Infertile women with and without endometriosis: a case control study of luteal phase and other infertility conditions.

OBJECTIVE: To assess the relationship in infertile women between endometriosis and luteal phase defects and other infertility conditions. SETTING: University Medical School. DESIGN: Case-control study. PATIENTS: One hundred and seventy-four infertile women with endometriosis and 174 infertile women without endometriosis, all of them studied by laparoscopy. The inclusion criteria were failure of six cycles of ovulation stimulation or abnormal preliminary study (abnormal pelvic physical, hysterosalpingographic or ultrasound findings). RESULTS: The frequency of out-of-phase biopsies (16.1% vs 16.7%) as well as other endometrial abnormalities were similar in endometriosis associated infertility (EAI) and in non-endometriosis infertility (NEI). Mean progesterone and prolactin levels, as well as the proportion of abnormal progesterone levels (37.4% vs 44.3%) and of hyperprolactinemia (12.9% vs 13.2%) were similar in EAI and NEI. The frequency of uterine myomas was similar in both groups. Sperm characteristics were not better in EAI than in NEI. Higher endometriosis stages were not associated with a higher frequency of the aforementioned conditions. CONCLUSION: Luteal phase defects, sperm abnormalities and myomas are no more common in EAI than in NEI or in advanced stages of endometriosis.

Fluctuation of cardiotocographic tracings during labor in fetal growth retardation.

The objective of our study was to analyse the fluctuation of cardiotocographic scores during labor in fetal growth retardation (FGR). The study took place at the University hospital "Principe de Asturias", Alcalá de Henares, Madrid, Spain. 170 at term FGR fetuses and 170 at term fetuses without FGR as control group were compared using modified Fischer scores, which were blindly performed at 3, 5 and 10 cm of cervical dilatation. As results we found out that the mean value of the Fischer score was significantly lower in FGR at 3 cm of cervical dilatation (8.1 +/- 1 vs 8.7 +/- 0.6), as well as at 5 cm (7.6 +/- 0.9 vs 8.1 +/- 0.7) and at 10 cm (6.7 +/- 0.8 vs 7.4 +/- 0.8). Poor prognosis cardiotocograms were also more common in FGR than in the control group in the three cut-offs points studied (7.5% vs 0.6% at 3 cm, 9.8 vs 1.8 at 5 cm and 37.9 vs 11.6 at complete cervical dilatation). The afore mentioned differences were more remarkable as labor advanced. Conclusions are that poor fetal heart rate recordings were detected in FGR at the beginning of labor when compared with control group fetuses and the differences between both groups increased while the labor prolonging.

Epidemiology of endometriosis in infertile women.

OBJECTIVE: To assess the epidemiological factors linked to endometriosis in infertile women. SETTING: Academic tertiary hospital. DESIGN: Case-control study. PATIENTS: One hundred seventy-four infertile women with endometriosis and 174 infertile women without endometriosis, all of them studied by laparoscopy. RESULTS: Women with endometriosis and infertility had a higher frequency of cycles < or = 27 days (26.4% versus 16.7%). A trend to a protective effect of smoking was detected. Social class, reproductive history, infertility history, previous abortions, age, family history, age at menarche, and duration of menstrual flow were similar in both groups. CONCLUSION: Cycles < or = 27 days was the only risk factor that seemed to be associated to endometriosis in infertile patients. However, predictive values were low. A trend to a protective effect of smoking was detected.

Levels of CA-19.9, CA-125, and CA-195 in seminal plasma and in vitro fertilization rates.

OBJECTIVE: To assess the relationship between carbohydrate tumor markers in seminal plasma and fertilization rates in IVF cycles. DESIGN: Prospective study. SETTING: Academic tertiary hospital. PATIENTS: Forty-six consecutive couples subjected to IVF in which at least one oocyte was obtained. MAIN OUTCOME MEASURES: Determination of CA-19.9, CA-125, and CA-195 in seminal plasma 1 week before IVF. RESULTS: CA-125 was higher in the group without fertilization (321 +/- 198 U/mL, mean +/- SD) than in the group in which at least one oocyte was fertilized (155 +/- 165 U/mL), whereas CA-19.9 and CA-195 were similar in both groups. However, at the cutoff 66%, lower values of CA-19.9 were found in the fertilization group rate < 66% (707 +/- 1,217 versus 1,069 +/- 1,084) and of CA-195 (67 +/- 44 versus 218 +/- 274). No correlation was found between tumor marker levels and fertilization rates. CONCLUSION: CA-125 seems to be an indicator of sperm fertilization capacity, whereas the importance of CA-195 and CA-19.9 will require further studies.

