Fox shield treatment of the UGH syndrome.

The uveitis-glaucoma-hyphema (UGH) syndrome has been described with all types of intraocular lenses. The usual forms of therapy include miotics, mydriatics, steroids, antiglaucomatous medications, laser therapy, and surgical explanation. I describe five patients in whom the UGH syndrome resolved after long-term Fox shield treatment to prevent nocturnal hand-eye trauma.

Fungal growth in aphakic soft contact lenses.

Fungal growth was detected in two aphakic hydrophilic contact lenses. Aspergillus species was cultured from the extended-wear contact lens and Dermatophilus congolensis was identified by specific antibody staining of the daily-wear contact lens. As in the 16 previously described cases of fungal contact lens growth, our patients had no evidence of ocular infection but did experience irritation which cleared up when contact lens wear was discontinued. Although only a small percentage of contact lenses are prescribed because of aphakia, these thicker contact lenses may be more susceptible to fungal growth and should be monitored carefully.

Ocular complications of electroconvulsive therapy.

Multiple cardioversion shocks were recently reported to cause disruption of the iris pigment epithelium. Pigment debris was deposited on the anterior lens surface, iris stroma, corneal endothelium, and trabecular meshwork. Similar findings were observed in 3 of 4 patients receiving more than 10 electroconvulsive therapy shocks and in a fifth patient who became attached to an electrified fence.
