Radioiodine therapy of carcinoma of the thyroid.

The discovery of functioning metastases of thyroid carcinoma and their appropriate treatment with radioiodine are the keys to long-term survival. Functioning metastases can be adequately found only if postsurgical residual normal thyroid tissue is ablated and total body scanning performed. Radioiodine treatment of functioning metastases following appropriate surgery is the treatment of choice, and long-term follow-up with total body scanning techniques is most important.

Correlation of nonspecific immune monitoring with rejection or impaired function of renal allografts.

Two nonspecific immunological assays were combined with radionuclide scanning to monitor 113 patients thrice weekly following allotransplantation. The nonspecific immune assays included measurement of the percentage of active T rosette-forming cells (% A-T RFCs) and spontaneous blastogenesis (SB). An increase in SB and decrease in % A-T RFCs (greater than 1 sd of normal controls) constituted an immune event. The immune parameters were correlated with thrice weekly radionuclide studies which were computer analyzed for glomerular and tubular function. Alteration of the immunological and radionuclide parameters significantly correlated (P less than 0.001) with 90 rejection episodes displayed by 72 nonantithymocyte globulin (ATG)-treated renal allograft recipients during the first 30 postoperative days. In the absence of clinically defined rejection, changes in immune parameters correlated with (1) decline of radionuclide parameters and (2) alterations in weight, temperature, creatinine clearance, and serum creatinine, suggesting subclinical events. As a result of the effects of ATG on lymphocytes, a similar comparison could not be made for 41 other patients treated with this immunosuppressive drug. The incidence of false positive tests was 12.7%. Thus, the combination of two nonspecific immune parameters, % A-T RFCs and SB, with computerized analysis of radionuclide scans may afford a reliable index to diagnose early rejection or impaired function of renal allografts.

Environmental equity and pesticide exposure.

Although people of color and low-income groups bear a disproportionate share of the health risks from exposure to pesticides, research attention has been meager, and data on acute and chronic health effects related to their toxic exposures are generally lacking. Increased resources are needed both to study this issue and to mitigate problems already identified. People of color should be a major research focus, with priority on long-term effects, particularly cancer, neurodevelopmental and neurobehavioral effects, long-term neurological dysfunction, and reproductive outcome. Suitable populations at high risk that have not been studied include noncertified pesticide applicators and seasonal and migrant farm workers, including children.

An immunohistochemical study of neurofibrillary tangle formation in post-encephalitic Parkinsonism.

Neurofibrillary tangle (NFT) formation is a feature of postencephalitic Parkinsonism (PEP) and Alzheimer's disease (AD). Tangle formation has been compared immunohistochemically in these 2 conditions. Staining patterns for tau protein, ubiquitin and beta/A4 amyloid protein were studied in frontal lobe, hippocampus, and midbrain in 2 classical cases of PEP, 2 cases of AD and 2 controls matched for age and sex. NFTs were present in all cases, but with varying frequency: all tangles were tau-positive and many were ubiquitinated. In the frontal cortex and hippocampus, irrespective of the case category, tangle formation was associated with beta/A4 amyloid deposition. A similar association was present in the 2 AD cases in the midbrain. However, in PEP tangle formation in the midbrain was not associated with adjacent beta/A4 amyloid deposition. This finding raises the possibility that the pathogenetic mechanism of tangle formation in PEP is different from that of AD, although the final cellular morphological expression of abnormality in both conditions is similar.
