Attentional responses to unattended stimuli in human parietal cortex.

Right-sided parietal lesions lead to lateralized attentional deficits which are most prominent with bilateral stimulation. We determined how an irrelevant stimulus in the unattended hemifield alters attentional responses in parietal cortex during unilateral orienting. A trial consisted of a central spatial cue, a delay and a test phase during which a grating was presented at 9 degrees eccentricity. Subjects had to discriminate the orientation of the grating. The unattended hemifield was either empty or contained a second, irrelevant grating. We carried out a series of functional MRI (fMRI) studies in 35 healthy volunteers (13 men and 22 women, aged between 19 and 30 years) as well as a behavioural and structural lesion mapping study in 17 right-hemispheric lesion patients, 11 of whom had neglect. In the patients with but not in those without neglect, the addition of a distractor in the unattended hemifield significantly impaired performance if attention was directed contralesionally but not if it was directed ipsilesionally. In the healthy volunteers, we discerned two functionally distinct areas along the posterior-anterior axis of the intraparietal sulcus (IPS). The posterior, descending IPS segment in both hemispheres showed attentional enhancement of responses during contralateral attentional orienting and was unaffected by the presence of an irrelevant stimulus in the ignored hemifield. In contrast, the right-sided horizontal IPS segment showed a strong attentional response when subjects oriented to a stimulus in the relevant hemifield and an irrelevant stimulus was simultaneously present in the ignored hemifield, compared with unilateral stimulation. This effect was independent of the direction of attention. The symmetrical left-sided horizontal IPS segment showed the highest responses under the same circumstances, in combination with a contralateral bias during unilateral stimulation conditions. None of the six patients without neglect had a lesion of the horizontal IPS segment. In four of the 11 neglect patients, the lesion overlapped with the horizontal IPS activity cluster and lay in close proximity to it in another four. The remaining three patients had a lesion at a distance from the parietal cortex. Our findings reconcile the role of the IPS in endogenous attentional control with the clinically significant interaction between direction of attention and bilateral stimulation in right parietal lesion patients. Functional imaging in neglect patients will be necessary to assess IPS function in those cases where the structural lesion spares the middle IPS segment.

Enterogastric intussusception: a rare postoperative complication.

The authors present a case of postoperative enterogastric intussusception after previous Billroth II-gastrectomy, associated with a Braun's W-anastomosis of the efferent small bowel loop. Loss of viability of the incarcerated jejunal loop required segmental small bowel resection. Total recovery was obtained. Literature is reviewed concerning diagnosis and treatment of adult intestinal intussusception.

Estimating daily fat yield from a single milking on test day for herds with a robotic milking system.

The objective of this study was to estimate the daily fat yield and fat percentage from one sampled milking per cow per test day in an automatic milking system herd, when the milking times and milk yields of all individual milkings are recorded by the automatic milking system. Multiple regression models were used to estimate the 24-h fat percentage when only one milking is sampled for components and milk yields and milking times are known for all milkings in the 24-h period before the sampled milking. In total, 10,697 cow test day records, from 595 herd tests at 91 Dutch herds milked with an automatic milking system, were used. The best model to predict 24-h fat percentage included fat percentage, protein percentage, milk yield and milking interval of the sampled milking, milk yield, and milking interval of the preceding milking, and the interaction between milking interval and the ratio of fat and protein percentage of the sampled milking. This model gave a standard deviation of the prediction error (SE) for 24-h fat percentage of 0.321 and a correlation between the predicted and actual 24-h fat percentage of 0.910. For the 24-h fat yield, we found SE = 90 g and correlation = 0.967. This precision is slightly better than that of present a.m.-p.m. testing schemes. Extra attention must be paid to correctly matching the sample jars and the milkings. Furthermore, milkings with an interval of less than 4 h must be excluded from sampling as well as milkings that are interrupted or that follow an interrupted milking. Under these restrictions (correct matching, interval of at least 4 h, and no interrupted milking), one sampled milking suffices to get a satisfactory estimate for the test-day fat yield.

