Fas expression on T cells and sFas in relapsing-remitting multiple sclerosis.

OBJECTIVES: To investigate the proportions of peripheral blood CD4+/Fas+ and CD8+/Fas+ cells and serum sFas levels in relapsing-remitting multiple sclerosis (RRMS) patients with relapses (active RRMS), those without relapses (stable RRMS), and controls over 1 year. MATERIAL AND METHODS: Sixteen RRMS patients and 10 controls were tested monthly. Cells were analyzed by dual immunofluorescence and the sFas levels by ELISA. There were 14 relapses which occurred 1223 days after the last control visits. The measurements performed at these visits in the active RRMS patients were considered as relapse-related, while the rest were regarded as relapse-unrelated. RESULTS: In active RRMS patients the median of CD4+ Fas+ to total CD4+ and CD8+ Fas+ to total CD8+ from relapse-related measurements were higher than the median from relapse-unrelated measurements (P=0.003, 0.004, respectively). The median of CD4+ Fas+ to total CD4+ from relapse-unrelated measurements in active RRMS was higher compared with stable RRMS (P = 0.005) and controls (P = 0.004). The sFas level from relapse-unrelated measurements was also higher in active RRMS than in stable RRMS (P = 0.04) and in controls (P = 0.004). CONCLUSIONS: We suggest that increased expression of Fas antigen on CD4+ subset and increased serum sFas level are valuable markers of clinical activity in MS.

[Multimodal evoked potentials in malabsorption syndrome].

The multimodal evoked potentials (visual, somatosensory and auditory brainstem) in 23 patients with malabsorption syndrome of different origin were investigated. The diagnosis of the disease was confirmed on the basis of histological examination and result of D-ksyloza test. The control group consisted of 30 healthy persons. Examination of visual evoked potentials revealed significant prolongation of latency of P 100 component in examined group in comparison with controls. Latency of N13 and N20 of somatosensory evoked potentials in patients with malabsorption syndrome were also significantly prolonged when compared to controls, otherwise transit time to cortex (TTC) was merely prolonged. Auditory brainstem potentials were also abnormal. Changes involved prolongation of latency of I, III and V responses and prolongation of interpeak latencies of I-III, III-V, I-V as well. The amplitudes of the examined evoked potentials between patients and controls did not differ significantly. On the basis of obtained results in was pointed out that different specific afferent systems are affected in patients with malabsorption syndrome what seems to be connected with vitamin's deficiency, especially B12 and E. Authors conclude that multimodal evoked potentials examinations are useful in the diagnosis and monitoring of the disease, specially in subclinical cases.

[Restless legs syndrome].

The authors present recent data about epidemiology, pathogenesis and clinical features of idiopathic and symptomatic restless legs syndrome. A new aspects of pharmacological treatment and modification of daily activity are also described.

[Aspergillus spondylodiscitis].

Four weeks after an Amanita phalloides poisoning, a 29-year-old man presented an ocular aspergillosis. One month later he developed a bilateral L5-S1 radicular syndrome. The diagnosis of aspergillar spondylodiscities was established on the basis of x-ray, CT-scan, MRI and pathology findings. Clinical and radiological improvement was obtained with fluconazole.

[The level of soluble forms of interleukin-2 receptor and adhesive molecules ICAM-1 and VCAM-1 in platelets of multiple sclerosis patients].

The levels of sIL-2R alpha, sICAM-1, and sVCAM-1 in sera of patients with clinically probable multiple sclerosis (ms) during the first relapse were assessed. In ms patients the levels of these 3 soluble molecules were significantly increased in comparison with the results obtained in controls. The results reflected activation of lymphocytes T and enhancement expression of adhesion molecules on brain endothelial cells, therefore they could serve indirectly as markers of immunological activation in multiple sclerosis. Any of the levels of soluble molecules in sera did not correlate with the severity of the disease.

[Neurological complications in patients with chronic renal failure].

