[Clinical evaluation of 99mTc-MAG3 renal imaging: comparison with 99mTc-DTPA, 99mTc-DMSA and 131I-OIH].

The newly developed 99mTc-labeled renal imaging agent, mercaptoacetyltriglycine (MAG3), is a tubular secreted compound. It is expected the most suitable agent to measure renal function and image urinary tract because of the excellent imaging properties and availability of 99mTc and preferred biological qualities of radiohippurates. We examined 15 patients (8 males, 7 females, mean age 55 years old) with renal and urinary tract disease. After drinking 400 ml of water, 185, 370 or 555 MBq of 99mTc-MAG3 was injected with the supine position. Dynamic scans were acquired for 20 minutes on a large field of view gamma camera (ZLC-7500 Siemens Medical Inc.) linked to a computer system (Scintipac-2400 Shimadzu Co.) in a 64 x 64 matrix. Regions of interest (ROI) were placed around each kidney. Curves of 12 seconds frame rate were generated from all two ROIs over the 20 minutes study. The time to peak count (Tmax) and time from peak count to a half count (T1/2) were calculated from the curve data. 99mTc-DTPA renal imaging was examined to the same patients with the same method like 99mTc-MAG3. The renogram of 131I-orthoiodohippurate (OIH) was performed with probe method. The excellent quality of the renal image was obtained with 185-555 MBq of 99mTc-MAG3 compared with the same dose of 99mTc-DTPA respectively. The diagnostic value of 99mTc-MAG3 renal imaging was as well as 99mTc-DMSA renal imaging. The Tmax of 99mTc-MAG3 renogram was same value of that of both 99mTc-DTPA and 131I-OIH renogram. The T1/2 of 99mTc-MAG3 renogram was significantly lower than that of 99mTc-DTPA renogram. The T1/2 of 99mTc-MAG3 renogram was slightly higher that of 131I-OIH renogram. We conclude that the renal imaging of 99mTc-MAG3 was able to acquire the most excellent image and the proper renogram pattern as same as 131I-OIH renogram.

In vitro branching morphogenesis of the fetal rat lung.

We studied the pattern of airways branching in the fetal rat lung in vitro. Lung primordia of gestational ages 13, 14, and 15 days were allowed to grow in culture to a gestational age equivalent to 21 days. The first generation airways appear by a single new bud (monopodial budding) from the left main airway (lateral appearing before the medial). They elongate to form branches and then bud dichotomously (2 buds occurring simultaneously and adjacent to each other) at their tips. Then monopodial branching takes place along their sides. The same cycle of budding and branching seems to be repeated for the following generation of the airways. The total number of the peripheral (subpleural) buds was greatest in the day 15 explants and least in day 13 explants throughout the whole culture period, but the statistical model used indicated faster budding in the 13 day explants. Morphometric assessment showed no difference in the ratios between the lung components in the 3 age groups and that the peripheral epithelial measurements were the same in the 3 groups at an equivalent gestational age of 21 days. We have also shown that lobes do not form in the right lung, although appropriate airways do. This may indicate the importance of mesothelial covering of the lung in the process of lobe formation. The method is useful for studying the control of lung morphogenesis.

Role of calmodulin as an endogenous initiatory factor in compensatory lung growth after pneumonectomy.

Compensatory lung growth after pneumonectomy is a well established phenomenon in young humans and experimental animals. To date, the cellular initiating and/or regulatory factor(s) responsible for this growth response have yet to be established. We have studied changes in lung content and activity of calmodulin, a calcium-dependent regulatory protein in relation with lung mass and DNA content during postpneumonectomy compensatory lung growth in 4-week-old rats. We observed that after left pneumonectomy, right lung calmodulin content (measured by radioimmunoassay) and calmodulin activity (measured by cyclic nucleotide phosphodiesterase activation) were increased at days 1 and 2 after surgery in the pneumonectomized rats, preceding the increase of lung mass and DNA content which started at day 3. Treatment of the pneumonectomized rats with a highly specific calmodulin antagonist, trifluoperazine, immediately after the surgery, resulted in diminished lung calmodulin activity, sparing the calmodulin content, and in concomitant reduction of lung mass and DNA content to values intermediate between those of controls and the pneumonectomized animals. Based on these findings, we conclude that calmodulin may be an important intracellular (possibly, autocrine) initiatory or facilitatory factor in compensatory hyperplastic lung growth after pneumonectomy.

Discordant regulation of human type I collagen genes by prostaglandin E2.