Sperm morphology analysis (strict criteria) in male infertility is not a prognostic factor in intrauterine insemination with husband's sperm.

OBJECTIVE: To assess the prognostic value of strict sperm morphology analysis in intrauterine insemination with husband's sperm (IUI) cycles performed because of male infertility. DESIGN: Prospective study. SETTING: Academic tertiary hospital. PATIENTS: Seventy-four consecutive couples subjected to 271 IUI cycles because of male infertility. MAIN OUTCOME MEASURE: Strict morphology analysis 1 month before the beginning of IUI following the criteria of Kruger et al. RESULTS: Pregnancy and no pregnancy couples were similar regarding strict normal forms (2.85% +/- 2.07% versus 3.13% +/- 2.63%), slightly amorphous forms, and the morphology index (11.79 +/- 6.06 versus 12.04 +/- 7.13). Pregnancy rates (PRs) were similar when normal forms were <4% (39.1%) or > 4% (35.7%). The PR, although higher in the group with morphology index > 10% (41.4%) than in the group < 10% (33.3%), lacked statistical significance. In pure male infertility group, mean values of morphology were similar in pregnancy and nonpregnancy group. A not significant trend was detected toward higher PRs in morphology index > 10% (50.0%) than in the group < 10% (33.3%). CONCLUSION: Strict morphology analysis 1 month before the beginning of IUI is not a useful prognostic factor in IUI performed because of male infertility.

Influence of fatty fish intake during pregnancy in the polyunsaturated fatty acids of erythrocyte phospholipids in the mother at labor and newborn infant.

BACKGROUND: Assessing the relationship between fatty fish intake, the main source of N-3 polyunsaturated fatty acids and maternal and fetal levels in erythrocytes at labor. METHODS: The fatty acid composition was studied in 64 mother-newborn infant pairs by means of capillar gas chromatography. Following a nutritional inquiry our population was classified as follows: superior intake group (SIG) (> 4 times fatty fish per month; 54.4 +/- 20.5 g/day), medium intake group (MIG) (2-4 per month; 16.5 +/- 4 g/day) and inferior intake group (IIG) (< 2 times per month; 4.3 +/- 4 g/day). RESULTS: In erythrocyte phospholipids higher levels were observed in SIG in relation to IIG in mothers regarding eicosapentaenoic (0.56 +/- 0.3% and 0.39 +/- 0.2%; p = 0.02) and docosahexaenoic acids (6.14 +/- 1.16% and 5.4 +/- 0.98%; p = 0.02) and in newborn infants regarding eicosapentaenoic acid (0.32 +/- 0.24% and 0.17 +/- 0.09%; p < 0.01). On the other hand, SIG had lower values of arachidonic acid than IIG in mother (11.15 +/- 1.61% vs 12.83 +/- 1.86%; p < 0.01) and newborn infants (16.08 +/- 3.42% vs 17.89 +/- 2.57%; p = 0.05). CONCLUSIONS: From a biochemical point of view, and taking into account the pre-existing literature, it is suggested that a minimum fatty fish intake of twice per month should be recommended (edible portion 85 g.) in order to reach the N-6/N-3 quotient usually proposed.

Crohn's disease presented by episiotomy dehiscence and perineal fistula.

A case of Crohn's disease is described which was first manifested by episiotomy dehiscence and perineal fistula after an instrumental delivery. The female newborn showed a congenital rectovaginal fistula and hexadactylia. Differential diagnosis and possible relationship between both fistulas are discussed.

Multivariate analysis of determinants of fetal growth retardation.

OBJECTIVE: To analyse the relative effect of the risk factors related to fetal growth retardation (FGR) in the central area of Spain. STUDY DESIGN: This is a case-control survey of 370 full-term gestations. Of the 370 women, 185 were diagnosed with intrauterine growth retardation, and 185 were in the normal group. All came from a central region of Spain. We took into account 25 possible risk factors. RESULTS: In the FGR group we found 12 risk factors with statistical significance in the univariate analysis (tobacco, drugs, low pre-pregnancy weight, low maternal height, little gestational weight gain, excessive physical activity during pregnancy, low socioeconomic status, low educational level, unmarried, urinary infection, mother's haematological changes, and previous infertility). After the multiple logistic regression analysis, five factors were shown to exist as independent risks (tobacco: OR, 23.50; 95% CI, 3.01-183.18; P < 0.01; low pre-pregnancy weight: OR, 4.01; 95% CI, 2.14-7.51; P < 0.001; low socioeconomic status: OR, 2.91; 95% CI, 1.72-4.90; P < 0.001; little gestational weight gain: OR, 2.52; 95% CI, 1.21-5.22; P < 0.05; and urinary infection: OR, 3.83; 95% CI, 1.49-9.87; P < 0.01). CONCLUSIONS: (i) Tobacco, low pre-pregnancy weight and low socioeconomic status are the three main cause-effect factors of relative importance in our sanitary zone for FGR. (ii) Disproportionate FGR is related more to older mothers and maternal previous illness than to the proportionate FRG.

