[The diagnosis and therapeutic approach in acute necrotic-hemorrhagic pancreatitis].

The work contains the clinical and therapeutic analysis of a group of 72 patients with acute necrotic-haemorrhagic pancreatitis (ANHP), admitted in the period 1979-1989 to the Ist Surgical Clinic of the Municipal Clinical Hospital of Bucharest. The etiology of ANHP, its clinical forms of manifestation, the peculiarities of shock and its dynamic course are shown and discussed. Stress is laid on the criteria of severity, on the course and on the complications occurred. The therapeutic results are commented in detail and the mortality is studied in a complex way, by analysing both the terminal clinical manifestations and the results of anatomicopathological examinations.

[Acute diffuse peritonitis, a rare complication of perianorectal suppuration].

The paper reports on two peculiar clinical observations of diffuse acute peritonitis in which the abdominal cavity was contaminated by perianorectal suppurative acute processes, insufficiently solved surgically. The diagnosis difficulties, peculiarities of the surgical treatment, and their special seriousness are revealed.

[Sensitivity to antibiotics and chemotherapeutic agents in some strains of Salmonella typhi isolated in Romania during the period 1974-1976].

Investigations were carried out on 781 S. typhi strains isolated from patients and carriers. Testing of the sensitivity to chemotherapeutical agents by diffusiometry and the incorporation technique led to the detection of four resistant strains: 1 strain isolated by coproculture from a carrier since 1958, resistant to A, K, N, Ca and Cf and 3 strains isolated by coproculture, biliculture and uroculture from a carrier since 1974, resistant to A and Ca. The R factors of these strains were transferred in their totality. The resistant strains did not manifest any particular pathogenicity for the laboratory animal, and their immunogenic potential was below that of the current typhoid vaccine. None of the 781S typhi were resistant to Cmx, C, G, T, Co and Po. Consequently, in Romania Chloramphenicol may be used in continuation in the treatment of typhoid fever, and Cotrimoxazol, Gentamycin, Tetracyclin and Ampicillin are and remain reserve substitutes. The possible unexpected appearance of resistant S. typhi strains demand, however, further control of the sensitivity to chemotherapeutics of all the isolated strains.

Anti-Bacterial Agents

[Tumors of the soft parts of the extremities. Anatomoclinical and therapeutic aspects].

A total of 60 cases are presented, of patients with tumours of the soft parts of the limbs, from a total of 14,000 surgical interventions. Of the total 18 were malignant tumours, while the remaining 42 cases had benign tumours. Tumours of the soft parts of the limbs include a large variety of histologic forms, and this raises particular problems of diagnosis and treatment. In the case of benign tumours the treatment is better codified, and consists in the removal of the tumour by simple enucleation. Recidives are possible because of the infra-clinical peritumoral invasion. In the case of malignant tumors the treatment is more differentiated, depending on the histologic type, the degree of malignancy, the stage of evolution, and on evolutive characteristics. Surgery remains an essential part of the treatment, and either limited or radical surgical solutions (amputation) are used. Frequently radiation and chemical therapy are associated, with good results. Six clinical observations are presented: rhabdomyosarcoma of the recidivating type, multi-recidivating fibrosarcoma, fibromyxosarcoma, varicose ulcers with malignant evolution, and two cases of liposarcoma. The cooperation is stressed, which should exist between the surgeon, the pathologist, radiologist and oncologist, for the diagnosis and treatment of these affections.

[Importance of hormone dependence of malignant tumors of the breast in orientation of the surgical treatment].

The authors have investigated the presence of oestrogenic and progesteronic receptors in 38 patients with breast cancer. It was noted that these receptors were present in 20 patients, and that the hormone-dependent tumours are more frequent in patients which are in the post-menopausal stage. The level of circulating oestrogens was also investigated, as well as the amount of oestrogens in the cytosol of tumor cells, and in the normal cells from the diseased breast. In patients with oestrogen-positive tumors, and/or progesteron-positive tumors, a rise was noted in the amount of circulating oestradiol, an increased contents of cytosolic oestradiol, especially in the tumoral cells as compared with the normal ones in the same breast. On the basis of these data patients were selected, with hormone-dependent breast tumors in view of ovarectomy. The indications are discussed, as well as the advantages of surgical ovarectomy.

[Phyllode tumors: anatomoclinical and therapeutic aspects (5 clinical cases)].

The phyllodes tumour is a bi-tissular mammary tumour. It is essentially benign but potentially sarcomatous, presenting multiple clinical and histologic-aspects, and raises difficult problems of diagnosis and therapeutic attitude. Four histological types are described, from the adenofibromatous type to the sarcomatous one. The notion of local malignancy is discussed, related to the marked recidivating character of the tumour. The morphologic substrate of the recidivating character is the result of the absence of a true capsule of the tumour. The treatment is differentiated according to the clinic, histologic, and recidivating aspects, and consists in either sectorectomy or breast amputation. Five cases are presented, of phyllodes tumour, each of them with particular characteristics of evolution, and imposing differentiated therapeutic attitudes.