Synergetic effect of pimozide and thyrotropin releasing hormone on prolactin and thyrotropin release during the drying off of ewes.

The effect of pimozide and/or TRH was investigated on plasma prolactin, thyrotropin, T(4) and T(3) and udder distension in 38 ewes during drying off by feed restriction. The effect of daily injections of 2mg pimozide (s.c.), combined or not with TRH stimulation (200µg, i.v.) on three different days of the drying off period was examined. Blood samples were taken twice daily in each group for 9 days, while blood sampling on the days of TRH injection was also performed at 0, 15, 30min, and 1, 2 and 4h post-injection. Plasma was assayed for PRL, TSH, T(4) and T(3) levels. Udder distension and mastitis incidence were recorded at the end of the drying off period. TRH and pimozide both resulted in elevated plasma PRL levels and acted in a synergetic way. Udder distension and the incidence of mastitis was only influenced by pimozide. The TSH as well as the T(3) response to TRH was increased in ewes under a continuous influence of pimozide and T(3) peaks following TRH injection occurred earlier than T(4) peaks. The higher effect of pimozide upon TRH stimulated PRL and TSH release at day 8 compared to days 0 and 3 indicates a progressive involvement of dopamine on the inhibition of PRL and the sensitivity of the thyrotrophs to TRH during drying off.

Regional physiological adaptation of the central nervous system deiodinases to iodine deficiency.

The goal of the present investigation was to analyze the types 2 (D2) and 3 (D3) iodothyronine deiodinases in various structures within the central nervous system (CNS) in response to iodine deficiency. After 5-6 wk of low-iodine diet (LID) or LID + 2 microg potassium iodide/ml (LID + KI; control), rats' brains were processed for in situ hybridization histochemistry for D2 and D3 mRNA or dissected, frozen in liquid nitrogen, and processed for D2 and D3 activities. LID did not affect weight gain or serum triiodothyronine, but plasma thyroxine (T4) was undetectable. In the LID + KI animals, D3 activities were highest in the cerebral cortex (CO) and hippocampus (HI), followed by the olfactory bulb and was lowest in cerebellum (CE). Iodine deficiency decreased D3 mRNA expression in all CNS regions, and these changes were accompanied by three- to eightfold decreases in D3 activity. In control animals, D2 activity in the medial basal hypothalamus (MBH) was similar to that in pituitary gland. Of the CNS D2-expressing regions analyzed, the two most responsive to iodine deficiency were the CO and HI, in which an approximately 20-fold increase in D2 activity occurred. Other regions, i.e., CE, lateral hypothalamus, MBH, and pituitary gland, showed smaller increases. The distribution of and changes in D2 mRNA were similar to those of D2 activity. Our results indicate that decreases in the expression of D3 and increases in D2 are an integral peripheral component of the physiological response of the CNS to iodine deficiency.

Type 2 iodothyronin deiodinase transgene expression in the mouse heart causes cardiac-specific thyrotoxicosis.

Type 2 iodothyronine deiodinase (D(2)) catalyzes intracellular 3, 5, 3' triiodothyronine (T(3)) production from thyroxine (T(4)), and its messenger RNA mRNA is highly expressed in human, but not rodent, myocardium. The goal of this study was to identify the effects of D(2) expression in the mouse myocardium on cardiac function and gene expression. We prepared transgenic (TG) mice in which human D(2) expression was driven by the alpha-MHC promoter. Despite high myocardial D(2) activity, myocardial T(3) was, at most, minimally increased in TG myocardium. Although, plasma T(3) and T(4), growth rate as well as the heart weight was not affected by TG expression, there was a significant increase in heart rate of the isolated perfused hearts, from 284 +/-12 to 350 +/- 7 beats/min. This was accompanied by an increase in pacemaker channel (HCN2) but not alpha-MHC or SERCA II messenger RNA levels. Biochemical studies and (31)P-NMR spectroscopy showed significantly lower levels of phosphocreatine and creatine in TG hearts. These results suggest that even mild chronic myocardial thyrotoxicosis, such as may occur in human hyperthyroidism, can cause tachycardia and associated changes in high energy phosphate compounds independent of an increase in SERCA II and alpha-MHC.

