Behavioural assessment of functional recovery after spinal cord hemisection in the bonnet monkey (Macaca radiata).

In spinal cord research, current approaches to behavioural assessment often fail in defining the exact nature of motor deficits or in evaluating the return of motor behaviour from lost functions following spinal cord injury. In addition to the assessment of gross motor behaviour, it is often appropriate to use complex tests for locomotion to evaluate the masked deficits in the evaluation of functional recovery after spinal cord injury. We designed a series of sensitive quantitative tests for reflex responses and complex locomotor behaviour in the form of a combined behavioural score (CBS) to assess the recovery of function in the Bonnet monkey (Macaca radiata). Monkeys were tested for various motor/reflex components, trained to cross different complex runways, and to walk on a treadmill bipedally. The overall performance of animal's motor behaviour and the functional status of individual limb movement during bipedal locomotion was graded and scored by the CBS. Surgical hemisection was then performed on the right side of the spinal cord at the T12-L1 level. Spinal cord hemisected animals showed a significant alteration in certain reflex responses such as grasping, extension withdrawal, and placing reflexes, which persisted through 1 year of follow-up. The spinal cord hemisected animals traversed the complex locomotor runways (Narrow beam and Grid runway) with more steps and few errors, at similar levels to control animals. These observations indicate that the various motor/reflex components and bipedal locomotor behaviour of spinal cord hemisected monkeys return to control levels gradually. These results are similar to those obtained in rat models by other investigators. These results demonstrate that the basic motor strategy and the spinal pattern generator for locomotion (SPGL) in adult monkeys for the accomplishment of complex motor tasks is similar, but not identical, to that in adult rats. This suggests that the mechanisms underlying recovery are probably similar in rats and monkeys, but that primates may take a longer duration to achieve the same functional end point.

On the thermal unfolding character of globular proteins.

A theoretical model is presented to study the stepwise thermal unfolding of globular proteins using the stabilizing/destabilizing characters of amino acid residues in protein crystals. A multiple regression relation connecting the melting temperature and the amounts of stabilizing and destabilizing groups of residues in a protein, when used for the thermal behavior of peptide segments, provides reliable results on the stepwise unfolding nature of the protein. In ribonuclease A, the shell residues 16-22 are predicted to unfold earlier in the temperature range 30-45 degrees C; the beta-sheet structures undergo thermal denaturation as a single cooperative unit and there is evidence indicating the segment 106-118 as a nucleation site. In ribonuclease S, the S-peptide unfolds earlier than S-protein. The predicted average and the range of melting temperatures, and the folding pathways of a set of globular proteins, agree very well with the experimental results. The results obtained in the present study indicate that (i) most of the nucleation parts possess high relative thermal stability, (ii) the unfolded state retains some residual structure, and (iii) some segments undergo gradual and overlapping thermal denaturation.

Relationship of mechanical factors to the strength of proximal femur fractures fixed with cancellous screws.

The decision of whether to attempt screw fixation of a femoral neck fracture is based partly on the estimated strength of the fixed bone/implant construct in relation to the loads it will be required to bear. The goal of this study was to determine in vitro the relation of the following biomechanical factors to the strength of internally fixed femoral neck fractures subjected to cyclic and failure loading: (a) square of the density of cancellous bone in the femoral head, (b) percent comminution of the inferior fracture surface, (c) moment arm of the joint force, or distance from the axis of the joint force to the fracture surface, and (d) orientation angle of the fracture surface in the medial/lateral plane relative to the axis of the femoral shaft. Femoral neck fractures were created in each of 38 fresh cadaveric proximal femora using a dropweight or with a materials testing machine. After sustaining a displaced fracture, fixation was achieved using three cannulated cancellous bone screws. The fixed femur was then subjected to 10,000 cycles of a sinusoidially varying load acting on the femoral head, parallel to the femoral shaft, with an initial peak magnitude of 2.2 times body weight, while the hip was flexed, extended, and rotated to mimic some motions of gait. Muscle loading was not simulated. The magnitude of the peak load decreased as the femoral head displaced during cycling. The mean of the peak load for each cycle over the duration of the test was defined as the average load. Following cycling, the bone/screw construct was loaded to failure in the same direction, and this measurement was termed the maximum load. Average and maximum load were then correlated to the four biomechanical factors using a multiple regression analysis. These factors correlated to a high degree with average force (R2 = 0.771; p < 0.0001) and to a lesser but still significant degree with maximum force (R2 = 0.458; p = 0.012), demonstrating that they can be used to estimate the strength of fixation under these loading conditions. The strongest correlation for average force was with fracture angle (p = 0.005) and for failure force was with moment arm length (p = 0.072).

Peripheral immune response in albino rats following cerebroventricular and intraperitoneal antigen challenge.

Success in neural tissue transplants at central nervous system suggest that the site may be immunologically privileged. However, this experimental study in which an antigen (Sheep Red Blood Cells) was administered into the third ventricle does not support the above concept. The antibody titre and soluble immune complex levels seen in these animals are similar to the levels seen in animals immunized with the same amount of antigen through the intraperitoneal route. Intraventricular immunization is rather a more potent modulator in decreasing the total WBC count (P < 0.05) and neutrophils (P < 0.001). Further a marked increase in lymphocytes (P < 0.01) in peripheral blood was observed in these animals. Intraventricular immunization also increased the killing power (NBT reduction) of the neutrophils (P < 0.05).