We examined the expression of type I collagen genes in prostaglandin E2 (PGE2)-treated human lung fibroblast cultures. Addition of PGE2 to fibroblast cultures inhibited alpha 1(I) mRNA levels by approx. 25% after 6 h and 60% after 24 h. Further studies showed that dibutyryl cAMP did not inhibit alpha 1(I) mRNA levels and that cycloheximide blocked the inhibitory effect of PGE2. In contrast, PGE2 treatment with or without cycloheximide did not affect alpha 2(I) mRNA levels. Moreover, in vitro translation of RNA derived from untreated and PGE2-treated cells yielded similar amounts of alpha 2(I) collagen peptides. Taken together, these results suggest that PGE2 induces a protein which inhibits alpha 1(I) transcription through distinct regulatory elements not under the control of cAMP and provide further evidence that the type I collagen genes can be discordantly regulated.

Effect of l-menthol on the permeation of indomethacin, mannitol and cortisone through excised hairless mouse skin.

The effect of l-menthol on the skin permeability of mannitol, cortisone or indomethacin was examined by an in vitro penetration technique with hairless mouse skin. The donor solution was prepared with phosphate buffered saline, ethanol:buffered saline (20:80, v/v) or ethanol:buffered saline (20:80, v/v) containing 1% (w/v) l-menthol. Although ethanol showed little enhancing effect, l-menthol in an aqueous ethanol vehicle at pH 7.4 increased the permeability coefficients of mannitol and indomethacin by about 100 times that of the control (an aqueous vehicle) and increased that of cortisone by about 10 times. l-Menthol, however, scarcely enhanced the penetration of indomethacin at pH 3.0, the majority of the species being in unionized form. These results suggested that the menthol-ethanol-aqueous system enhanced skin permeability through a direct effect on the polar and/or lipid pathways, while the thermodynamic activity of the penetrant molecule in the delivery vehicle might also influence the effectiveness of the penetration enhancer.

[Scrotal scintigraphy in the diagnosis and grading of varicocele].

Scrotal scintigraphy with 99mTc-red blood cells has been reported as a useful method for detecting varicocele. In this study we analysed the scintigraphy of 251 infertile males with clinically diagnosed or suspected varicocele, in an attempt to establish a grading system of varicocele. Scintigraphically varicocele was diagnosed in 207 patients on the basis of pooling in hemiscrotum in static images and/or early flow through the spermatic cord vessels in dynamic images; physical examination overlooked 17 of them. Of the 207 patients, all had pooling (153 left-sided, 1 right-sided, and 53 bilateral pooling) and 52 had early flow. The early flow was a less sensitive sign for varicocele than the pooling and invariably accompanied by the intense pooling. The early flow may be related to increased shunt flow through varicocele. According to the scintigraphic findings, varicocele was classified as follows: Grade I (small varicocele with faint early flow or mild localized pooling, n = 103), Grade II (medium varicocele with obvious early flow or curvilinear mottled pooling, n = 67) and Grade III (large varicocele with marked early flow or pooling, n = 37). Clinical assessment (93 small, 68 medium, and 54 large varicocele) supported the scintigraphic classification. Scrotal scintigraphy, therefore, facilitates precise evaluation of varicocele based on its morphology and hemodynamics.

Characterization of two genetically distinct type I-like collagens from lamprey (Entosphenus japonicus).

1. Type I-like collagens were isolated by limited pepsin digestion from various tissues of lamprey, a member of the cyclostomes. 2. Characterization of these collagens revealed the tissue-specific existence of two genetically distinct molecular species, each possessing the typical heterotrimeric nature of (alpha 1)2 alpha 2; one was designated skin collagen which existed in dermis and the other was designated body collagen which was distributed in muscle, intestine and cartilage. 3. The body collagen resembled invertebrate Type I-like collagens in many respects, whereas the skin collagen had a primordial form of vertebrate Type I collagen.

Cloning, sequencing and expression in Escherichia coli of cDNA for a non-A, non-B hepatitis-associated microtubular aggregates protein.

A 1.7 kb cDNA encoding a novel antigen (p44; apparent Mr 44K) associated with non-A, non-B (NANB) hepatitis, was isolated from the hepatic cDNA library of a chimpanzee infected with NANB hepatitis. The library was screened with a monoclonal antibody against this antigen. The cDNA cloned contained an open reading frame encoding a 444 amino acid protein with an Mr calculated to be 50,468. The cDNA hybridized to a 1.9 kb mRNA obtained from chimpanzee hepatocytes infected with either the NANB or hepatitis delta viruses. It hybridized weakly to mRNA from hepatitis B virus-infected hepatocytes, and not at all to mRNA from normal chimpanzee hepatocytes. Southern blot analysis revealed that p44 is a host protein in chimpanzees, and that an identical gene exists in the human genome.

