[Evaluation of bilio-intestinal anastomoses using radioisotope cholangiography].

In patients subjected to bilio-intestinal anastomosis as treatment of several diseases of the biliary tract and the Oddi's sphincter, with some frequency have symptoms that make suspect that the anastomosis is stenosed. The experience obtained in the evaluation of 8 cases is presented. The method used was radioisotopic cholangiography, with the administration of pyridoxiphenylalanine labeled with 99m-Technetium. Serial images were obtained in Polaroid film. In three of the eight patients, an obstruction of the principal bile duct was shown. After the operation, there was normal patency of the anastomosis. In the other cases, there was normal patency of the anastomosis, and the symptoms were due to other causes. We consider the radioisotopic cholangiography as the method of election in the study of these patients, as the results may indicate a surgical re-intervention, avoid another harmful method or make unnecessary another operation.

[Cardiorespiratory evaluation of midazolam and thiopental during their use in patients undergoing translumbar aortography in decubitus prone position and ASA IV].

AIMS: To verify the cardiorespiratory effects of midazolam and thiopental in ASA IV patients undergoing translumbar aortography in the decubitus prone position. METHODS: A prospective double blind study was carried out on 26 patients (mean age, 65.9 yr) allocated randomly to receive either midazolam (0.13 mg/kg) or thiopental (4 mg/kg) i.v. Without being premedicated all the patients had fentanyl 1.5 micrograms/kg i.v. and were preoxygenated with 50% oxygen before induction of anaesthesia. Blood pressure was measured by oscilometry and blood oxygenation by pulseoxymetry. RESULTS: No significant differences were found in the hemodynamic values (blood pressure, heart rate, double product) upon comparison of the two groups. Apnea was more frequent in the thiopental group 61.5% (8 out of 13 patients) than in the midazolam group 15.3% (2 out of 13 patients). Hypoxemia appeared only in the former group (61.5% of the cases). CONCLUSIONS: In patients undergoing translumbar aortography midazolam and thiopental behaved similarly with regard to hemodynamics. Midazolam produced less apnea than thiopental.

[New forms of Parkinsonism in hospice care].

Parkinson's disease is a well known phenomenon with characteristic evolution and clinical picture although having considerable variation in its presenting symptoms. From an extra-hospital consulting room we carried out a study of the characteristics of the clinical presentation of parkinsonisms in a series of one hundred cases so diagnosed from onset. The average age of the patients was around 66 years with a slight female predominance. Most patients had been referred to us by their own GPs. Some 70% of patients came suffering from shaking, 23% for slowness or clumsiness and 10% for unsteadiness. 71% of cases had unilateral onset and 29% had bilateral onset. Most of the patients we saw arrived at level 2 on the Hoehn and Yahr scale. The duration of the symptoms was approximately one year. In some 27% of cases drug consumption might have brought on parkinsonism. Almost two-thirds of these cases brought about by medicaments were being treated with cinaricine or flunaricine and nearly one third were undergoing neuroleptic treatment. Practically one out of every four cases, and one out of three women, could be considered as having iatrogenic parkinsonism. The onset of shaking with bilateral affectation is particularly suggestive of drug induced parkinsonism.