On the basis of current literature and their own experiences, the authors present the most common symptoms and syndromes of dysfunction of central and peripheral nervous system which appear in patients with chronic renal insufficiency. Current views on patomechanism of neural dysfunction due to renal failure in dialysed and non-dialysed patients are discussed.

[Sensory conduction and central somatosensory conduction time in toxic polyneuropathy].

The "transit time to cortex" was estimated in the group of 15 patients living around the copperworks. The somatosensory evoked potentials method has been applied. Clinical and neurographic disturbances (polyneuropathy with prolonged sensory conduction velocity) in this group of patients was accompanied by disturbances of the central nervous system ("transit time to cortex" was prolonged). The examination of averaged evoked potentials has a great value in estimation of early stages of toxic damages of the central nervous system and can be applied in the prevention of chronic industrial poisonings.

HLA antigens and susceptibility to myasthenia gravis.

HLA-A, -B, -C and -DR antigen frequencies determined in a group of 73 myasthenia gravis (MG) patients were compared with those of a control group of 205 subjects. The strongest positive association with MG was found antigens B8 and DR3 (relative risks 9.56 and 8.84 respectively). Analysis of our data indicates that both antigens, independently from their linkage disequilibrium, are involved in susceptibility to MG. No relationship between HLA antigens on thymic pathology was observed in our material. In male MG patients the association with DR3 was weaker than in female patients. The difference in DR3 frequency between male and female patients was statistically significant; no significant difference was found for antigen B8. It appears that DR3 contributes to development of MG only in females. In male patients aged more than 30 years at the onset of disease, MG was not associated with B8 or DR3. In contrast, in female patients aged more than 30 years at the onset of disease there was a strong association of B8 and DR3 with the disease.

[Behavior of visual evoked potentials (pattern reversal VECP) in smokers].

Twenty six clinically healthy smokers were studied with transient pattern visual-evoked potentials. In comparison with a sample of "normal" subjects we found a difference of some parameters (P less than 0.05): the P2-latency and the interocular difference of latency on average were 6-7 ms respectively 2 ms larger. Thirteen subjects have had a prolonged P2-latency (P less than 0.05) and/or a difference in interocular latencies larger than 5 ms. These findings may suggest on subclinical disturbances of visual system and indicators of an special sensitivity of that person. The findings are possibly related to tobacco-alcohol amblyopia.

[Cauda equina neurography. Results from normal persons and patients with lumbosacral root compression syndrome].

The results with the cauda-equina-neurography in more than 150 patients demonstrate a value for differentiation of lumbosacral disorders. The action potentials of normal adults are mostly biphasic, have the same configuration for both sides and their amplitude is between 0,3 and 20 uV. The conduction velocity decreases with age for both nerves. In cases of lumbosacral disorders with isolated nerve root affection by discogenic compressions the potentials are delayed, reduced, polyphasic or absent. Mostly the clinical, electromyographic, myelographic and surgical results correspond with neurographic examination. The examination is possible since technical problems permit a good differentiation and localisation of root lesions.

[Clinical-electrophysiological characteristics of Alzheimer's dementia and cerebrovascular dementia based on determination of cerebral refraction time].

The aim of the study was an electrophysiologic assessment of cerebral refractory period in relation to chosen clinical parameters, EEG and CT in patients with dementia of various origin. The study included 30 patients (11 female, 19 male) aged 50-75: 15 with dementia of Alzheimer type (DAT) and 15 with vascular dementia (VD), and 15 age-matched healthy controls. Cerebral refractory period (CRP) was assessed by means of somatosensory evoked potentials with the use of paired stimulation with varying interstimulus interval, applied to the median nerve. Interstimulus interval, with which one somatosensory response was obtained to paired stimuli, was determined as absolute refractory period. CRP was significantly prolonged in demented patients, when compared to the control group, with no significant difference between groups with DAT and VD. CRP correlated negatively with age and positively with Mini Mental State Examination results, assessing the degree of mental deterioration. Prolonged CRP was noted in patients with vast lesions shown in CT. No relationship between CRP and EEG changes was found. CRP is a sensitive parameter, which may contribute to electrophysiological assessment of cerebral function.
