The incidence of childhood diabetes in certain parts of Scotland.

The incidence of diabetes in children under 12 years of age has been studied in three regions of Scotland from 1976 to 1986. These areas represent more than half of the Scottish population of that age group. The overall annual incidence is 17.6 per 100,000, ranging from 16.8 in densely populated areas to 23.4 in more rural areas. Earlier studies have concerned the age group 0-18 years so the present results from a younger age group are, as expected, slightly lower but show the same trends. After a rise to 1983 there appears to have been a drop, but not in all areas. The danger of drawing conclusions from areas of low population or from short periods of time is stressed.

Frequency and severity of apnoea in lower respiratory tract infection in infancy.

Twenty four hour recordings of heart rate and respiration were made on 60 infants with lower respiratory tract infection (LRTI) during the acute phase of the illness, on 20 infants with upper respiratory tract infection (URTI) and on 21 control infants. Infants with LRTI and URTI had more apnoeic episodes of longer than 10 seconds compared with controls. The length of apnoeic episodes was greatest in the LRTI group but their frequency decreased with age. In both the LRTI and URTI groups apnoea was associated with bradycardia but the LRTI group also suffered an appreciable number of episodes of bradycardia without apnoea.

Long-term, low-dose treatment with pivmecillinam alone and in combination with pivampicillin in patients prone to recurrent bacteriuria.

The results of three separate studies aimed at evaluating the efficacy of long-term, low-dose treatment with pivmecillinam alone, and in combination with pivampicillin, in patients prone to recurrent bacteriuria, are presented. Overall, pivmecillinam and pivmecillinam/pivampicillin appeared to be equally effective, both clinically and bacteriologically, in a group of elderly subjects treated for 6 months. Pivmecillinam was also used successfully for up to 1 year in 10 out of 12 children in whom prophylactic therapy was indicated. The various regimens employed were without any significant toxic effects and were well tolerated.

A simple insulated cot for neonates.

Commercially available transport incubators are sophisticated and expensive since they are designed to provide life support for premature or critically ill neonates. In maternity hospitals with inadequate heating systems, some healthy infants have difficulty maintaining their body temperature in wards and during transfer within the hospital. These babies do not require the sophistication of commercial incubators. We describe a simple insulated cot which meets these requirements at minimal cost.

Infant respiration monitoring--evaluation of a simple home monitor.

A new, inexpensive commercial infant respiration monitor, the Baby-Sitter by Direct Diagnostics Ltd, has been evaluated and subjected to clinical trial. Dual respiration recordings were made from infants exhibiting apnoeic episodes to compare the new monitor with an acceptable reference monitor (Graesby Medical Model MR10). Both monitors are designed to detect respiration movement by means of pressure pulses produced in an air-filled sensor attached to the infant's abdomen. The new monitor is found to have unacceptable performance.