Pre-ovulatory follicular characteristics and ovulation rates in different breed crosses, carriers or non-carriers of the Booroola or Cambridge fecundity gene.

Terminal follicular dynamics and ovulation rates (OR) were compared in different local breeds after introducing fecundity genes of different origin. Crossbred ewes which were carriers (F+) or non-carriers (++) of Booroola (BFec) or Cambridge genes (CFec) were included: CambridgexCambridge (CC), CambridgexSuffolk (CS), CambridgexTexel (CT), BooroolaxTexel (BT) and BooroolaxGerman Mutton Merino (BGM). The numbers of small (diameter 2-3.5 mm), medium (diameter >3.5-5.0 mm) and large (diameter >5.0 mm) growing follicles, the maximum diameter before ovulation and the regression and artesia rates of ovarian follicles >/=2 mm in diameter were studied laparoscopically and repeatedly during the last 5 days of an induced oestrous cycle. The ORs were determined one cycle before and two cycles after the repeated laparoscopy. BFec and CFec significantly enhanced the OR of all crossbreeds. Carriers of BFec or CFec did not have significantly different ORs due to any crossbreeding effect. The same observation was made for non-carriers of both Fec gene types. Whatever the crossbreed, the number of small, medium and large growing follicles were similar between carriers and non-carriers in spite of a higher number of ovulating follicles in carriers of both Fec gene types. The diameter of ovulatory follicles did not differ among crossbreds, or between carriers and non-carriers except in the BT (5.2+/-0.2 vs. 6.5+/-0.8 mm, respectively) and CC (6.6+/-0.2 vs. 5.6+/-0.3 mm) ewes.The higher OR in the presence of the Booroola gene was associated with a low atresia rate of large follicles in all crossbreeds (BT: 52+/-8% (F+) vs. 61+/-7% (++); BGM: 51+/-6% vs. 75+/-5%). The high OR of the carriers of the CFec gene seemed to be associated with a lower number of large growing follicles with a lower (P<0.05) atresia rate as compared with Booroola crossbreeds. In conclusion, follicular features were similar between purebred Cambridge and its crossbred CS and CT. In ewes carrying the BFec or CFec gene, the reduction in follicular atresia seemed to be one of the main follicular features implicated in the higher OR.

Special designed RF-antenna with integrated non-invasive carbon electrodes for simultaneous magnetic resonance imaging and electroencephalography acquisition at 7T.

The construction of a high quality MR RF-antenna with incorporated EEG electrodes for simultaneous MRI and EEG acquisition is presented. The antenna comprises an active decoupled surface coil for receiving the MR signal and a whole body coil for transmitting the excitation RF pulses. The surface coil offers a high signal-to-noise ratio required for fMRI application and the whole body coil has a good B(1) excitation profile, which enables the application of homogeneous RF pulses. Non-invasive carbon electrodes are used in order to minimise the magnetic susceptibility artefacts that occur upon application of conductive materials. This dedicated set-up is compared to a standard set-up being a linear birdcage coil and commercially available Ag/AgCl electrodes. As the acquired EEG signals are heavily disturbed by the gradient switching, intelligent filtering is applied to obtain a clean EEG signal.

Non invasive in vivo anatomical studies of the oscine brain by high resolution MRI microscopy.

