Free gingival grafts using a CO2 laser: results of a clinical study.

Free gingival grafts are a reliable method for treatment of gingival recessions: An autograft is taken from the palate as replacement for the lost keratinized gingiva. A technique using the CO2 laser and results from a clinical study are presented. The receptor beds for 23 free gingival grafts were prepared with a CO2 laser. After a trapezoid incision in the epithelium, the connective tissue fibers were cut parallel to the periosteum avoiding coagulation of this delicate tissue. The grafts from the palate were taken conventionally. In all cases wound healing and incorporation of the grafts were good; no delay could be seen. Controls after 1 year revealed good results of the treatment. The CO2 laser can be useful for the free gingival graft procedure. Good hemostasis and low mechanical stress of the receptor bed can be achieved without affecting wound healing or incorporation of the delicate free gingival grafts.

[High-frequency dental surgery near metal reconstructions].

Dental electrosurgery carries the risk of incidental contact between the active electrode and metallic restorations. So far, on-line measurements of basic physical parameters during dental electrosurgery have not been performed under realistic conditions. The aim of our experimental study in pig jaws was to describe the physical phenomena more closely. 86 teeth in the jaws of freshly slaughtered pigs received occlusal amalgam fillings. Using 3 different units electrosurgical procedures were performed adjacent to the fillings. Voltage, current, impedance and power output were registered using a computer-controlled measuring system with high temporal resolution and broad dynamic range. The impedances of the amalgam fillings ranged from 0.5 to 9 k omega, with half of them being in the lower range of 0.5 to 1.5 k omega. During contact to amalgam fillings the dental electrosurgical unit had a high power output. Contact to fillings with low impedance yielded a power output up to 50 W. The other two electrosurgical units were equipped with different power output control systems thus preventing high power output during contact to metallic restorations. In conclusion, dental electrosurgery should be improved by units with dynamic power output control.

Histology of laser- and high-frequency-electrosurgical incisions in the palate of pigs.

The histological effects of CO2-laser-, Nd-YAG-laser- and electrosurgical incisions were investigated in the oral mucosa of pigs. Laser- as well as electrosurgery caused necrosis by thermal effects. Undermining vacuolisation in the stratum basale and denaturation of collagenous fibers in the lamina propria were typical findings. The CO2-laser incisions were trough-like with a sharply bordered coagulation. The Nd-YAG-laser produced exclusively coagulation but no incision. Electrosurgery yielded radiating coagulation and the best ratio of depth of incision to width of coagulation.

[Need for preventive dentistry in adult women: investigation in maternal convalescent homes. 2. Tooth and periodontal findings].

For this study an extensive oral examination was performed in 111 women between 21 and 48 years of age recreating themselves in special rest homes. A mean DMF-T of 19.1 and a mean DMF-S of 62.5 was found; women with a low education (Sonderschule) showed an especially high DMF-S of 85.0, participants with a secondary education (Abitur) had a DMF-S-Index less by half. The oral hygiene of the women was insufficient (mean QHI 2.6), papilla bleeding was easy to provoke in most of them. 98% of the women needed periodontal treatment, 10.8% showed a probing depth of 6-8 mm. 60.8% of 942 examined restorations needed to be renewed, the reason was mostly marginal leakage.

[Measurement of physical parameters in HF surgical techniques in dentistry].

A method for measuring electrical parameters in dental electrosurgery and new data on this surgical technique are presented. A computer-controlled system which measures voltage, current, phase, power and impedance with high temporal resolution was applied to dentistry. Statistical analysis of many operations on freshly butchered pig jaws was performed. The impedance of oral soft tissues is higher during cutting, the power required smaller than stated in the literature. The influence of frequency, power-output control, anatomical structures, and metallic restorations on dental electrosurgery is discussed in detail.

[Recommendations for training with dental HF surgery apparatus].

A training program for the application of dental electrosurgery is presented. Exercises on muscle tissue and pig jaws are suitable to get informations of the proper adjustment of the units as well as the treatment procedures. The necessity of proper handling is illustrated by the results of wrong manipulations. Better understanding of the facts during electrosurgery enables the dentist to avoid adverse effects in treating his patients.

[HF surgery vs. laser surgery. Experimental study of use in periodontology].

The purpose of this study was to evaluate the suitability of CO2 laser and two different electronically controlled electrosurgical units in periodontal surgery; both of the electrosurgical generators were recently designed for application in medicine. The study was performed on freshly butchered pig jaws. In addition to the management of the devices, the macroscopic and histologic effects of laser and high frequency energy on palatal gums, alveolar bone, tooth structures and filling materials were investigated. The handling of the electrosurgical units was efficient and smooth, the incisions showed accurate and reproducible results. In contrast to this the laser was uncomfortable to work with, additional safety arrangements were required and lesions in teeth and alveolar bone occurred easily. While the electronically controlled electrosurgical generators offer interesting aspects in some indications, CO2 laser seems to be unsuitable for periodontal surgery.

[Need for dental prophylaxis in grown women: an investigation in maternal rest centers. 1. Information knowledge and or oral hygiene status].

111 women out of 316 women relaxing in three rest homes volunteered to participate in this study and answered a questionnaire concerning following subjects: daily oral hygiene, knowledge in hygiene procedures, frequency of dental treatment and interest in dental prophylaxis. Oral hygiene behaviour and knowledge were not sufficient. Only half of them knew dental floss and fluorides. Interdental care was practised by 20 women and only 10 used fluorides, they had without exception a secondary education. Most of the participants were educated in dental health care mainly by their parents. 36 women went to see the dentist only in case of necessity. 15 women were very afraid of dental treatment. 60% out of 111 women participated in a lecture on preventive dentistry and prophylaxis.

[Periodontal-orthodontic treatment possibilities for migrated front teeth--case selection and theoretical concept].

Migration of maxillary incisors caused by advanced periodontal disease induces functional, periodontal and esthetical problems to the patient. Under certain conditions a combined periodontal-orthodontal treatment should be considered. A successful treatment result depends on the selection of the patients. The presented concept offers good long-term results for a combined periodontal-orthodontal therapy.

[Comparative investigations of new dental electrosurgical instruments].

11 electrosurgical units of several manufactures were analysed by physical measurements. Conventional units and those with constant output voltage demonstrate nearly identical behavior in their voltage- and power-characteristics at load-impedance occurring during dental electrosurgery. So far there are no advantages of simple voltage control. We did not find technical deficiencies concerning security in the modern units examined.

[Effects of orthodontic expansion of the mandibular arch on the periodontal condition of the posterior teeth].

In the present study we examined the long term influence of orthodontic mandibular arch expansion therapy on the periodontal condition of the posterior teeth. 11 years after the end of treatment 31 former patients of our clinic were compared with a control group of matched age and sex distribution. Although we found some additional loss of periodontal attachment for the treated group this differences were not considered to be of clinical significance. Within the treated group the stability of the orthodontic treatment result seemed to be correlated with more favourable periodontal conditions.

[Importance of oral hygiene in the treatment of capillary hemangioma of the gingiva].

This is a case report on two patients, 21 resp. 25 years old, who suffered from recurrent capillary hemangioma of the gingiva. Prior to surgical therapy the hygienic conditions were improved in several sessions with professional preventive treatment and oral hygiene instructions. Post-surgical treatment consisted of efficient plaque control and adequate three-monthly oral prophylaxis sessions. In both cases the hemangioma was not observed to recur up to 2 1/2 resp. 1 1/2 years later.
