Bacteremic W-135 meningococcal pneumonia.

Two epidemiologically unrelated cases of bacteremic meningococcal pneumonia are reported. Both patients were civilians without antecedent viral illness. The organism involved was Neisseria meningitidis, Group W-135, a serogroup of rapidly increasing importance in the United States. The difficulties in diagnosis and scope of meningococcal pneumonia are discussed.

Reversible oxacillin hepatotoxicity.

Eight patients developed elevations of hepatic enzymes while receiving oxacillin intravenously. In all instances the patients were asymptomatic and anicteric. Peripheral eosinophilia was present in five of eight patients. In each patient change of therapy to a different pencillinase-resistant penicillin or to penicillin G was associated with a rapid decrease in liver function abnormalities and evenutal return of the enzymatic values to normal. Change of medication to an alternative penicillinase-resistant penicillin or to penicillin G is suggested as a safe procedure for completion of antistaphylococcal therapy in patient who develop oxacillin-related hepatotoxicity.

R-factor transfer in selenite and tetrathionate broths.

After overnight incubation of R(+)Escherichia coli with R(-)Salmonella typhimurium in selenite and tetrathionate with Brilliant Green (TBG) broths, R-factor transfer was demonstrated in 10 of 12 experiments. R-factor transfer in these enrichment broths occurred at a markedly reduced frequency in comparison to that in Trypticase soy broth, apparently due to an adverse effect either on the viability of the donor E. coli or the conjugation process itself. Transfer of R factors in commonly used enrichment broths may give rise to falsely resistant antibiotic patterns in Salmonella. However, the frequency of R-factor transfer is so low that it is unlikely to affect significantly the interpretation of R-factor studies.