We describe in this paper an in vivo Magnetic Resonance Imaging (MRI) procedure that allows one to obtain three-dimensional high quality images of the entire brain of small passerine birds such as the canary with a slice thickness of 58 micron and an image resolution of 78 microns. This imaging procedure was completed in 70 min on anaesthetised birds that later recovered uneventfully and could be reused for subsequent additional imaging. To illustrate the high resolution and anatomical detail that can be achieved, examples of coronal images through the entire hypothalamus are provided in the same sectioning plane as the previously published canary brain atlas. The data set can be used to create sections in any desired plane and the entire data set can be viewed from any point of view in a volume rendered image. This provides a useful tool in understanding the three-dimensional organisation of the brain. Similar procedures can also be applied on fixed brains and might allow an even better anatomical resolution of images because time constrains no longer limit the duration of image acquisition. The non-invasive MRI technique enables to study neuroanatomical features with a high resolution and without killing the animal subjects so that measures can be obtained in a same individual both before and after an experimental treatment.

Burns to the genitalia and the perineum.

PURPOSE: We conducted a study to determine the treatment of perineal and genital burns, the results of therapy and the complications of such burns. MATERIALS AND METHODS: A review of the records of 4,216 patients treated between 1981 and 1995 at the Burn Center of the Academic Surgical Center Stuivenberg, revealed 87 male and 30 female patients, 6 months to 86 years old who had associated burns to the perineum or genitalia. Mean burn size was 21% of the total body surface area. Causes of burn injury were scald in 55% of the cases, flame in 24%, chemical in 16% and others in 4%. RESULTS: There were 16 deaths in this group (13.6%) but none was related to the perineal or genital burns. Of the 101 survivors 41% required Foley catheters but the catheters were indwelling during resuscitation only (range 1 to 99 days). Perineal and genital burns were treated by topical antimicrobials. Only 10 patients (9.9%) required split-thickness skin grafts. As late complication 2 patients had scar formation of the penile shaft, which was treated with multiple Z-plasties, and of the prepuce, which was treated by circumcision, respectively. In 1 patient, who presented with erectile dysfunction diagnostic evaluation was negative. CONCLUSIONS: Conservative management of perineal and genital burns is recommended.

Robustness testing of a liquid chromatography method for the determination of vorozole and its related compounds in oral tablets.

The robustness of a method for the determination of vorozole in oral tablets was examined by applying a two-level, seven factor Plackett-Burman statistical experimental design. Five method variables that are sensitive to variation, especially during method transfer, were evaluated for their influence on the system suitability criteria set in the method procedure and on the analysis time. The method variables were investigated in a specified range above and below the nominal method conditions. They included the concentration of an ion-pairing agent, the percentage organic modifier at the start of the linear gradient, the mobile phase flow rate, the percentage organic modifier at the end of the linear gradient and the pH of the mobile phase. Two dummy factors were included in the design to estimate the experimental error. It was found that none of the five studied variables affected significantly (t-test, alpha = 0.01) the capacity factor, the tailing factor or the analysis time. The resolution of the critical peak pair on the other hand, was significantly influenced by the factor pH. However, the responses for the resolution of all the experimental runs in the design were well above the system suitability limit stated in the normal assay procedure. Therefore, the method can overall be considered robust.

Evidence for the presence of immunoreactive inhibin in extragonadal tissues of ovariectomized ewes.

