Delirium in psychiatric inpatients: a case-control study.

OBJECTIVE: To investigate the clinical and pharmacoepidemiological determinants of delirium in a psychiatric inpatient population. METHOD: A case-control study design was used. Potential cases and potential controls were identified using hospital discharge data. The clinical record of each subject was reviewed using a validated protocol to confirm case and control status. Subsequently, exposure data were recorded from clinical records. RESULTS: Subjects admitted to hospital with delirium tended to be older, to have pre-existing cognitive deficits, and to have diagnoses of substance use disorders. Subjects who developed delirium after their admission to hospital were older than control subjects, more likely to have a history of cognitive impairment, and were significantly more likely to be treated during the hospitalization with lithium or anticholinergic antiparkinsonian medications. Antipsychotic medication exposures were also associated with delirium, but only at standard or above-standard dosage levels. Antidepressant and sedative-hypnotic medications were not associated with delirium. CONCLUSIONS: These findings indicate that using conservative dosages of antipsychotic medications and minimizing the use of anticholinergic medications for parkinsonian symptoms may help to prevent delirium in psychiatric inpatients. Anticonvulsant mood stabilizers may convey less delirium risk than lithium. Antidepressant medications and sedative-hypnotics were not important determinants of delirium in this population.

Pulmonary sequestration presenting as a prenatally detected suprarenal lesion in a neonate.

An echogenic suprarenal mass was detected antenatally in the fetus by routine ultrasound scan. Resection of the mass after delivery confirmed an extralobar pulmonary sequestration with features of a type II congenital cystic adenomatoid malformation of the lung. The lesion should be considered in all neonates presenting with a suprarenal or upper abdominal mass particularly when the mass has been detected antenatally. Ultrasound-guided needle biopsy if feasible could provide a reasonably confident diagnosis. Congenital anomalies in other organs should be looked for carefully.

Screening of newborn infants for hypothyroidism. Two years' experience in South Autralia.

Over a period of two years, 40,000 newborn infants in South Australia have been screened for congenital hypothyroidism by the measurement of thyroxine and, on selected samples, of thyroid stimulating hormone. Samples were those obtained for an established newborn screening programme in the State. Eight infants with hypothyroidism were detected; in seven, this was due to congenital agenesis of the thyroid gland and one infant had residual thyroid tissue. A further three sick infants had abnormalities of thyroid function; in two, this was transient and reverted to normal after several weeks and the third infant died. Three infants had thyroxine-binding globulin deficiency.

Statistical modelling in specific case analysis.

In spite of the problems associated with the development of a statistical approach in specific case analysis, the potential of two statistical methods, logistic regression and Bayesian belief networks, has been investigated and found encouraging in the context of a database relating to child murders with a sexual connotation. Continual collaboration between statisticians and detectives would be needed in the collection of the data, the choice and construction of the models, and the interpretation of the results.
