Rapid determination of tannins in tanning baths by adaptation of BSA method.

A rapid and reproducible method for the determination of tannins in vegetable tanning baths is proposed as a modification of the BSA method for grain tannins existing in literature. The protein BSA was used instead of leather powder employed in the Filter Method, which is adopted in Italy and various others countries of Central Europe. In this rapid method the tannin contents is determined by means a spectrophotometric reading and not by means a gravimetric analysis of the Filter Method. The BSA method, which belongs to mixed methods (which use both precipitation and complexation of tannins), consists of selective precipitation of tannin from a solution containing also non tannins by BSA, the dissolution of precipitate and the quantification of free tannin amount by its complexation with Fe(III) in hydrochloric solutions. The absorbance values, read at 522 nm, have been expressed in terms of tannic acid concentration by using a calibration curve made with standard solutions of tannic acid; these have been correlated with the results obtained by using the Filter Method.

[Cytological diagnosis and cytohistological correlations of malignant epithelial tumors of the rhinopharynx].

Nasopharyngeal carcinoma frequently has as its first manifestation cervical lymph node metastases before local symptoms appear. In the Instituto Nazionale per lo Studio e la Cura del Tumori of Milan, routine nasopharyngeal cytology has been carried out on these patients. Positive cytology in 55 cases of undifferentiated carcinoma (83.6%) and positive cytology of biopsies was obtained in 87.3%. The combination of cytology and histology gave a yield of 98.2% positive cases. For squamous carcinomas positive cytology was less frequently obtained, the same as for malignant lymphomas. The cytology of undifferentiated carcinoma has such marked morphological characters that it is very easy to diagnose it in a very high percentage of cases. In the differential diagnosis cytologically it is necessary to bear in mind other neoplasias, in particular some of the malignant lymphomas.

A randomized EORTC study on the effect of preoperative polychemotherapy in pyriform sinus carcinoma treated by pharyngolaryngectomy and irradiation. Results from 5 to 10 years.

226 patients with an operable epidermoid carcinoma of the pyriform sinus were entered in this clinical trial; 187 cases were considered evaluable, they were distributed as follows: 16 T2, 171 T3, 44 N0, 103 N1 and 40 N3 (UICC TNM Classification 1979). The 89 patients of the chemotherapy group received a 3 day course of Vincristine (1.5 mg/m2), Bleomycin (15 mg), Methotrexate (80 mg). Both the chemotherapy group and the 98 patients of the control group went through the surgical procedure of total laryngectomy with pharyngectomy and radical neck dissection. Radiotherapy was administered postoperatively according to tumor margins and nodal involvement. On examination of the surgical specimen, no evidence of tumor regression was observed in the chemotherapy group. The 3 and 5 year survivals are respectively 45% and 35% in both groups. Node metastasis was studied after N size, N status, capsular rupture (PR +) and revealed early and extensive nodal involvement with 25% capsular ruptures in N0 patients, up to 70% rate in N3. Capsular rupture is also associated with a significantly lower survival and a higher incidence of distant metastases.

[Synchronous association of carcinoma of the superior aerodigestive ways and lymph node metastasis of papillary adenocarcinoma of the thyroid: presentation of 6 cases].

Six cases are reported of an unusual synchronous association between aerodigestive squamous cell carcinoma and papillary carcinoma of the thyroid. Six males (47-62 years) were observed until 1975 at the Department of Head and Neck Surgical Oncology of the Istituto Nazionale Tumori, Milan. All were affected by aerodigestive cancer: oral cavity (1), pyriform sinuses (1), larynx (2), lip (1) and oropharynx (1). Clinical examination of the thyroid was always negative. For this reason they underwent surgery, complete surgical approach to aerodigestive neoplasm; one hemithyroidectomy and one total thyroidectomy were also performed. Unexpectedly pathological examination of the dissected lymph nodes indicated the presence of metastases of papillary carcinoma of the thyroid. Three of the six cases had only metastases of thyroid papillary carcinoma while the others presented both squamous cell and papillary carcinoma metastases. Excluding the 2 patients who had undergone thyroid surgery during aerodigestive cancer therapy, in 3 of the 6 patients, two total thyroidectomies and one hemithyroidectomy were performed after the metastases were discovered. In all cases, primary thyroid cancers were demonstrated. One patient did not undergo any subsequent surgery on the thyroid gland because of negative thyroid scanning and bad prognosis due to aerodigestive cancer. Two of the 6 patients died of aerodigestive cancer at 54 and 34 months, 2 are still alive and free of both diseases while one was lost after a 43 month follow-up.

[Clinical classification of laryngeal carcinoma. Critique of existing classifications and proposal of a new working classification].

Clinical classification of cancer of the larynx is an intriguing problem that has not been yet solved by the several proposals followed on during thirty years. The main disagreement deals with their aims that have become strictly prognostic, with no egard for therapeutic indications in individual cases. Apart from this aspect, the prognostic factors taken into account even by the last drawing (TNM UICC 1987) seem too rough to allow a correct statistical comparison among the different therapeutic modalities that can be applied to each clinical situation. This lack becomes exceedingly relevant when controlled therapeutic trials are to be planned, comparing radiotherapy with the various types of surgical procedure. A careful critical review of the successive clinical classifications (UICC, AJC) is made and a new descriptive working classification (CLM) is proposed. It aims first of all at a more rational organization of the main prognostic factors to be considered for all locations and stages, with no radical distortion of the last TNM UICC staging system, some details excepted. Afterwards each stage is subdivided into substages according to the presence or absence of those prognostic factors of known influence on applicability and outcome of all various therapeutic modalities, with particular emphasis to some pending problems. CLM classification makes it possible to set up homogeneous groups of cases on which every type of prospective or retrospective comparison of different treatments can be made. Advantages of the CLM working classification are: no need for further modification; possibility of giving information, after appropriate check, about therapeutic indications; possibility of moving quickly and simply subcategories of cases with similar prognosis from CLM to TNM.

[Comparison of transcutaneous radiotherapy and cordectomy in the treatment of T1a glottic carcinoma].

770 patients affected by squamous cell carcinoma of the vocal cords, classified as T1a (U.I.C.C. TNM - P978) were studied retrospectively by the Head and Neck Oncology Cooperative Group. Several such studies have demonstrated the substantial equality of results, in terms of survival, obtained by the two common therapeutic approaches (radiotherapy and cordectomy). On the other hand, from a functional point of view, radiotherapy undoubtedly offers the best outcome. Site of origin has never been considered a possible prognostic factor for such cordal neoplasms; such factors could possibly make the choice between treatments significant. Therefore, the present study evaluated tumors, separating them by the anterior, middle and posterior third of the vocal cord and taking into consideration local check-up as a function of therapy. Local check-up, in terms of Relapse Free Survival (R.F.S.) was 83% at 10 years and no significant difference was observed between the different treatment modalities. Moreover, the site of origin did not appear relevant to prognosis. More accurate and prospective studies should be encouraged so as to assess the value of such data.