Percutaneous mitral valvotomy with the Inoue balloon catheter in children and adults: immediate results and early follow-up.

Percutaneous mitral balloon valvotomy (PMV) using the Inoue balloon catheter was attempted in 60 consecutive patients with severe symptomatic mitral stenosis. There were 10 children (mean age 13 years) and 50 adults (mean age 31 years). Forty patients were females and 20 were males; 53 were in sinus rhythm. The procedure was technically successfully performed in 57 (95%) patients. There were no deaths or thromboembolic complications. Balloon valvotomy was done using a 22 to 30 mm diameter catheter with the echo/Doppler guided stepwise mitral dilatation technique. After PMV the mean left atrial pressure decreased from 23.0 +/- 5.0 to 14.0 +/- 4.0 mm Hg (p less than 0.001). The mean mitral valve gradient (MVG) decreased from 15.0 +/- 4.0 to 6.0 +/- 2.0 mm Hg (p less than 0.001). The mitral valve area (Gorlin formula) increased from 0.7 +/- 0.2 to 1.6 +/- 0.4 cm2 (p less than 0.001). The mitral valve area as determined by echocardiography increased from 0.8 +/- 0.1 to 1.9 +/- 0.3 cm2 (p less than 0.001). Mild mitral regurgitation (MR) developed in six patients (11%) and increased by one grade in another five patients (9%). No patient developed severe mitral regurgitation. Mitral valve area at mean follow-up of 4.8 months remained unchanged at 1.9 +/- 0.3 cm2. We conclude that PMV, using the Inoue balloon catheter, is safe and effective in the treatment of severe mitral stenosis in children and adults, without inducing significant mitral regurgitation.

Right-sided endomyocardial fibrosis with recurrent pulmonary emboli leading to irreversible pulmonary hypertension.

A 26 year old Saudi man with features of both Loeffer's endocarditis and endomyocardial fibrosis presented with mild symptoms and pulmonary emboli. Echocardiographic examination showed obliteration of the right ventricular apex by an attached mass. The results of haemodynamic studies were somewhat abnormal and medical treatment was started. Despite anticoagulation with warfarin the patient's condition deteriorated rapidly over a four month period after a further episode of pulmonary embolism and the development of pulmonary hypertension. Two haemodynamic studies performed four months apart were typical of pulmonary hypertension and later right ventricular failure; they showed none of the characteristics of restriction. Pulmonary embolectomy was attempted but there was no cleavage plane between the organised thrombi and the endothelium of the pulmonary artery. The patient died of severe pulmonary hypertension and right ventricular failure several days after operation. Surgical intervention in the early stages of right-sided endomyocardial fibrosis might have prevented the development of pulmonary embolism and pulmonary hypertension.

The influence of side chain structures on the residue-residue associations in globular proteins.

The relationship between the physical make-up of the side chain structure of an amino acid and its capacity to associate with other side chains in the crystalline state of protein molecules has been investigated by a linear regression study. Relevant inter- and intra-molecular forces and steric factors are taken to represent the side chain structure, while the observed atom-atom interactions in a set of protein crystals are taken to represent the residue-residue association potentials. Correlation equations are set up in such a way that, (i) the dependence of the association potential of a residue on the side chain structures of associating residues, and (ii) the dependence of the association potential of a residue on its own side chain structure are brought out separately. The results show definite quantitative relationships in each of the above two views. Particularly, the behaviour of the charged residues is highly correlated to the structural make of their side chains. It is demonstrated how and to what extent the effects of associating/preferring residues are taken care of when they associate with a certain kind of residue in the protein environment.

Percutaneous balloon pulmonary valvuloplasty in sickle cell anemia: a case report.

Percutaneous balloon pulmonary valvuloplasty was performed on a 19-year-old female who had moderate pulmonary valve stenosis with sickle cell anemia. The patient developed sickle cell crisis resulting in occipital infarction, but she made a good recovery. We describe this case and the methods of reducing the risk of sickle cell crisis.

Variation of amino acid properties in protein secondary structures, alpha-helices and beta-strands.

A study was made on the physical, chemical, energetic, conformational, geometric, and dynamic property potentials of amino acid residues in protein secondary structures: alpha-helix and beta-strand. Property patterns were obtained by computing the average property values for specified residue units partitioned longitudinally and transversely about the chain. It was found that in alpha-helices with not more than 15 residues, there exist longitudinally opposing portions, one characteristically higher in average property potentials than the other. The helical chain, in general, acquires either an increasing or decreasing average potential in the N-terminal to C-terminal direction. The sequence-wise and surface-wise variations of property potentials in the elements of beta-structure also revealed such general patterns. Possible wrong predictions in statistical methods of one secondary structural class over the other are pointed out.

Sonography in the diagnosis of rhabdomyolysis.

This case report describes the use of musculoskeletal sonography in the diagnosis of rhabdomyolysis. The case involved an episode of severe muscle lysis following a heroin overdose in an addict who lay comatose for an uncertain period. Sonography revealed multiple hyperechoic areas within the muscles examined, consistent with a recent injury. The clinical diagnosis of rhabdomyolysis may be difficult but is important in view of the attendant danger of acute renal failure, and sonography was instrumental in the diagnosis in this case.