Intrauterine growth retardation and plasma fatty acids in the mother and the fetus.

BACKGROUND: Assessing the relationship between plasma fatty acids, specially polyunsaturated fatty acids in the mother and the fetus and intrauterine growth retardation (IUGR) in an apparently well-nourished population. DESIGN: Case-control intrapartum analysis. SETTING: Academic tertiary hospital. PATIENTS, PARTICIPANTS: There were 17 IUGR newborn cases (23 mothers) and 34 newborn control cases (46 mothers). MAIN OUTCOME MEASURE: Eleven fatty acids were blindly analysed in plasma by means of gas chromatography. RESULTS: Percentage values of eicosapentaenoic acid among mothers from the IUGR group were significantly higher than controls, whereas stearic absolute values were significantly reduced. On the other hand there were no differences in percentage values nor in absolute values regarding the 11 fatty acids analysed in newborn infants. CONCLUSION: In an apparently well-nourished population IUGR is not associated with an alteration in plasma fatty acids on the fetal side. However, on the maternal side some imbalance in eicosapentaenoic and stearic acids were found which will require further study.

Intrauterine growth retardation: fluctuation of fetal pH measured between beginning and at the completion of labor.

UNLABELLED: The aim of this study was to evaluate the fluctuation of fetal pH in blood samples taken at the beginning of labor and at the moment of birth as related to intrauterine growth retardation syndrome. This is a prospective follow-up of term gestations, of which 41 were diagnosed as intrauterine growth retardation (IUGR) and 61 as normal ones. pH was measured in scalp blood sample at the beginning of the labor and in umbilical artery right after birth in both groups respectively. The rate of decrease of pH value in relation to duration of labor was determined for each case. Our results are: 1) Lower baseline pH were found in the IUGR group (pH 7.32 vs 7.34, p < 0.01), lower arterial blood pH at birth/.23 +/- 0.08 vs 7.27 +/- 0.08, p < 0.05). 2) Faster decrease of the pH during the labor (0.019 unit/hour vs 0.012, p < 0.05) as related to IUGR. CONCLUSION: IUGR fetuses are more acidotic at the beginning of the labor, and the rate of decrease of the fetal pH per unit of time during the labor is faster at least in a theoretical situation nevertheless it will require more studies if any practical applications are thought of.

Blue fish intake and percentual levels of polyunsaturated plasmatic fatty acids at labor in the mother and the newborn infant.

To assess the relationship between maternal blue fish intake during pregnancy and n-3 and n-6 polyunsaturated fatty acid percentual levels in the mother and the newborn infant. Retrospective dietetic investigation at labor regarding fatty fish intake and blind plasma fatty acid analysis. Patients participants: 35 mothers and their newborn infants were studied in the Academic tertiary hospital. Mothers were classified in "blue fish high intake" group (blue fish intake > 35 g/day), intermediate intake group (12-35 g/day) and low intake group (< 12 g/day). Eleven fatty acids were analysed in plasma by means of gas chromatography. Percentual arachidonic acid levels were lower in "blue fish high intake" group mothers. Regarding the newborn infants, no differences were found concerning maternal intake. In the "intermediate intake" group of mothers, a correlation was found for maternal and fetal levels in the following fatty acids: myristic, linoleic and linolenic acids, as well as in the sum of n-6 fatty acids. Although blue fish intake influences maternal plasma levels of arachidonic acid, it does not play a decisive role in either maternal or fetal eicosapentaenoic or docosahexaenoic acid levels. Other factors need to be considered such as n-6/n-3 metabolic interaction, other nutrient composition and especially the role of placenta and fetal liver in the metabolism of polyunsaturated fatty acids.

Factors that influence the outcome of the intrauterine insemination with husband's sperm.

A retrospective analysis was undertaken in 160 intrauterine insemination with husband's sperm (IUIH) cycles, performed in 45 couples. The male factor represented the main indication (80%). Women who conceived had a lower duration of infertility (4.4 years +/- 3.0 vs 7.7 +/- 2.98). A significantly and pardoxically lower motility (3+ and 2+) was observed in raw specimens within conceptional cycles. However in prepared specimens in conceptional cycles the percentage of motility 3+ and 2+ was significantly increased (80.6% +/- 8.1 versus 70.7% +/- 20.2). The most significant difference was found in regard to a better response to preparation among conceptional cycles, expressed as the percentual increase in motility 3+ and 2+ (55.6% +/- 3.9 vs 41.3% +/- 19.4).

