[Diagnosis of vascular compression of the trigeminal and glossopharyngeal nerve roots].

Sixty-four patients with trigeminal and 12 with glossopharyngeal neuralgia were examined. Use was made of trigeminal and glossopharyngeal somatosensory evoked potentials (TSEP and GSEP). These results were compared to the finding during surgery in exploration of the cerebellopontile angle. Recording of TSEP and GSEP was found to be of high diagnostic value for the establishment of vascular compression of the nervous radicle in the cerebellopontile angle to determine indications for microvascular decompression.

Extraintracranial anastomosis performed by means of biological gluing materials: experimental and clinical study.

A new surgical technique of extraintracranial anastomosis (EIA) is reported. End-to-side microvascular anastomosis is performed by applying four crossed-fixing sutures and by mantling a hemostatic sponge muff (eg, Spongostan) impregnated with fibrinogen-thrombin glue. Histopathological studies of microvascular anastomosis in animals and clinical results of 70 EIA operations showed that this surgical technique of anastomosis has certain advantages, namely that it reduces the number of sutures needed, is less traumatic to the vascular wall, ensures elastic vascular junction, and the glue and Spongostan used possess high biological inertness that provides a better physiological connection of vessels than conventional suturing. The highly hermetic vascular junction and absence of blood flow at the anastomosis site are achieved without additional suturing. The duration of the operation is significantly reduced. A full EIA function sets in comparatively early.

[Utilization of magneto-biological effects in neurosurgery].

After referring to the application of magnetoes used until now for fixation of ferromagnetic substancies in aneurysms, the new results obtained with externally exployed magnetic fields are discussed. Target site is calculated according to stereotactic principles. In the magnetic field the circulating blood is influenced in such a manner, that in the anode region clotting will occure, wherease in the region of cathode the clotting will be inhibited. After developing the appropiate equipment, these conditions were studied on the basis of experimentally produced aneurysms in thyroid arteries of animals as well as in 30 ptients. It is assumed, that magnetic fields increase the immunoresistance of the organism and also exert a direct influence on the growth of tumourcells.

[Treatment of aneurysms of the cerebral arteries using stereotaxically oriented external uniform magnetic fields].

The paper presents a theoretical substantiation of the magnet-haemodynamic method of induced thrombosis of arterial aneurysms. The method is based on an "induced" bioelectrical phenomenon that develops in the circulating blood after the vessel in question is placed in a constant magnetic field, the field lines of which are perpendicular to the blood flow direction. Animal and in vitro experiments with induced thrombosis of 100 experimental aneurysms are described. For extrapolating the experiments into clinical conditions a special magnetic device was created that is provided with appliances for external stereotaxis and angiographic control. Nine clinical observations with induced thrombosis of aneurysms were conducted, the follow-up reaching 2 years.

[The diagnosis of the nature of the lesion of the trigeminal nerve root in trigeminal neuralgia].

The authors examined 64 patients with unilateral trigeminal neuralgia by computed tomography and registration of trigeminal somatosensory evoked potentials. The values of the evoked potentials on the involved side differed essentially from those on the healthy side. This allows vascular compression of the trigeminal nerve root in the posterior cranial fossa to be identified authentically and the indications for surgical treatment of trigeminal neuralgia by microvascular decompression thus determined. The diagnosis of vascular compression of the root of the trigeminal nerve was established in 33 patients, who were then operated on. The diagnosis was confirmed during the operation: pain was relieved completely in all patients after surgical treatment.

[Results of clinical application of methods of individual selection of chemotherapeutic preparations in the treatment of gliomas].

The method of autoradiography for testing the efficacy of cytostatics with subsequent individually chosen chemotherapy is one of the components of rational therapy for neuro-oncological patients. Purposeful chemotherapy was applied in 17 cases with gliomas. Favourable results were recorded in 70.6% of them. Twelve patients received regular courses of treatment with one agent which inhibited DNA synthesis in vitro most potently (in 7 the therapeutic result was favourable), 5 patients were given course of polychemotherapy (favourable effect in 100%). Prolonged survival and a good remission allow autoradiography to be recommended as a method for the individual choice of antiblastic agents.
