[Non-parasitic splenic cysts].

Nonparasitic cysts of the spleen are uncommon and often result from blunt abdominal trauma. Nonsurgical management of blunt splenic injuries increases the number of observations of the post-traumatic cysts. Complications (infection, rupture and hemorrhage) are lifethreatening, difficult to diagnose and require urgent surgical management. Until recently, splenectomy has been the primary choice of treatment of these cysts. Small (< 4 cm) asymptomatic post-traumatic pseudocysts stand a reasonable chance of involution with time (3-36 months) and so may be initially observed. Splenic preservation by partial splenectomy, enucleation or by marsupialization is actually recommended in children when technically feasible. Splenectomy is required for voluminous, central, multifocal cysts, in the presence of complications and in the adults with low immunologic risk. The Authors report 5 cases of large cysts successfully treated by splenectomy with one 12-year-old girl treated in emergency for infection by Salmonella.

Correlations between sinusoidal pressure and liver morphology in cirrhosis.

Twenty-three patients with cirrhosis (alcoholic, post-hepatitic and cryptogenetic), who underwent hepatic venous catheterization and hepatic biopsy, were included in a histopathologic study to identify morphologic features correlated with the values of corrected sinusoidal pressure. The alterations in acinar architecture were investigated by histomorphometry and could be described as: a) multiacinar fibrosis with partial persistence of normal afferent and efferent vascular structures; b) complex acinar fibrosis with partial persistence of afferent structures alone; and c) simple acinar and subacinar fibrosis with almost complete disappearance of normal vascular structures. A significant negative correlation (p < 0.05) was found between corrected sinusoidal pressure values and the percentage of portal spaces not involved in the process of bridging fibrosis, indicated as "residual portal spaces". Residual portal space evaluation can therefore be suggested as an easily quantifiable morphologic feature indicative of portal pressure.

[Radical surgery and loco-regional chemotherapy in the treatment of liver metastases of stomach and colorectal cancer].

The results of surgical treatment of hepatic metastasis from stomach and colo-rectal cancer in a series of 28 patients have been assessed. 21 cases were subjected to hepatic resection (stomach cancer: 7 cases; colorectal: 14 cases) and 7 underwent locoregional chemotherapy through the gastroduodenal artery with fully implantable catheters (stomach ca.: 1 case; colorectal ca.: 6 cases). Of patients who underwent hepatic resection, 3 died postoperatively and 4 are still alive 5 years after the operation. Of the 7 patients treated with locoregional chemotherapy, 4 can be evaluated: 2 responded to treatment, 1 stabilised and 1 progressed. Clinically noteworthy toxicity was observed in one patient only.

The legionellosis.

Following the discovery of Legionella pneumophila as the cause of an epidemic of pneumonia at an American Legion Convention in Philadelphia, a group of related bacteria were recognized as additional human pathogens. This newly established bacteria genus, Legionella, includes the agents of Legionnaires' Disease, Pittsburgh pneumonia and several related infections. A number of researches have been performed in the past few years about these bacteria; many of these data are here summarized to give an idea of the most important characteristics of Legionella and of the diseases they cause.