[MRI of pheochromocytoma].

MRI features of 8 patients were reviewed and correlated with pathological findings. It was characteristic finding that the intensity of pheochromocytoma on T2-weighted image and proton image was very high. The detectability of pheochromocytoma by MRI and CT was almost equal and MRI was also useful for determining aberrant pheochromocytoma. MRI was more helpful for detection of necrotic lesion than CT. We considered that the double ring sign on MRI was characteristic for pheochromocytoma. In the cases which are difficult for diagnosis, other diagnostic studies such as endocrine and other imaging studies are necessary.

[Clinical evaluation of 123I-orthoiodohippurate (123I-OIH) for diagnosis of renal function: dose finding study].

The safety, clinical usefulness and the optimal dose of 123I-orthoiodohippurate (123I-OIH) were evaluated in patients with mild renal disorders. 123I-OIH was administered intravenously to 45 patients, and 39 of those patients were analyzed for the clinical usefulness. The numbers of cases analyzed were 10 for 9.25 MBq (0.25 mCi), 18.5 MBq (0.5 mCi) and 74 MBq (2.0 mCi) and 9 for 37 MBq (1.0 mCi). Neither adverse reaction nor abnormal laboratory findings due to 123I-OIH administration were observed in the 45 patients. The clinical usefulness of the drug was evaluated in terms of individual renal function, regional renal functions, renal blood flow, parenchymal function and urinary function, and ability to differentiate between the renal and urinary functions, and the overall clinical usefulness. In the evaluation by the investigators, there were no significant differences in each of these parameters among any of the doses administered. In the evaluation by the Committee, however, the usefulness with 74 MBq superior to those with 9.25 MBq and 18.5 MBq, while there was no significant differences between those with 37.5 MBq and 74 MBq. The qualities of the blood flow phase images, functional phase images, functional images, and the statistical noises of the individual and the regional kidney renograms were also evaluated by the Committee. The image qualities and the statistical noises with 37 MBq were superior to those with 9.25 MBq and 18.5 MBq except for the functional image quality, and there were no significant differences between those with 37 MBq and 74 MBq other than the regional renograms and the functional images, which were superior with 74 MBq.(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of dipicolinate, a chelator of zinc, on bone protein synthesis in tissue culture. The essential role of zinc.

The present investigation was undertaken to clarify the essential role of zinc on bone protein synthesis in tissue culture. Calvariae were removed from 3-week-old male rats and cultured for periods up to 72 hr in Dulbecco's Modified Eagle Medium (high glucose, 4500 mg/dl) supplemented with antibiotics and bovine serum albumin. The calvariae were incubated for 24 hr at 37 degrees in 5% CO2/95% air in medium containing 10(-6)-10(-3) M dipicolinate, a chelator of zinc, and then the bones were transferred into medium containing either 10(-4) M zinc sulfate or vehicle without dipicolinate. Zinc content in bone tissues was decreased when the culture was treated with 10(-4) and 10(-3) M dipicolinate for 24 hr. When calvariae treated with 10(-4) M dipicolinate for 24 hr were further cultured in medium without dipicolinate for 24 and 48 hr, bone alkaline phosphatase activity was decreased by about 40% (P less than 0.01) of untreated bone enzyme activity. The decreased alkaline phosphatase activity was increased markedly by the presence of 10(-4) M zinc (about 2.5-fold of control value). This effect of zinc was blocked completely by the presence of 10(-7) M cycloheximide, but 10(-8) M actinomycin D caused only a partial inhibition. When calvariae treated with 10(-4) M dipicolinate were pulsed with [3H]proline, the incorporation of [3H]proline into the acid-insoluble residues of bone tissue was decreased by about 40% (P less than 0.01) of the value obtained from calvariae not treated with dipicolinate. The presence of 10(-4) M zinc caused an increase of about 2-fold in [3H]proline incorporation. Bone DNA content was not altered significantly by treatment with 10(-4) dipicolinate or 10(-4) M zinc. These results clearly indicate that endogenous zinc induces the stimulation of protein synthesis at the translational process in bone cells. The present study further supports the view that zinc plays an essential role for protein synthesis in bone cells.

Connective tissue, mechanical, and morphometric changes in the lungs of weanling rats fed a low protein diet.

