Comparison of the cost-effectiveness of three approaches to screening for and treating sight-threatening diabetic retinopathy.

The purpose of this study was to analyse and compare the costs involved in screening for and treating sight-threatening diabetic retinopathy in three different clinical settings. In the first setting, diabetologists screened using ophthalmoscopy and color photography, according to the St. Vincent Declaration guidelines, and selected patients for further assessment by a visiting ophthalmologist and for treatment in another hospital. In the second setting, all patients were regularly referred to ophthalmologists, either in the same hospital or elsewhere, for all aspects of eye care. In the third setting, screening was done again with ophthalmoscopy alone by diabetologists who followed the St. Vincent Declaration guidelines; however, further assessment and treatment were carried out in the eye department of the same hospital. Costs to the Italian National Health Service and to patients were calculated per screening performed and per patient subjected to laser treatment as a result of screening. A sensitivity analysis was then performed to simulate the costs of standardised patient populations going through the three different settings. It is concluded that absolute costs would be lower, both for the Italian National Health Service and for patients, if screening, assessment and treatment were all carried out in the same hospital. Equipping a diabetic clinic specially for screening would not be more expensive than delegating eye care to external parties, even for a hospital without an eye department. Moreover, delegating eye care more than doubles costs for patients. Screening for, assessing and treating sight-threatening diabetic retinopathy may be a cost-effective procedure for society as a whole in Italy.

Multiple transcription start sites of the carrot dihydrofolate reductase-thymidylate synthase gene, and sub-cellular localization of the bifunctional protein.

The analysis of clones obtained by rapid amplification of the 5' end and by primer extension of the mRNA for carrot bifunctional dihydrofolate reductase-thymidylate synthase showed transcripts of differing lengths that belonged to two sub-populations. The longer transcripts were found to contain a translation start site 147 nt upstream of, and in frame with, the one which is present in the shorter transcripts. The ORF that begins at this ATG codes for a protein of 64714 Da, which is much larger than mature DHFR-TS subunit. The N-terminus region of this polypeptide shows features typical of plant transit peptides. Immunogold labelling studies and immunorecognition of the plastid-containing sub-cellular fraction suggested a plastidial localisation of the bifunctional protein. Although plant cells were shown to contain folate pools in plastids, in mitochondria and in the cytosol, few enzymes of the folate pathway have been associated with any sub-cellular compartment. Thus, this is the first indication for the presence of an enzyme of the folate biosynthetic pathway in plastids. The longer transcripts revealed the presence of a TC microsatellite at the 5'-untranslated end.

[Results 37 years after insertion of an acrylic implant for scaphoid pseudoarthrosis].

Prosthetic treatment by means of a scaphoid implant was described by Swanson in 1970. The authors report the long-term results of pseudoarthrosis of the scaphoid treated by an acrylic implant at the Rizzoli Orthopaedic Institute in Bologna in 1956. Only one other similar case of long-term treatment by an artificial Vitallium (r) prosthesis was found in the literature.

Surgical treatment of Allman type III acromio-clavicular dislocation. A long-term follow-up study.

The authors report the results of a long-term follow-up study of surgical treatment of Allman type III acromio-clavicular dislocation. Despite numerous publications regarding the validity of closed treatment, the authors believe, in accordance with the anatomo functional and pathological knowledge, that open treatment is justified not only in young people but in older ones too. Among the different techniques, the authors prefer acromio-clavicular fixation with threaded wires because the operation time is short and the functional results are good with the lowest number of complications.

[Correlation between the grade of histological malignancy and nuclear positivity using the AgNOR technique in 6 cases of isolated bone NHL].

Several reports have shown that the degree of positivity for the AgNOR count can be used to evaluate the aggressiveness of malignancies since it express an increased protein-synthesis activity. This technique was applied to six cases of stage I osseous non-Hodgkin's lymphoma--4 intermediate grade (2 diffuse small, non-cleaved cell and 2 diffuse large, non-cleaved cell lymphomas) and 2 high-grade immunoblastic lymphomas--in order to assess the histologic grade and the number of AgNOR-positive regions. It was noted that survival together with the trend to invade the surrounding soft tissues (i.e., the tumour aggressiveness) correlated with the AgNOR granule count. Both factors were also related with Ki-67 cell-proliferation antibody positivity. Such correlations were even higher than those found with the histologic features conventionally evaluated in the Working Formulation, so they seem to convey more reliable indices of neoplastic growth potential.

Proteolytic and partial sequencing studies of the bifunctional dihydrofolate reductase-thymidylate synthase from Daucus carota.

