteriological results (>98% streptococcal eradication) after the 7th or 10th day of therapy after taking either cephalexin or penicillin.IT WAS CONCLUDED THAT: (a) for effective surveillance and follow-up special attention should be given to the uncooperative segment of the patient population; (b) a seven-day course of penicillin may be satisfactory in the eradication of BHS from the throat; and (c) cephalexin appears to be an effective alternative to penicillin for the treatment of streptococcal pharyngitis.

Correlation of tumor volume with local control in laryngeal carcinoma treated by radiotherapy.

An analysis of 37 patients with laryngeal carcinoma (T2 or greater) treated with radical radiotherapy, with surgery reserved for failure, was performed to determine if tumor volume, alone or in association with other prognostic factors, accurately predicted the probability of local control. Patient records were reviewed retrospectively and the following data extracted: age, sex, laryngeal region and number of sites involved by tumor, T and N categories, and success or failure of radiotherapy. Tumor volume for each patient was calculated from pretreatment computed tomograms by summing the products of the cross-sectional tumor area on each CT cut and the interval in millimeters between sequential CT cuts. The mean tumor volume for patients failing radiotherapy was 21.8 cm3, and the mean volume for patients primarily controlled by radiotherapy was 8.86 cm3. Tumor volume significantly predicted disease-free interval (p = .045) and outcome with radiotherapy (p = .02). The study suggests that tumor volume is a significant factor in determining the outcome of primary radiotherapy in advanced laryngeal carcinoma.

Immunophenotyping of leukaemias by flow cytometry and APAAP: a two-year comparative analysis in hospital practice.

Using a panel of monoclonal antibodies for the immunophenotyping of haematological malignancies, we have made a direct comparison of the usefulness of indirect immunofluorescence using flow cytometry (IFC) alongside the alkaline phosphatase-anti-alkaline phosphatase (APAAP) immunoenzyme method. Analysis of 400 consecutive patient samples (blood and bone marrow) over a 2-year period, resulted in unequivocal phenotyping agreement by both methods in 98 per cent of cases. The positive results accounting for 2 per cent discordance were obtained by the APAAP method. The value of the technique in the clinical management of patients with leukaemia is documented together with guidelines for clinical laboratories.
