Applied humanism: a model for managing inappropriate behavior among mentally retarded elders.

1. Federal regulations mandate that elderly persons with mental retardation are provided with specialized programming which meets with the needs of the individual. 2. Behavior modification techniques employed to provide services to elderly persons with mental retardation may not meet individual patient needs, lacks constructs which apply to nursing philosophy/process and is difficult to implement with today's limited resources. 3. The philosophy of Applied Humanism may be more congruent with nursing practice, can be utilized with nursing process and can meet the individual needs of elderly patients with mental retardation.

Neuroleptic malignant syndrome.

Neuroleptic malignant syndrome is a potentially life-threatening reaction to neuroleptic agents. Its symptoms develop rapidly and may occur at the initiation of the neuroleptics or after long-term use. The pathogenesis remains unclear. The authors reviewed 93 documented cases published in the literature over the past five years. Demographic and clinical features of the syndrome are identified. Diagnostic criteria are proposed along with recommendations for treatment and further research.

Delayed hemiparesis following nonpenetrating carotid artery trauma.

A 21-year-old man developed transient, recurring right arm and leg weakness 2 weeks after a motorcycle accident. He ultimately proved to have an intimal tear and thrombosis of the left internal carotid artery with transient cerebral ischemia due to distal emboli. Neurologic deficits eventually resolved after anticoagulation and balloon occlusion of the vessel. The differential diagnosis of the patient's presentation is discussed, and the topic of nonpenetrating trauma of the carotid artery is reviewed, with special attention to mechanism of injury and clinical presentation.

Spinal epidural abscess: a rapidly progressive disease.

We report a case of spinal epidural abscess in a 42-year-old diabetic. The patient first presented with four days of low back pain with radicular symptoms following exertion. When he returned five days later with overt symptoms and signs of spinal cord compression, an epidural mass was diagnosed by myelogram and computed tomography scan. Following drainage of a large posterior epidural abscess, the patient was left with residual weakness requiring ongoing physical therapy.

Once-a-day drug regimen for psychiatric patients.

Patient compliance can be improved by reducing the complexity of drug dosage and by better labeling, packaging and informational exchange. Single bedtime doses not only increase compliance but also decrease side effects. The once-a-day regimen can reduce drug automatism and thus decrease acute confusional states, especially in the elderly.

Three cases of manic-depressive illness in mentally retarded adults.

The authors point out that manic-depressive illness is often not diagnosed in mentally retarded patients because they are already regarded as "different." They discuss the use of lithium carbonate as a prophylactic agent in the treatment of manic-depressive illness in mentally retarded patients and present three case histories illustrating some of the difficulties encountered in diagnosing mania or hypomania in such patients. They provide a number of suggestions to help physicians recognize and treat these disorders.

Reversible toxic psychosis.

Psychotropic medications may produce a toxic confusional state, especially in elderly patients. Tricylcic antidepressants particularly, and the phenothiazines to a lesser extent, may induce these behavioral changes by their anticholinergic activity. The situation may be compounded by the use of more than one psychotropic drug or the addition of an antihistamine or a belladonna alkaloid. Physostigmine can reverse the anticholinergic effects and clear the mental state.
