The complication of infection after a septorhinoplasty is extremely rare. An unusual occurrence of necrotizing periorbital cellulitis after a routine septorhinoplasty in a 30-year-old pediatrician is presented. It consisted of marked swelling and superficial necrosis of the left eyelids and high fever that started one day after surgery. beta Hemolytic streptococcus was identified as the causative pathogen. The patient responded well to systemic antibiotics and conservative local treatment. After three years of followup the cosmetic appearance is reasonable.
Nine patients with aplasia cutis congenita were treated in our department during the past 30 years. Of these, 1 patient died due to uncontrolled hemorrhage and 1 patient was operated on by using a pericranial flap to cover a 7 x 5-cm scalp defect with large bony involvement. After 5 years of follow-up, the skull defect closed completely by normal bone that can be palpated on examination and seen on radiographs.
Previously we observed that systemic CyA induces hair growth in an experimental model of human scalp skin graft transplanted onto nude mice. In the present study we investigated the role of topical CyA in the murine transplantation model, using human split-thickness skin grafts (HSTSG). Ten mice grafted with 1-mm-thick skin and another 10 mice grafted with 0.4-mm-thick skin were treated topically with CyA in olive oil. Ten other mice, treated with olive oil only, served as a control group. At the end of the study we observed hair growth only on the grafted skin of the CyA-treated group. Four out of 10 grafts showed hair growth in each of the groups. Quantitative analysis of transverse sections of cylindrical punch biopsy specimens of HSTSG before transplantation revealed anagen follicles, including small ones and telogen/catagen follicles, whereas specimens after skin transplantation showed terminal follicles mostly in the anagen phase. The present study provides further support to previous observations regarding the beneficial effect of CyA on hair growth.
A recently introduced polymer preparation (Iodoplex) is described. It is based on a new type of hydrophilic polymeric carrier into which iodine can be incorporated to the extent of 50% or more. Its main feature is that the polymer and the petroleum jelly base are water-insoluble. The iodine is slowly released from the polymer by body fluids, which are rich in protein and dead cells. Iodine has been found to be useful in the treatment of donor sites; no pain or discomfort attends its use, and the dressing peels off easily from the healed epithelium. A controlled study comparing iodoplex-impregnated and scarlet red-impregnated gauze on donor sites was carried out, and the use of Iodoplex was found to have some real advantages over scarlet red.
We present a case of septal perforation in Osler-Weber-Rendu's disease, treated by rotating bilateral labial-buccal flaps up into the nose to close the perforation and resurface the adjacent areas on the septum.