The bifunctional dihydrofolate reductase-thymidylate synthase (DHFR-TS) of Daucus carota has been further characterized as regards molecular weight, amino acid composition, protease digestion and microsequencing of proteolytic peptides. Data reported in this paper demonstrate that the carrot protein has a calculated Mr of 124,000 thus indicating that, contrarily to what has previously been suggested, it occurs as a dimer of identical subunits. Results of partial amino acid microsequencing show the presence of sequences highly homologous with those of the active sites of both DHFR and TS from other organisms confirming, at the structural level, the bifunctional nature of the carrot protein. As in the case of Leishmania tropica DHFR-TS, incubation of the carrot protein with V8 protease led to a rapid loss of TS activity while retaining that of DHFR. However the pattern of proteolysis did not allow to establish whether the sequence of domains is DHFR-TS as in Leishmania, or vice versa. Low homology of other amino acid sequences, as judged by computer analysis, and absence of common epitopes indicate an apparent divergence between carrot and leishmanian proteins.

Osseous anomalies of the craniovertebral junction: a case report.

The authors describe the principal anomalies of the craniovertebral junction with particular reference to atlantooccipital fusion. They discuss the case of a female patient with pain and cervical contracture in whom a paracondylar process was revealed. Unless trauma brings it out, this abnormality can go unnoticed. CT scan is the only examination useful for diagnosis.

Biochemical, histochemical and immunohistochemical study of glycosaminoglycans in human meningiomas.

The localization and quantitation of glycosaminoglycans classes (GAGs) were studied in human meningiomas. Meningiomas presented high amounts of these compounds and electrophoretic separation revealed that they were 90% sulphated. The Alcian method and a polyclonal antiserum against chondroitin sulphate were used to localize the different GAGs in tissue sections. Quantitative and qualitative differences and different tissue distributions of GAGs were observed among transitional, syncytial and fibroblastic meningiomas. Syncytial meningiomas presented the lowest amount of GAGs and the immuno- and histochemical studies showed that they were located only in vessels and connectival trabeculae. Transitional meningiomas contained the highest concentration of GAGs; the percentage of the different GAG classes was similar to that observed in the syncytial oncotype indicating a quantitative but not qualitative difference between the two oncotypes. The high amount of GAGs in transitional meningiomas was attribute to the whorls, the structures stained by the histochemical and immunohistochemical techniques. The tumoral parenchyma of these two oncotypes was negative. On the contrary, fibroblastic meningiomas showed a fine meshwork among tumoral cells containing chondroitin sulphate and heparan sulphate. Biochemical data were consistent with the histochemical and immunohistochemical findings revealing a high percentage of chondroitin sulphate and heparan sulphate in fibroblastic meningiomas. This study suggests that the three meningioma types have different abilities to produce extracellular matrix components.

[Tunnel syndromes of the upper limb].

The main canalicular syndromes affecting the upper extremities are reviewed with a description of their aetiopathogenesis. Particular emphasis is placed on the need for early diagnosis and microsurgical treatment that on the basis of personal experience is held to guarantee early recovery.

[Prosthetic treatment of rhizoarthrosis of the thumb].

The aetiopathogenesis of trapeziometacarpal arthrosis or rhizoarthrosis of the thumb is described with particular emphasis on the secondary biomechanical alterations involved. The treatment of the condition and the use of the Swanson technique for the implantation of a Silastic prosthesis are described.

Lesions of the kidney in tourniquet shock: ultrastructural study.

Light microscope and ultrastructural changes in the rat kidney in tourniquet shock are described. The animals (24) were divided into two series; in the first one, the hind limb was kept at room temperature; in the second series, prerefrigeration was used. The effects of different ischemia were studied (2,4,8, and 12 hours for warm ischemia; 4,8,12, and 16 hours for cold ischemia). The animals were sacrificed at 3, 10, and 30 days. Pathological changes were evident, consisting in an increase of cytosomes in the proximal tubules and apical vacuolation in collecting ducts; numerous cytosomes including myelin-like structures were also seen. Large differences between warm and cold ischemia alterations were found. The changes in the tubules were clearly less important in cold ischemia. The authors conclude that there is a protective effect in regard to prevention of renal pathology affordable by prerefrigeration of a limb for reimplantation.

Flexor tendon grafts in the fingers.

The authors present the results of twenty-five tendon grafts performed between 1972 and 1982. The end results were evaluated according to these parameters: the pre-operative condition, the donor tendon, the level of the lesion, the age of the patient, and the time elapsed from injury to operation.