Six ewes were ovariectomized to determine the immediate and long-term effects of removal of ovaries on the immunoreactive concentrations of FSH, LH and inhibin. Three months after ovariectomy, ewes were slaughtered and tissue samples of brain, pituitary, spleen, liver, perirenal fat, lung, kidney, adrenals and uterus were collected to determine the immunoreactive inhibin content. Both gonadotrophins, FSH and LH, increased significantly after ovariectomy. The increase of FSH, however, was more pronounced and remarkably faster than the changes of LH after ovariectomy. Immunoreactive concentrations of inhibin decreased sharply as early as 15 min after ovariectomy and subsequently decreased more gradually until 2 weeks after surgery. From this moment on, the level stabilized at 56% of the initial value. In control ewes, a considerable amount of immunoreactive inhibin is found in tissue samples of ovary, lung, kidney, pituitary and spleen. After ovariectomy, the level of immunoreactive inhibin decreased in spleen and lung samples while an important increase of immunoreactive inhibin is found in adrenals and pituitary. These results demonstrate a differential regulation of LH and FSH after ovariectomy and support an involvement of inhibin only in the immediate changes of FSH after ovariectomy in sheep. They further suggest that the adrenals and the pituitary may be extragonadal sources of inhibin. To explore the eventual contribution of the adrenals to circulating inhibin, dexamethasone (1.4 mg/ewe) and ACTH (200 IU/ewe) were in a following experiment injected intravenously in control and ovariectomized ewes. The lack of any effect of dexamethasone or ACTH on the plasma concentration of immunoreactive inhibin indicate that adrenal inhibin probably does not contribute to circulating inhibin.

Burns to the genitals and the perineum in children.

OBJECTIVE: To determine the management of perineal and genital burns in children. PATIENTS AND METHODS: Twenty-seven children (aged 6 months to 12 years) suffering from genital or perineal burns between 1981 and 1995 were reviewed. Most burns occurred in boys (70%), were due to scalds (85%) and caused superficial second-degree lesions (63%). The initial treatment consisted of topical antimicrobials or grafting with allografts. RESULTS: Spontaneous healing by conservative therapy, not early excision therapy, occurred in 96% of the patients. Contractures occurred in two patients and were treated with multiple Z-plasties and circumcision. CONCLUSION: The management of perineal and genital burns in children should be conservative.

Cervical cotinine and macrophage-Langerhans cell density in the normal human uterine cervix.

Cotinine levels in blood and cervical fluid of smokers and non-smokers were analysed using capillary-column gas chromatography. These levels were not related to numerical cell densities of intraepithelial S100-protein- and LN2-positive Langerhans cells or to MAC-387-positive macrophages in the stroma of the transformation zone of normal uterine cervices. A decrease in the number of Langerhans cells was noted in smokers, especially in those using oral contraceptives (OCs). Macrophages were more numerous in the endocervical stroma of smokers, suggesting a local response to smoke constituents. These findings may indicate a synergistic suppression of local cervical immunity by smoking and OCs.

[Illegal drug use by secondary school children in Belgium: north-south differences].

The study "Health Behaviour of Youngsters" regularly provides comparative data on health behaviour and related lifestyle of pupils from 11 to 18 years old. This study is based on repeated surveys among randomised representative samples of pupils, following the international standardised protocol from the WHO cross-national survey "Health Behaviour in School-aged Children". In Belgium, results from the 1990 survey (sample = 3528) show differences in illegal drugs use between the South (French-speaking) and the North (Flemish-speaking). For example, among the 17 year olds, 9.8% of the French-speaking pupils and 5.5% of the Flemish-speaking have already tried illegal drug(s) and in the same age groups, 2.9% and 1.5%, respectively declare to be regular users. The first hypotheses concerning those differences take into account the socio-economic and cultural environment as well as methodological problems inherent to these surveys.

Tobacco smoking and the uterine cervix: cotinine in blood, urine and cervical fluid.

Cotinine levels in blood, urine and cervical fluid of smokers and nonsmokers were analyzed by capillary-column gas chromatography. The sensitivity of this method appeared to be 100%. The specificity was lower (87.5% in blood, 25% in urine and 75% in cervical fluid). Nonsmokers exposed to smoke by others had low but detectable cotinine levels in the three body fluids. The highest cotinine levels in cervical fluid were detected during the proliferative phase of the cycle. Cotinine levels in cervical fluid and blood correlated well, but the correlation was less during the proliferation phase. Cotinine measurement in cervical fluid proves to be a reliable method to quantify exposure to tobacco smoke, even when induced by others.