Cross-sectional study of percentual changes in total plasmatic fatty acids during pregnancy.

Total plasma fatty acids were analysed in 75 first trimester pregnant women, 32 second trimester pregnant women, 27 women at delivery and 38 non-pregnant women. Eleven fatty acids were studied by capillar gas chromatography SP-column. Compared with non-pregnant women, in the first trimester there was a significant percentual increase in palmitic, palmitoleic, stearic and docosahexaenoic acids, whereas linoleic and eicosapentaenoic percentually decreased. Between the first and second trimester a significant increase in the proportion of palmitic acid and a significant percentual decrease in arachidonic acid was detected. Between the second trimester and delivery the aforementioned changes were more marked. Between the second trimester and delivery there was also a significant percentual increase in oleic and linolenic acids, whereas the proportions of stearic, linoleic, dihomogammalinolenic and docosahexaenoic acids significantly decreased. It is suggested that the intake in polyunsaturated fatty acids should be increased during pregnancy, specially in the third trimester.

Variations in estrogen and progesterone receptor levels after short-term tamoxifen treatment in breast carcinoma.

The modification of estrogen and progesterone receptor status was prospectively studied in mammary carcinoma after short-term (7 days) administration of tamoxifen citrate (30 mg/day). Seventeen patients fulfilled the stringent criteria required for final evaluation. Tamoxifen was administered to them immediately prior to surgery and receptor status was studied on the surgical specimen. Estrogen receptor levels were significantly (p < 0.05) decreased after treatment, whereas progesterone receptors remained statistically unaltered, although they showed a tendency towards significant increase (p = 0.12). Following these results, short-term tamoxifen treatment seems inadequate for the induction of significant levels of progesterone receptors in mammary carcinoma.

Monitoring endometrial adenocarcinoma with a four tumor marker combination. CA 125, squamous cell carcinoma antigen, CA 19.9 and CA 15.3.

The combined value of four tumor markers, in the follow-up of endometrial adenocarcinoma, is analyzed. Cancer antigen 125 (CA 125), squamous cell carcinoma antigen (SCC), carbohydrate antigen 19.9 (CA 19.9) and carbohydrate antigen 15.3 (CA 15.3) were used in 213 evaluations from 105 patients. Sensitivity as regards recurrence or progression of disease was 45% (CA 125), 9% (SCC), 51% (CA 19.9) and 21% (CA 15.3). Specificities as regards the 'no evidence of disease' ranged from 95% to 99%. Single tumor marker efficiency ranged from 90% for CA 125 to 84% for SCC (p = 0.08). With the two tumor marker combination sensitivity increased up to 77% achieved with CA 125-CA 19.9, but efficiency increased only slightly (92.0% for CA 125-SCC). In the best three tumor marker combination, a sensitivity of 85% was achieved (CA 125-CA 19.9-CA 15.3), and an efficiency of 92.2%. The simultaneous use of the four tumor markers did not improve assay results. The possibility of recurrence or progression of disease in some combinations was very low (4.6% when CA 125 and CA 19.9 negative, 3% when CA 125, CA 19.9 and CA 15.3 negative), a fact to be considered in order to avoid aggressive management in such cases. The tumor markers were of limited value for the prediction of recurrences. The suggestion of recurrence when the increase in tumor markers was the only finding was confirmed in only 7%, while confirmation was made in 100% when there was another pathological finding.

Intrapartum chemoprophylaxis of early-onset group B streptococcal disease.

A randomized study in 121 pregnant women carrier of group B streptococci is undertaken in order to assess the administration of 500 mg of intrapartum ampicillin intravenously to interrupt mother-to-fetus group B streptococcal transmission. In the prophylaxis group there was a significant reduction in neonatal colonization (3.7 vs. 42.9%) and in severe neonatal colonization (0 vs. 25%). There was no case of group B streptococcal sepsis in the prophylaxis group compared to 4.6% (3 cases) in the control group (P greater than 0.05). Clinically infected newborns represented 3.3% in the prophylaxis group vs. 13.8% in the control group. When the organism was isolated during delivery in the vagina or amniotic fluid, prophylaxis was quickly followed by second negative cultures. Ampicillin levels in the amniotic fluid were detected early, and they increased significantly till the third hour. Bactericidal levels in the umbilical cord were detected in 60% of newborns. All these findings support the usefulness of ampicillin prophylaxis in the prevention of early-onset group B streptococcal sepsis.