We studied the effect of low-protein diet (8% casein) on lung growth in rats from 3 to 7 weeks of age. Their diet was isocaloric with that of control animals fed a diet of 20% casein. The calorie intake of experimental animals was increased during the first 3 weeks of the experiment, but they increased less (about 10%) in body weight, had less protein and less water when the whole body was examined, and had lower serum proteins and decreased urinary hydroxyproline. The experimental animals remained in positive nitrogen balance by maintaining low urinary nitrogen excretion. The lungs of the experimental animals were abnormal, with decreased absolute amounts of hydroxyproline and desmosine and of these relative to unit lung weight. The lungs contained more air per gram of lung tissue, and the volume of air in the lung was increased at all transpulmonary pressures above zero. When corrected for increased total lung capacity, there was a loss of recoil at mid-lung volumes. Saline-filled volume-pressure curves, corrected for lung volume, showed similar loss of recoil. Alveolar multiplication was quantitatively normal, but the experimental animals had larger alveoli. We conclude that the protein deprivation in isocalorically fed animals has a specific effect on lung scleroprotein content, which may be due to diminished synthesis, and this results in both structural and functional abnormalities in the lung. Our results indicate the importance of dietary protein in lung development and possibly as one of the causes of emphysema. Further studies are needed to know whether this would be a problem in infants of Kwashiokor.

Stimulatory effect of calcitonin on bone formation in tissue culture.

The present investigation was undertaken to clarify the in vitro effect of synthetic [Asu1,7]eel calcitonin (CT) on bone metabolism in tissue culture. Calvaria were removed from weanling rats (3-week-old male) and cultured for periods up to 96 h in Dulbecco's Modified Eagle Medium supplemented with antibiotics and bovine serum albumin. The experimental cultures contained 1.0 to 100 ng/ml CT. All cultures were incubated at 37 degrees C in 5% CO2-95% air. Bone calcium content was increased significantly by the presence of 10 and 100 ng/ml CT. This increase was blocked by the presence of 10(-6) M cycloheximide or 10(-7) M actinomycin D. Bone alkaline phosphatase activity was significantly increased by the presence of 100 ng/ml CT for 48 and 96 h. Bone acid phosphatase activity was not altered significantly by CT (1-100 ng/ml). The incorporation of [3H]proline into the acid-insoluble residues of bone tissue was significantly increased by the presence of CT (1-100 ng/ml) for 96 h. This increase was completely blocked by the presence of 10(-7) M cycloheximide. Bone DNA content was significantly raised by the presence of 10 and 100 ng/ml CT for 96 h. Furthermore, the culture with CT (10 and 100 ng/ml) produced a significant decrease in glucose concentration in the medium. Also, CT (10 and 100 ng/ml) stimulated the production of pyruvic acid from bone tissue. These results suggest that CT had a direct stimulatory effect on bone formation and mineralization in vitro, and that the hormone stimulates energy metabolism in bone cells.

[Pulmonary complications subsequent to fractured neck of the femur in the elderly].

Fracture of the neck of the femur (FNF) is a common disorder in the elderly. A total of 618 cases consisting of 117 males and 501 females, whose age was 65 years or more, were enrolled in a prospective study. A total of 45 cases among them revealed pulmonary complications. These were divided into the following three groups: Group 1 (4.7%) who had respiratory disease(s) or symptoms prior to the fracture; Group 2 (1.9%), diagnosed as having pulmonary thromboembolism (PTE). In Group 3 (0.6%), PTE was a possible diagnosis but it was not distinguished from pneumonia in precise. In the patients of group 2 and 3, respectively, the following respiratory symptoms were observed: dyspnea (31.3%), productive cough (25%), syncope (12.6%), chest pain (6.3%), tachycardia (46.7%), and tachypnea (50%). An abnormal chest roentgenogram was found 56.4% in both group 2 and 3. Seven patients in group 2 showed remarkable reduction of PaO2 on admission, however these all recovered within 7 days without any thrombolytic treatment. The prevalence of PTE caused by FNF in the elderly was close to that in younger cases, but the clinical symptoms were less in the former.

Unilateral hemispheric cerebral changes similar to Creutzfeldt-Jakob disease in a case of hemiconvulsion.

A 77-year-old man suffered intermittent hemiconvulsions of unknown etiology on the left side for a period of about 5 weeks. At the autopsy, there was marked neuronal loss, severe proliferation of astrocytes and spongiform changes in the right cerebral cortex. The cerebral white matter showed loosening with astroglial proliferation in areas on the same side. These neuropathological changes were slight or absent in the left cerebral hemisphere. Histopathological changes were similar to those seen in unilateral Creutzfeldt-Jakob disease (CJD). Although unilateral CJD can not be ruled out, these unilateral hemispheric changes might be induced by intermittent hemiconvulsions.
