[Postnatal monitoring of the newborn infant using cardiotachography. 1. Normal cardiotachogram in the newborn].

Cardiotachograms in a group of 30 vigorous newborns have permitted the study of the typical signs of the regulatory mechanism of neonatal circulation during the first minutes of life. Immediately after birth neonatal heart rate rise to a peak heart rate of 177 bpm at the age of two minutes. Thereupon neonatal heart rate falls gradually and reaches a stabil level in the 13. minute after delivery. In the phase of gradually fall in neonatal heart rate many brief episodes of cardiac deceleration ("spikes") were observed.

[Intraamniotic amino acid infusions in chronic placental insufficiency].

12 pregnant women in the gestational weeks 33 to 39 who had symptoms of chronic placental insufficiency received 22 intraamniotic amino acids infusions. 250 ml of a 5 per cent solution of L-amino-acids was infused within 12 hours. During these infusions nearly 80 per cent of the amino acids (according to alpha-amino-nitrogen) were eliminated from the amniotic fluid in 24 hours. The highest elimination rate with 83.4 per cent has been found in cases with intrauterine growth retardation. The smallest rise of urea-nitrogen-level after intraamniotic amino acid infusion has been observed in cases with intrauterine growth retardation and in the second or third infusion. The decrease of amniotic fluid protein content after infusion was on the average about 30 per cent. The interpretation of these results is difficult, because so far not exist examinations of the gastrointestinal resorption and metabolism of amino acids and amniotic fluid proteins by the human fetus. According to our experiences it is possible to supply fetus with severe intrauterine growth retardation with vital substrates on paraplacental way. But the postnatal treatment of the small for dates babies is more effective.

[Production of obstetrical reports using a computer].

A rational method of production of medical reports in obstetrics is reported. The basis of this method is the approved computer guided system (DOKUMEDA) in clinics of radiology. Advantages and disadvantages of using are discussed.

[Results of standard-value determinations in newborn infants in the German Democratic Republic. II. Head circumference, biparietal and fronto-occipital diameter].

Using the measurements taken from 6645 male and 6381 female newborns from the GDR-population the normal values from head circumference, fronto-occipital and biparietal diameter are reported. The results were given in relation to duration of pregnancy (31. to 44. week of pregnancy) expressed in percentile tables. Head circumference and fronto-occipital diameter were larger in males than in females. The head circumference values are compared directly and in their relation to birth weight and length with the data reported by other authors.

[Results of a standard value determination concerning newborn infants in the German Democratic Republic. III. Morphologic assessment of maturity].

A simple technique for estimating of maturity in 15589 newborns without technical expense were performed in connection with the examination of birthweight, length, head circumference and head diameters at a gestational age of 31st to 44th week in 21 women's clinics in the GDR. The estimation of maturity is based on the gradual assessment of 8 external physical characteristics. The coefficients of correlation is not differed between characteristics of maturity, measures of newborns and duration of pregnancy. It is not possible to make an accurate statement of duration of pregnancy by this score of maturity than by measures of the newborns.

[Sialic acid, steroids and proteohormones in maternal, cord and retroplacental blood].

BACKGROUND: To study the storage of sialic acid in newborns reference concentrations for sialic acid were measured in maternal, retroplacental and cord blood and compared with the concentration of human placental lactogen (hPL) and estriol (E3). High serum concentrations of hPL and E3 in retroplacental blood indicate the synthesis of these products in the fetoplacental unit. The comparison of the serum concentrations give first informations for a possible role of the placenta as a place of production and storage of the investigated products. METHODS: The concentrations of sialic acid, hPL and unconjugated E3 were determined in maternal and retroplacental blood samples of 126 pregnant women (16-42 years old) between 28 and 42 weeks of gestation. 84 of these pregnant women had uncomplicated pregnancy with birth after 37 gestational weeks. Measurements of E3 and hPL were performed by solid phase radioimmunoassays. Concentrations of sialic acid were determined by HPLC (high performance liquid chromatography). RESULTS: Means and medians of the three parameters for both groups differentiate hardly. The retroplacental serum concentrations of hPL and E3 are increased significantly compared with maternal blood. The same trend was found for sialic acid without significance. The highest concentrations of E3 were found in the cord blood (298.2 +/- 138.0 ng/ml) (p < 0.01). On the other hand the lowest concentrations of sialic acid (36.1 +/- 19.6 mg/l) (p < 0.01) were estimated in cord blood samples. It was estimated a significant correlation between fetal and retroplacental concentrations of E3. Significant correlations (p < 0.01) were found for sialic acid between maternal and retroplacental blood on the one side and maternal and the cord blood on the other side. Significant increased mean sialic acid concentrations in retroplacental blood (x = 102.67 mg/l) were found in female newborns in comparison with male newborns (x = 80.58 mg/l). There were not significant differences between prematurity and term delivery. CONCLUSION: Increased sialic acid concentrations in retroplacental blood samples are a sign of sialic acid accumulation in the fetomaternal area aiming to induce the tolerance of fetal allotransplantat. There are no evidence for a take up of free sialic acid by fetus.
