Cryptosporidiosis: multiattribute evaluation of six diagnostic methods.

Six diagnostic methods (Giemsa staining, Ziehl-Neelsen staining, auramine-rhodamine staining, Sheather's sugar flotation, an indirect immunofluorescence procedure, and a modified concentration-sugar flotation method) for the detection of Cryptosporidium spp. in stool specimens were compared on the following attributes: diagnostic yield, cost to perform each test, ease of handling, and ability to process large numbers of specimens for screening purposes by batching. A rank ordering from least desirable to most desirable was then established for each method by using the study attributes. The process of decision analysis with respect to the laboratory diagnosis of cryptosporidiosis is discussed through the application of multiattribute utility theory to the rank ordering of the study criteria. Within a specific health care setting, a diagnostic facility will be able to calculate its own utility scores for our study attributes. Multiattribute evaluation and analysis are potentially powerful tools in the allocation of resources in the laboratory.

Preparing for accreditation: an opportunity for faculty development and curriculum renewal.

An approach to ACEHSA accreditation is described which turns this exercise into an occasion for faculty self reflection, development, and renewal. The carefully facilitated process examines the curriculum, its fundamental bases, processes, and products from multiple points of view including those of the faculty, current students, graduates, and employers. The process is derived from deliberative curriculum inquiry and results in curriculum specifications that are ordered by actual practice and truly understood and supported by faculty, students, and practitioners. Feedback from participants indicates that the carefully guided process of introspection turned a negative expectation surrounding the preparation for accreditation into a positive experience of faculty development and curriculum renewal. The process was improved across the two replications documented here.

Efficiency of skin sterilization for a venipuncture with the use of commercially available alcohol or iodine pads.

Skin sterilization for a venipuncture is routinely done with commercially available alcohol or iodine pads. Selection of the antiseptics, alcohol or iodine, however, in most situations has been made on the basis of very little scientific data. With many patients with granulocytopenia who are undergoing venipunctures, the choice of antiseptic may be an important factor in preventing infections. We investigated two widely and commercially available disinfectant pads, alcohol and iodine, in the efficacy of skin sterilization. Seventy subjects (35 adults and 35 children) were randomly selected for this study. A designated area of the right and left forearm was sterilized either with alcohol or with an iodine pad in a predetermined uniform fashion. Specimens were obtained for cultures before and after sterilization. The bacterial cultures were performed with the use of blood agar plates and trypticase soy broth. For data analysis growth of any organisms on agar plates or trypticase soy broth after sterilization was interpreted as a sterilization failure. The iodine swab was significantly more efficient than the alcohol swab; the former yielded an 80% sterilization rate whereas the latter resulted in a rate of 61% (p less than 0.02). If, however, the growth only in agar plates was compared, the alcohol wipe yielded no growth in 83% and iodine in 84%, virtually identical success rates. Bacillus spp. predominated the residual organisms after either the alcohol or the iodine wipe. A variety of other organisms, however, including Staphylococcus and Corynebacterium spp., grew after alcohol but not after iodine sterilization.(ABSTRACT TRUNCATED AT 250 WORDS)

Fungal cellulase and hemicellulase prediction of forage digestibility.

An enzymatic procedure using Trichoderma viride carbohydrases, a fungal hemicellulase, and pepsin was developed to provide a laboratory method for predicting forage digestibility. The amount of forage dry matter solubilized by enzymes and incubation buffer was less than that in vivo or by in vitro fermentation by rumen microorganisms. Total forage dry matter solubilized by the enzymatic procedure was correlated (.92) with in vitro digestibility. Simple correlation coefficients between in vivo true digestibility of 18 forages and total dry matter solubilized by enzymes and buffer was .87; that soluble in the buffer only, .65; and that solubilized by the enzymes, .82. Correlation coefficients with in vivo true digestible amount of cell-wall and protein were greater when the forage species were considered separately than when pooled together. With this restriction, the enzymatic procedure can be a useful method for predicting forage digestibility. Fermentations with rumen inoculum however, provided more accurate predictions of in vivo digestibility in a variety of forage species.

Spontaneous resolution of myelofibrosis and pancytopenia followed by the development of acute myeloid leukemia with an extramedullary mass.

PURPOSE: Spontaneous resolution of myelofibrosis is extremely rare. A patient with myelofibrosis and pancytopenia that spontaneously resolved with subsequent development of acute myelomonocytic leukemia and an extramedullary mass of the ovary is described. PATIENT AND METHODS: A 2-year-old girl had severe pancytopenia and myelofibrosis without signs of myeloid metaplasia. The patient was transfused with packed red blood cells and platelets and monitored. RESULTS: The pancytopenia and myelofibrosis resolved spontaneously within 1 month after initial presentation and her blood counts normalized. The patient developed a chloroma of the ovary and acute myelomonocytic leukemia 13 months later and eventually died from disease. CONCLUSIONS: This patient initially had acute myelofibrosis which was most likely secondary to occult acute myelomonocytic leukemia. The transfusions may have been a contributing factor in the spontaneous remission.
